This system is designed for a 3 - bedroom residence and a 1 -bedroom ADU. Sewage will flow via gravity to a 1500 gallon 2 -compartment septic tank. A minimum of 1 foot of topsoil shall cover the Eljen modules. The aggregate and sand material shall be placed in an over -excavated pit to create a minimum of 4 feet of separation between the STA and groundwater. Two wells are located on site that will be within 100 feet of the new STA and shall be properly abandoned. A third well shall be a minimum of 100 feet to the STA. The water line must be encased where it is within 10 feet of the sewer line. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. H January 8, 2013 Jacky Noons jacky�a 3menmovers.com Onsite Wastewater Treatment System (OWTS) Installation Observations 262 Woods Road (Main House and future additional dwelling unit) Pitkin County, Colorado OWTS Construction Permit Number 0009.2013.pow2 Jacky, Project No. C1008 ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS) on November 15 and 22, 2013 for the subject property. Aspen Earthmoving installed the system. Multiple site visits were conducted with Aspen Earthmoving prior to the system installation to evaluate groundwater conditions impacting the absorption area. The existing tank was removed and existing drywell properly abandoned. There were also two wells on the north side of the house that were properly abandoned. Copies of the well abandonment reports are enclosed. The installation included a new 1500 -gallon, two-compartment Valley Precast concrete septic tank with an Orenco® Biotube Pump Vault with a PF 3005 pump in the second compartment of the septic tank. Anchors and "dead men" have been used to secure the septic tank due to high groundwater fluctuations. Pump start up was conducted by Valley Precast on January 7, 2014. The effluent filter in the Biotube pump vault must be cleaned annually or more frequently, if needed. The pump panel will be installed on the outside of the future additional dwelling unit, once constructed. Effluent is pressure distributed to an over -excavated, mounded Eljen® bed. The 24 -foot by 33 -foot bed consists of four rows of eight A42 Eljen® Modules for a total of 789 square feet of infiltrative area. The field area was over -excavated in preparation for a drywell installation when seasonally high groundwater infiltrated the area. The on-site, previously imported clay material was removed. Elevations for the field were bench -marked from the concrete slab at the existing shed (elevation of 100 -feet). Please see the attached field notes from Aspen Earthmoving for more detail. The excavation was filled with native, sandy backfill from an elevation of 95.1 -feet to 97.1 -feet. Filter fabric was placed on top of the sandy backfill to create a "cap" over the fluctuating groundwater conditions. 4 -feet of #67 Aggregate was placed from an elevation of 98 -feet to 101 -feet, followed by 1 -foot of concrete sand (sand specifications enclosed). The A42 Eljen® Modules were placed on the sand filter material, at an elevation of 102 -feet. This elevation is at approximate natural grade. Cover over the modules was mounded with a 3:1 slope. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The quality of the installation is dependent of the expertise of the installer, soil type, and weather conditions. Please call with questions. The Well Construction Contractor is responsible for notifying the owner of this requirement. 4/2012 Information and Standards Conditions of approval of a replacement well permit generally require that the old well be plugged and sealed according to the Water Well Construction Rules. A blank report form is provided to the well owner when a replacement permit is issued. This report is also used to affirm that other types of wells and holes have been plugged and sealed. These reports must be completed and submitted to the Division of Water Resources, usually within 90 days of plugging and sealing the well/hole. This form may be reproduced by photocopy or computer means. This design should replace the September 6, 2013 design and OWTS Construction Permit 0008.2013.powt2. An amended OWTS Construction Permit must be obtained from Pitkin County Environmental Health Department prior to installation of this system. SITE CONDITIONS A 3 -bedroom, single-family residence presently exists and is utilizing an existing OWTS. There is a desire to upgrade the existing OWTS. The existing components will either be removed or abandoned in-place. There are three wells on the property; two of which must be abandoned as these wells are located within 100 -feet of the drywell. One in the area of the proposed absorption area must be located and abandoned. The other well is located to the north of the main residence, on the west side of the property, and must also be abandoned. A future 1 -bedroom ADU is proposed on the north portion of the property where there is currently a shed. SUBSURFACE The subsurface was investigated on June 29, 2012 by digging one profile pit and three percolation holes. Water remained in the perc holes, which were 32" to 36" in depth, after the pre-soak. According to the property owner, soil was imported from the Starwood subdivision 30 to 40 years ago, when the lot was being improved. This would explain the layer of clay at the depth of the perc holes. The materials encountered in the profile pit consisted of 2.5 -feet of sandy clay, with root zone in upper 1 -foot, underlain by medium stiff sandy clay, dark brown to 4 feet, underlain by sandy clay to clayey sand with rounded cobbles (maximum size of 2 -feet) and gravel to the maximum depth explored of 10 -feet. Backhoe refusal was encountered at 10 -feet. A gradation was performed below the clay layer (4.0 -feet) and the soil consisted of 12% gravel, 46% sand, and 