Changes must be approved by this Department and the engineer prior to construction. This permit if for the replacement of a failing, 2000 gallon concrete tank which is part of an existing and approved OWTS, permit #06057. The failed 2000 gallon tank shall be properly abandoned and pumped and filled or crushed. The new septic tank is a 1500 gallon, 2 -chamber, plastic tank. It be installed in a different location than the existing failed tank and it shall be installed a minimum of 50 feet from all water ways, springs, and creeks. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE. The installation included a 1500 -gallon, two-compartment poly Roth septic tank with a Biotube pump vault in the second chamber of the tank. Effluent is pumped to the existing field, previously permitted by Pitkin County Environmental Health Department, Number 06057. The discharge tank that was part of the original system installation was found to be leaking and was abandoned in-place. The pump was rewired and tested as part of the installation observation. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS. LIMITS: The observations were based on information submitted. If conditions are different from conditions described in this letter, ALL SERVICE septic, LLC should be notified. Any additional OWTS construction must be according to the county regulations. After several unsuccessful efforts to seal the tank, the existing tank will be abandoned in place and a new tank placed into service. SITE CONDITIONS There are two creeks in the vicinity of the system. The new tank must be placed a minimum of 50 - feet from any creek or water way. DESIGN SPECIFICATIONS The new tank will be a 1500 -gallon, two compartment plastic tank, either Roth or Infiltrator. The existing Biotube pump vault will be relocated into the new tank. The new tank will dose to the existing absorption area. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components.
