This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This system will consist of one 1500 gallon two-compartment tank with an Biotube pump vault in the
second compartment. A 2" line will pump effluent to the distribution box and then to 4 trenches, each with
15 Quick 4 gravelless chamber units in each for a total of 60 chambers. There must be at least 6' of
undisturbed soil between each trench. The distribution box must be have a baffle, should be leveled with
water at the time of final inspection to be sure effluent is being distributed evenly, and be accessible from
finished grade.
This Permit must be kept on-site curing installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby
given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved by EH/NR before final approval of the system will be issued.

ALL SERVICE septic, LLC performed installation observations for the onsite wastewater treatment
system (OWTS) on June 23rd and December 7th 2010 for the subject property.
The installation included a 1500 -gallon, two-compartment, concrete septic tank with a Biotube Pump
Vault, followed by gravelless chamber trenches. There were 4 trenches, each with 21 `Quick -4'
chambers. The number of proposed chambers was increased due to not having the required spacing
between trenches during construction. A total of 84 chambers, equaling 829 SF of area were installed.
A distribution box, brought to grade, was installed to distribute effluent to the drain field.
The control panel, pump, and alarm were installed and was tested on December 7th. The pumping
system was wired and working and actively dosing the drain field. Drain back was observed.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWTS.
LIMITS:
The observation was based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWTS construction must be according to the county regulations. The installer should have
documented and demonstrated knowledge of the requirements and regulations of the county in which
they are working.

Subsurface Evaluation and Revised Onsite Wastewater Treatment System Design, 4 -Bed Residence

ALL SERVICE septic, LLC evaluated the subsurface conditions prepared by CTL Thompson, Pro No
GSO4478-130 and prepared a revised onsite wastewater treatment system (OWTS) design for the
subject residence. The revision is based on a change in the total number of bedrooms. The property is
located outside of Woody Creek, in an area where OWTS are necessary.
SITE CONDITIONS
A 4 -bedroom residence is proposed as indicated. The proposed drain field area is relatively flat and is
located approximately 100 feet to the northeast of the proposed residence. The proposed drain field
area and surrounding area has been set aside by Pitkin County to serve the wastewater needs for the
subdivision. Water is provided by the local water district.
SUBSURFACE
The subsurface was investigated by CTL Thompson, under Pro No GSO4478-130. The subsurface
materials consisted of silty clay to sandy clay with gravel and cobbles. Groundwater or bedrock was not
encountered. Percolation tests were conducted and a 20 minutes per inch (MPI) rate will be used to
design the OWTS.
DESIGN SPECIFICATIONS
The OWTS design is based on 4 -bedrooms and an average sewage load of 600 GPD. The installation
must include a 1500 -gallon, two-compartment, precast concrete septic tank with a Biotube pump
vault, followed by a gravelless chamber trench drain field. The trenches must have 4 rows of 15
chambers for a total of 60 `Quick -4' chambers, equaling 593 square feet (SF) of infiltrative area. A
distribution box with a baffle must be installed. The distribution box must have a riser to surface for
access.
Construction must be according to the Pitkin County Environmental Health and Natural Resources
Department OWTS regulations, the OWTS Construction Permit provided by Pitkin County
Environmental Health and Natural Resources Department, and this design.

OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should be placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS.
For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWS. The OWTS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWTS during construction. The OWTS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.

This letter presents the results of our percolation testing for Filings 2 and 4 at
Lowe W/J in Pitkin County, Colorado. Our scope of services for the project were
outlined in our Proposal No. GS 05-133. The following sections describe our
percolation test method and present percolation test results.
Percolation 'rest Method
The individual sewage disposal systems (ISDS) for lots in Filings 2 and 4 at
Lowe W/J will be grouped together. Each of the twelve lots has been provided with a
1/2 acre parcel for the ISDS (see Figure 1). Our experience at the site indicates
subsurface conditions are generally similar throughout Filings 2 and 4. Therefore,
because of the close proximity of the ISDS locations, we judged percolation testing
could be reduced to less than the typical, three pits per ISDS parcel. From a
geotechnical engineering perspective, one percolation test per ISDS parcel is
appropriate to estimate the design percolation rate.for all twelve of the ISDS systems
in Filings 2 and 4.
We discussed our proposed approach to percolation testing with Ms. Nancy
MacKenzie of the Pitkin County Department of Environmental Health. Ms. MacKenzie
agreed that one percolation pit in each of the ISDS parcels would likely be sufficient,
provided actual conditions exposed in the pits were consistent across the site.
Based on our conversation, we also decided to limit the number of profile pits at the
site to two.
Our staff engineer directed excavation of the profile and percolation pits on
April 20, 2005. One percolation pit was excavated with a backhoe at the center of
each of the twelve ISDS parcels. The centers of the ISDS parcels were staked by
Schmueser Gordon Meyer, Inc., prior to excavation operations. Two profile pits

(Profile 1 and Profile 2) were excavated at the approximate locations shown on Figure
1. Profile pits were backfilled immediately after subsurface conditions were logged.
Subsoils at the site generally consist of gravel, cobbles and boulders in a silty to
clayey matrix. Graphic logs of the soil encountered in the profile and percolation pits
are shown on Figure 2.
Percolation Test Results
The percolation pits were prepared for testing on April 21, 2005. Our staff
engineer performed percolation testing in the pits on April 22, 2005. Inconsistent
percolation test results occurred in pits P-1, P-3 and P-8. Thesethree percolation pits
were cleaned out with a backhoe on May 11, 2005 and the pits were prepared for
retesting. The retests were performed by our field representative on May 12, 2005.
Test results are presented on Figures 3 through 6. Results from P-1, P-3 and P-8 are
from the retests. Our percolation test results indicate that a percolation rate of about
20 min/inch should be used for design of ISDS systems in Filings 2 and 4. The
absorption zone for the ISDS on each parcel should be in the silty to clayey gravel
soils.