42% fines. On August 12, 2013, the water level in the excavation had increased to approximately 3 to 4 -feet below natural grade. On August 26, 2013, we conducted another site visit and observed the water level in the excavation at 4 -feet below grade and water levels in the profile pit were at 7'8" below grade. We are estimating a high ground water level at 5 -feet below grade. We believe the level in the profile pit to be a more accurate representation of ground water levels. An average percolation rate of 30 MPI will be used to design the system. This rate is based on the native soils observed and tested, the imported materials to be used to fill in the over -excavated area, and the concrete sand to be used under the Eljen® modules. DESIGN SPECIFICATIONS The system will accommodate the existing 3 -bedroom residence and a future 1 -bedroom ADU. An average daily wastewater flow of 450 GPD will be used. The installation must include a 1500 -gallon, two-compartment septic tank with a Biotube® pump vault and a PF3005 Orenco® pump in the second compartment. Other tank and pump alternatives may be considered. Effluent will be pumped to a 24' x 33' over -excavated area. Native material from the area will not be used due to the variable clay layer present. Three-inch minus road base may be used to backfill the excavated area up to 4 -feet below the proposed absorption area. The pit will be filled with a minimum of 3 -feet #67 Coarse Aggregate above the road base. This material will fill the excavation to 1 -foot below native grade. Twelve inches of concrete sand will be placed on top of the aggregate, followed by four rows of six A42 Eljen® modules. The modules will be placed at the elevation of native grade. A minimum of 1 -foot of topsoil must cover the Eljen® modules, distribution pipes, and geotextile fabric. A minimum 4 -foot separation between groundwater and the bottom of the absorption area must be maintained. Effluent will be dosed through a 1.25 -inch diameter discharge assembly, to a 1.25 -inch diameter discharge pump line, to a 1.25 -inch diameter manifold and four 1.25 -inch diameter laterals. Laterals will have 3/16 -inch holes facing down, four feet on center. The 1.25 -inch diameter laterals will be placed in a 4 -inch diameter pipe with holes at 5 and 7 o'clock. A vacuum breaker may be placed at the high point of the pump line. A pressure test should be performed on the system prior to burial. LWI 161 Page 3 A water line must be run from the existing well, located south of the main residence, to the proposed ADU. This water line will cross the sewer line and either the water or sewer line must be encased within 10 -feet of the crossing. "Crossings or encroachments may be permitted provided that drinking water or wastewater pipe is encased for at least 10 feet on each side of the crossing. Pipe of Schedule 40 rating or better must be used that has a sufficient diameter to easily slide over and completely encase the pipeline. Ridged end caps of at least Schedule 40 rating must be glued or secured in a watertight fashion to the ends of the encasement pipe. There must be a hole of sufficient size to accommodate the pipe in the lowermost section of the ridged cap so that the conveyance pipe rests on the bottom of the encasement pipe. The area in which the pipe passes through the end caps must be sealed with an approved underground sealant compatible with the pipe used. " (Pitkin County OWTS Regulations) Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Amended Construction Permit provided by Pitkin County Environmental Health Department, and this design. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating absorption systems, and any other components needing maintenance. The surface of the drain field should be seeded upon completion. Vegetation is an important factor in drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo - fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or materials should be placed on backfilled drain field. Livestock should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a drain field. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes should be installed to allow pump lines to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain field due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is optimal. Groundwater was encountered at 7 -feet during excavation of the drywell. The excavation was left open for several more weeks and the water level in the excavation increased to approximately 3 to 4 -feet below natural grade. This was measured on August 12, 2013. The groundwater level in the excavation remained constant and another site visit was conducted on August 26, 2013. The water level in the excavation was slightly lower, and the water level in the original profile pit (accessible via a 4 -inch pipe) was at 7'8". We believe the water level in the open excavation is higher than usual because the excavation allowed a pathway for the water to flow freely and fill the open area. We think the water level in the profile pit is more indicative of actual groundwater levels. This design should replace the August 1, 2013 design and OWTS Construction Permit 0019.2013.powt2. An amended OWTS Construction Permit must be obtained from Pitkin County Environmental Health Department prior to installation of this system. SITE CONDITIONS A 3 -bedroom, single-family residence presently exists and is utilizing an existing OWTS. There is a desire to upgrade the existing OWTS. The existing components will either be removed or abandoned in-place. There are three wells on the property; two of which must be abandoned as these wells are located within 100 -feet of the drywell. One in the area of the proposed absorption area must be located and abandoned. The other well is located to the north of the