CONTRACT RENEWAL LETTER

According to Paragraph #10 of contract FAACPD 960228, Routing Number 960228 (copy attached and by this
reference made a part hereof) between the State of Colorado, Department of Public Health and Environment and
Pitkin Board of County Commissioners, the State will cause to be paid to the Contractor the sum of twenty-eight
thousand, four hundred and forty-one ($28,441) to be reimbursed monthly based on a sum equal to 1/12 of the
contract amount upon receipt of signed affidavits or expenditures beginning July 1, 1998 and ending June 30,
1999.
Payment pursuant to this contract will be made as earned, in whole or in part, from available State funds
encumbered in an amount not to exceed two hundred thirty-four thousand, three hundred and forty-five dollars
($234,345) for the purchase of County Environmental Health Services. This amount was determined using the
1999 Long Bill appropriation under Local Health Services for Public Health Sanitarians in areas not served by
local and regional health departments. The terms and conditions of the original contract shall remain the same.
This letter of amendment is intended to be effective July 1, 1998, but in no event shall it be deemed valid until
it shall have been approved by the State Controller or such assistant as he may designate.
Please sign all four (4) copies of this letter and return to Julie Akin at the address below by June 30, 1998.

1997 Long Bill appropriation under Local Health Services for Public Health Sanitarians in areas not served by
local and regional health departments. The terms and conditions of the original contract shall remain the same.
This letter of amendment is intended to be effective July I, 1996, but in no event shall it be deemed valid until
it shall have been approved by the State Controller or such assistant as he may designate.