Re: Lot 69 Water Service
Dear Mr. Tefft:
In follow up to your request, Lot 69 is in the W/J PUD and is within the W/J Metro
District. In the BOCC approval dated June 93, 2006, up to a 3,000 square foot unit was
approved. This represents one EOR of domestic service from the District. The original
service plan for the District also provided for up to 2,500 square feet of irrigation. The
District can and will serve Lot 69 under these terms. Please provide a house plan and
landscape plan for review. The tap fee of $2,000.00 must be paid and meter installed
per District regulations prior to service.
The WJJ District wiil also be responsible for operation of the OWTS system once it is
installed. The system must be constructed to District specifications and is subject to
District review prior to installation.
Based on the above terms, the W/J District can provide service to Lot 69

1.12 Liabili . Any person violating any of the provisions of these rules and regulations shall be
liable to the board for any expense, loss or damage to the District occasioned by such
violation, including the District's reasonable costs and attorneys' fees to require adherence
to thes rules and regulations or to collect the penalties, fees, tolls and charges due
hereon er.
1.13 District Standards and Specifications. The Standards and Specifications of the W/J
Metropolitan District are incorporated herein as an integral part of these Rules and
Regulations. In the event any rule, regulation, or standard, adopted by the District conflicts
with another rule, regulation, of standard, the District in its sole discretion shall determine
which rule, regulation, or standard shall control.
1.14 Coopgative Agreement. The District has entered into a cooperative agreement with the
W/J Homeowners Association. This agreement calls for the District and HOA to cooperate
on matters that affect both entities, such as damage to common facilities, roads, signs,
water systems, open space, et cetera, and to withhold deposits or other fees paid to ensure
compliance with covenants, guidelines, rules and regulations of both entities.
SECTION II. SERVICE POLICY
2.1 Eligibility of Service. All properties situated inside the corporate boundaries of the District
as of its date of incorporation shall be eligible to receive water, and other services provided
by the District, upon compliance with these rules and the payment of all required fees.
2.2 Inclusion of Proaerty into the District. Property not eligible for service by the District may
petition the District Board for inclusion into the District, however, the decision on whether
to include any property lies solely within the discretion of the District's Elected Board of
Directors. Landowners wishing to include property into the District must petition the
District for inclusion of such property in strict compliance with Colorado Revised Statutes,
Sections 32-1-401 and 32-1-402 complete, and all administrative rules of the District as of
the date of petition for inclusion. The Petitioner must pay all fees and costs associated with
the inclusion and agree to be bound by all District Rules, Regulations, Standards and
Specifications.
2.3 Exclusion of Property from the District. Fee owners of property within the District may
petition the Board for exclusion of such property in accordance with Colorado Revised
Statutes, Section 32-1-501.
2.4 Systempevelopment Charge (Tap Feel The right to take and use water distributed through
or have collected by District facilities shall exist only under license and before any tap or
connection is made to the District's water mains. The right to build and use an ISDS shall
also exist only under similar license. Written permits, therefore, shall be obtained from the
District and all required fees shall be paid. For owners building new homes on
undeveloped property, there is an initial tap fee, or system development charge, of
$2000/EQR *for non free-market properties and $6000IEQR * for free-market properties.
This fed shall be collected at closing when any person purchases a parcel of undeveloped

proper from Lowe W1J, LLC. Tap fees may be used for any purpose authorized by the
Board. Tap fees do not include any other fees or expenses related to connecting water
lines or building ISDS, including all inspection fees. (*One EQR = 3000 square feet of
grossAoor space excluding garages.)
All pro erties within the District which were already developed and receiving water
service from Lowe W/J, LLC, the owner of the water system prior to the transfer of'the
Water stem to the District, are automatically licensed and not charged this tap fee.
However, if and when any property owner's ISDS fails and has to be replaced old or new,
said pr�perty owner shall pay this tap fee prior to replacement. Except for this fee,
however, the replacement cost of the ISDS will be the responsibility of the District.
Applieotion for water tap permits or road access shall be made to the District on forms
furnished by the District which shall give a full description of the work to be done, the
address of the property or unit seeking to connect, the name of the licensed & approved
contractor to perform the work under the permit, and such other information as may be
required by the individual permit. Any permit may be revoked if the installation or use
covered by the permit is not in accordance with requirements of these regulations or any
prescribed standard and specification of the District or any governing rule of the Board.
The Licensee shall have the right to use water only on the licensed premises and for the
purposes specified in the license, subject to the modification, suspension or revocation of
such license as provided herein.
Licenses attach to the licensed premises only. Licenses are not affected by changes in
ownership of the licensed premises. Licenses can not be transferred from one premises to
another without the prior written consent of the District.
2.4.1 Accessing District Facilities. No District water main or other district facility shall
be tapped into or connected to without first having received a tap permit as
described in Section Il, paragraph 2.4, above, and second, having scheduled a
tapping or connection date and time with the District's manager. All taps and
Connections to District facilities shall be made in accordance with the District's
Standards & Specifications, Appendix II. No tap made to any District main shall be
buried prior to inspection by a District Representative. Taps to District facilities
shall only be made by District approved contractors or District Staff. See Appendix
2.4.2 Sewer or Water Service and Main Access Easements. Any landscaping
improvements shall be precluded from easements that are expressly designated and
anted upon which an access platform has been constructed, and which route is
ecessary to serve as the only assess to District facilities. Examples of such
improvements may include but are not limited to trees, berms, bushes, rock walls,
d any landscaping or improvements that would inhibit the District's access to and
long the access easement. Any landscaping in these areas will be the responsibility
f the homeowner to install, maintain, or replace.