main residence, on the west side of the property, and must also be abandoned. SUBSURFACE The subsurface was investigated on June 29, 2012 by digging one profile pit and three percolation holes. Water remained in the perc holes, which were 32" to 36" in depth, after the pre-soak. According to the E • Page 2 property owner, soil was imported from the Starwood subdivision 30 to 40 years ago, when the lot was being improved. This would explain the layer of clay at the depth of the perc holes. The materials encountered in the profile pit consisted of 2.5 -feet of sandy clay, with root zone in upper 1 -foot, underlain by medium stiff sandy clay, dark brown to 4 feet, underlain by sandy clay to clayey sand with rounded cobbles (maximum size of 2 -feet) and gravel to the maximum depth explored of 10 -feet. Backhoe refusal was encountered at 10 -feet. A gradation was performed below the clay layer (4.0 -feet) and the soil consisted of 12% gravel, 46% sand, and 42% fines. During the excavation of the drywell outlined in the design dated July 17, 2012, groundwater was discovered at 7 -feet and the clay layer mentioned above was observed as deep as 4 -feet below grade. On August 12, 2013, the water level in the excavation had increased to approximately 3 to 4 -feet below natural grade. On August 26, 2013, another we conducted another site visit and observed the water level in the exaction at 4 -feet below grade and water levels in the profile pit were at 7'8". We are estimating a high ground water level at 5 -feet below grade. We believe the level in the profile pit to be a more accurate representation of ground water levels. DESIGN SPECIFICATIONS The OWTS design will be modified based on soil conditions. The system will accommodate the existing 3 -bedroom residence. An average daily wastewater flow of 450 GPD will be used. The installation must include a minimum 1000 -gallon, two-compartment septic tank with a Biotube® pump vault and a PF3005 Orenco® pump in the second compartment. Other tank and pump alternatives may be considered. Effluent will be pumped to a 24' x 25' over -excavated area. Native material from the area will not be used due to the variable clay layer present. The pit will be filled with #67 Coarse Aggregate to create a 4 -foot separation between groundwater and the bottom of the absorption area. This material will fill the excavation to 1 -foot below native grade. 12 -inches of concrete sand will be placed on top of the aggregate, followed by four rows of six A42 Eljen® modules. The modules will be placed at the elevation of native grade. A minimum of 1 -foot of topsoil must cover the Eljen® modules, distribution pipes, and geotextile fabric. Effluent will be dosed through a 1.5 -inch diameter discharge assembly, to a 1.5 -inch diameter discharge pump line, to a 1.25 -inch diameter manifold and four 1.25 -inch diameter laterals. Laterals will have 3/16 -inch holes facing down, four feet on center. The 1.25 -inch diameter laterals will be placed in a 4 -inch diameter pipe with holes at 5 and 7 o'clock. A vacuum breaker may be placed at the high point of the pump line. A pressure test should be performed on the system prior to burial. Page 3 Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Amended Construction Permit provided by Pitkin County Environmental Health Department, and this design. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating absorption systems, and any other components needing maintenance. The surface of the drain field should be seeded upon completion. Vegetation is an important factor in drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo - fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or materials should be placed on backfilled drain field. Livestock should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a drain field. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes should be installed to allow pump lines to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain field due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is optimal. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. Groundwater was encountered at 7 -feet during excavation of the drywell. This design should replace the July 17, 2012 design and OWTS Construction Permit 0019.2013.powt. An amended OWTS Construction Permit must be obtained from Pitkin County Environmental Health Department prior to installation of this system. SITE CONDITIONS A 3 -bedroom, single-family residence presently exists and is utilizing an existing OWTS. There is a desire to upgrade the existing OWTS. The existing components will either be removed or abandoned in-place. There are three wells on the property; two of which must be abandoned as these wells are located within 100 -feet of the drywell. One in the area of the proposed absorption area must be located and abandoned. The other well is located to the north of the main residence, on the west side of the property, and must also be abandoned. SUBSURFACE The subsurface was investigated on June 29, 2012 by digging one profile pit and three percolation holes. Water remained in the perc holes, which were 32" to 36" in depth, after the pre-soak. According to the property owner, soil was imported from the Starwood subdivision 30 to 40 years ago, when the lot was being improved. This would explain the layer of clay at the depth of the perc holes. The materials encountered in the profile pit consisted of 2.5 feet of sandy clay, with root zone in upper 1 foot, underlain by medium stiff sandy clay, dark brown to 4 feet, underlain by sandy clay to clayey sand with rounded cobbles (maximum size of 2 feet) and gravel to the maximum depth explored of 10 feet. Bedrock, or backhoe refusal, was encountered at 10 feet. A gradation was performed below the clay layer (4.0 -feet) and the