The in- stallation Was in accordance with our design dated July 22, 1997 and the Pitkin County ISDS Permit 97042. The OWS is for a 5 bedroom residence. The only change is a pump was required to access the upper trenches to maintain the distance to the well due to the separation distance between trenches. The chamber trenches were installed as indicated on the attached Figure 1. A Hydro -Matic OSP 33 pump was installed in the lift sta- tion. The lift station was a 1000 gallon two compartment septic tank. The pump was installed in the second chamber of the new tank. The effluent line to the existing trenches were disconnected.
