The fee for a ISDS application is $600 for a permit that fakes 6 hours or less for the department to approve. if approval takes
longer than 6 [lours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any; will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain
tiles, irrigation ditches, lakes, water courses, stream's, floodplains, dry gulches, and existing septic systems. 1 hereby certify
that any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of
any other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all
approvals and permits have been obtained.
The Pitkin County Environmental alth Department; Pitkin County and employees of these agencies will be held harmless

This letter is to advise you that the Pitkin County Environmental Health & Natural Resources
Department has not been able to final the Individual Sewage Disposal System (ISDS) permit for
the above stated residence/property.
It is mandatory that this office receives an "As Built" letter from the engineer who designed the
system. The letter will confirm that they observed the installation of the system as designed.
Any changes to the system must be noted in the letter or by a new set of plans. We cannot sign
the Certificate of Occupancy for this property without the "As Built" letter.
This office must receive the "As Built" letter from the engineer no later than June 7, 2006.

OWS design was designed in our letter of February 24, 2006.
The upgraded OWS included the installation of a 1000 -gallon, two-compartment septic tank. The
outlet piping discharged to the sewer line up -gradient- of the concrete distribution box. The locations
of the tank and piping are indicated on Figure 1.
The components of the OWS were installed in general conformance with our plans and specifications.
Our installation observations do not imply a guarantee or warranty of materials or workmanship.

Septic permit approved per compliance with the engineer design and specifications dated February 24, 2006. Any
changes must be approved by this department and the design engineer prior to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations.
The design calls for one 1000 gallon two compartment tank with an effluent filter on the outlet. The line will
line coming from the main house just before the distribution box that goes into the absorption field that sery

This department does not endorse any brand of products. This permit must be kept on-site during instal

" Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly
encouraged.

As requested, CHURCH OWC, LLC has prepared an OWS design for the proposed Shop/Storage
building to be located at the subject residence. CHURCH & associates, Inc prepared the OWS design
for the site under Job No. 16576 dated November 10, 2004.
PROPOSED CONSTRUCTION
A shop/storage building is proposed. It will have a half -bathroom with typical fixtures. The building will
be used by the homeowner. Sewage and effluent flows greater than that presented in the CHURCH &
Associates design noted above are not anticipated. If the proposed construction is different than that
presented herein,. CHURCH OWC should be contacted to revise this design, if necessary.
RECOMMENDATIONS
The OWS should be installed as indicated on Figures 1 and 2. The OWS includes one septic tank with
an effluent filter. The effluent pipe should be connected to the existing effluent pipe as indicated on
Figure 1.
OPERATION AND MAINTENANCE
The owner muss: realize an OWS is different from public sewer service. The owner must assume
responsibility for maintenance of the system. The system is relatively maintenance free, but the owner
must have the septic tanks pumped and filters cleaned. It is recommended the tank be pumped and
filter cleaned every two years. There are daily considerations, such as not putting plastic or other non -
biodegradable material into the system. Water use must be monitored so toilets are not allowed to run
when seals malfunction. To illustrate the point, a running toilet will consume in excess of 1000 gallons
per day if allowed to flow. An excess 1000 GPD loading could flood and irreparably harm the field.
We caution against installation of a water softener. The hydraulic loading from the backwash of a
water softener can be detrimental to OWS and a separate drywell should be constructed for the
backwash waste, if a water softener is installed. Chemically treated water from a swimming pool or
spas should not be introduced into the OWS.

LIMITATIONS
If the health department makes modifications to this design, we should be contacted to evaluate the
impact on the performance of our OWS design. All construction is to be in accordance with the ISDS
regulations. Pipe type and size, burial requirements, septic tank construction, and other specifications,
which are not depicted in our report, are to conform to the ISDS regulations. The installer of the
system is to have demonstrated knowledge of the ISDS regulations and requirements.
