NOTICE
Special Con itions of !Issue: When properly signed for issuance, this application
/ becomes your permit. Application valid one year from
date. If an individual sewage disposal permit is issued
for property on which no building permit has been
issued, the individual sewage disposal permit shall
expire 120 days after its issuance if construction has
not been commenced. Any change in plans or speci-
fications after the permit has been issued invalidates
the permit, unless approval is secured from the Health
Officer for such changes.

Dear Mr. Rossito:
Investigation of the individual sewage disposal system serving
your residence conducted on March 13, 1979, revealed an apparent
broken tank outlet, causing sewage effluent discharge upon the
surface of the ground. This is in violation of provisions 3.01
and 3.20 F of the Pitkin County Individual Sewage Disposal
Regulations.
Please consider this letter an an official notice to make the
appropriate repairs to comply with regulations and abate the
nuisance and hazard to public health. We remind you that all
repairs must be done under a permit issued by this office and
that this permit must be obtained before any construction is
commenced.
Please contact this office at 925-4366.within two :)usiness days
after receipt to make appropriate arrangements as required under
Section 3.15 of the regulation. Your cooperation and prompt attention
to this matter will be appreciated.