soil consisted of 12% gravel, 46% sand, and 42% fines. C Page 2 During the excavation of the drywell outlined in the design dated July 17, 2012, groundwater was discovered at 7 -feet and the clay layer mentioned above was observed as deep as 4 -feet below grade. DESIGN SPECIFICATIONS The OWTS design will be modified based on soil conditions. The system will accommodate the existing 3 -bedroom residence. An average daily wastewater flow of 450 GPD will be used. The installation must include a minimum 1000 -gallon, two-compartment septic tank with a Biotube® pump vault and a PF3005 Orenco® pump in the second compartment. Other tank and pump alternatives may be considered. Effluent will be pumped to a 24' x 25' over -excavated area. Native material from the area will not be used due to the variable clay layer present. The pit will be filled with #67 Coarse Aggregate to create a 4 -foot separation between groundwater and the bottom of the absorption area. 12 -inches of concrete sand will be placed on top of the aggregate, followed by four rows of six A42 Eljen® modules. Effluent will be dosed through a 1.5 -inch discharge assembly, to a 1.5 -inch pump line, to a 1.25 -inch manifold and four 1.25 -inch laterals. Laterals will have 3/16 -inch holes facing down, four feet on center. The 1.25 -inch laterals will be placed in a 4 -inch pipe with holes at 5 and 7 o'clock. A vacuum breaker may be placed at the high point of the pump line. A pressure test should be performed on the system prior to burial. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating absorption systems, and any other components needing maintenance. The surface of the drain field should be seeded upon completion. Vegetation is an important factor in drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo - fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or materials should be placed on backfilled drain field. Livestock should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. E Page 3 For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a drain field. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes should be installed to allow pump lines to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain field due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is optimal. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be according to the county regulations. Requirements not specified in this report must follow applicable county regulations. The property is located outside of Aspen, in an area where OWTS and wells are necessary. SITE CONDITIONS A 3 -bedroom, single-family residence presently exists and is utilizing an existing OWTS. There is a desire to upgrade the existing OWTS. There are three wells on the property; two of which must be abandoned as these wells are located within 100 -feet of the drywell. One in the area of the proposed drywell must be located and abandoned. The other well is located to the north of the main residence, on the west side of the property, and must also be abandoned. SUBSURFACE The subsurface was investigated by digging one profile pit and three percolation holes, as indicated. Water remained in the perc holes, which were 32" to 36" in depth, after the pre-soak. According to the property owner, soil was imported from the Starwood subdivision 30 to 40 years ago, when the lot was being improved. This would explain the layer of clay at the depth of the perc holes. The materials encountered in the profile pit consisted of 2.5 feet of sandy clay, with root zone in upper 1 foot, underlain by medium stiff sandy clay, dark brown to 4 feet, underlain by sandy clay to clayey sand with rounded cobbles (maximum size of 2 feet) and gravel to the maximum depth explored of 10 feet. Bedrock, or backhoe refusal, was encountered at 10 feet. A gradation was performed below the clay layer (4.0 -feet) and the soil consisted of 12% gravel, 46% sand, and 42% fines. A long term acceptance rate (LTAR) of 0.72 gal/sq. ft will be used to design the OWTS. Page 2 DESIGN SPECIFICATIONS The OWTS design is based on 3 -bedrooms. An average daily wastewater flow of 450 GPD will be used. The installation must include a 1000 -gallon, three -compartment Valley Precast septic tank with a .5 MicroFAST Treatment system in the second compartment and a discharge pump in the third compartment. Other tank and secondary treatment alternatives may be considered. Effluent will be pumped to a 22' x 22' drywell with two 5 -foot concrete perforated risers. The drywell must extend at least 5.5 -feet below the inlet pipe. The inlet pipe must enter the drywell no more than 0.5 -feet below grade. The drywell will consist of clean, graded rock ranging from '/2" to 2 '/2" in diameter. The rock will extend from the bottom of the well to at least 2" above the inlet pipe. The sidewalls of the drywell will be scarified. A covering of geotextile fabric will be placed over the aggregate before the drywell is covered with soil. The backfill material shall be graded to deflect precipitation and surface drainage. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. OPERATION INFORMATION AND MAINTENANCE The surface of the drain field should be seeded upon completion. Vegetation is an important factor in drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo - fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or materials should be placed on backfilled drain field. Livestock should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a drain field. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes should be installed to allow pump lines to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain field due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is optimal. Page 3 INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be according to the county regulations.
