If approval takes longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second, before final approval of the ISDS permit. Individual Sewage Disposal System No. 05080. The construction of the on-site ISDS has been completed pursuant to plans and specifications prepared by Schmueser Gordon Meyer, Inc. with a revision date of June 8th, 2005. The ISDS design at the Marolt residence, on Lot 32 in Filing 2 of W/J Ranch, was configured as 7 row 11 chamber infiltrator seepage beds fed by treated wastewater routed through a septic tank totaling 2,250 gallons in capacity. An Orenco low head effluent pump and vault were part of the original design of the septic tanks but this was omitted on site because of shorter distance to convey the effluent in comparison to the other W/J Ranch Filing 2 & 4 septic systems where the pump is a critical component. SGM has inspected the installation of the septic tank, collection system, flow splitter and gravelless chamber seepage beds pursuant to our original plans and specifications. Our inspection during the construction process indicated that the system was installed in a manner consistent with our planset with no substantive revisions to the original design. We have project photographs of the work in progress at the time of the W/J project inspections. At the last site visit, the septic tanks were in place and the PVC piping was installed, the distribution box had been leveled and the infiltrator beds were complete. We have observed the beds to be level and showing the appropriate number of chambers per our design. I trust this letter will provide adequate certification of the installation of the Marolt residence on-site wastewater disposal system pursuant to plans and specifications prepared by our firm. I understand that Environmental Health has agreed to issue me a septic permit for a "tank only" permit. The septic tank will be plugged and used as a vault until the leach field has been installed. By doing this the Environmental Health department will be able to okay the issuance of the building permit for my lot. I further understand that I will NOT be able to obtain a Certificate of Occupancy (CO) for my house until the complete septic system has been installed (tanks, sewer pipes, leach field). Our scope of services and methods for the project were outlined in our task agreement dated July 26, 2004. The following sections describe our percolation test method and subsurface conditions, and present percolation test results. Percolation Test Method The ISDS systems at the site are planned as dry wells with soil absorption depths between about 12 and 16 feet deep. We considered performing percolation tests for the project in borings and pits. Our experience indicated the soils at the site consist of it high percentage cobbles and boulders. Therefore, drilling percolation borings with standard, solid -stem auger techniques would have resulted in refusal above the planned absorption depths. Other drilling methods that can penetrate the cobbles and boulders are cost -prohibitive for the project. Alternatively, excavating safe percolation pits to depths of 16 feet would have been problematic. We discussed potential percolation test methods with Ms. Nancy MacKenzie of the Pitkin County Department of Environmental Health. It was agreed that CTL I Thompson„ Inc. would excavate profile pits to about 19 or 20 feet deep. The percolation pits adjacent to each profile pit would then be excavated to depths to perform percolation testing in the various soil types exposed in the profile pit. The experience of CTL I Thompson, Inc. indicated that this approach would allow safe percolation pits of 3 to 5 feet deep because the soils would likely be similar between © the ground surface and 20 feet deep. 234 Center Drive I Glenwood Springs, Colorado 81601 ielephone:970-945-2809 Fax: 970-945-7411 c Subsurface Conditions Our project engineer, Mr. James Kellogg, chose the locations of profile and percolation pits shown on Figure 1. We discussed the number and locations of pits with Ms. MacKenzie to check that Pitkin County requirements would be satisfied. The pit locations were then staked in the field by Schmueser Gordon Meyer, Inc. On August 5, 2004, the profile pits were excavated at the staked locations by Meldor Construction. Mr. Kellogg observed subsurface conditions exposed in the profile pits and determined appropriate depths for the percolation pits. Graphic logs of the soils encountered in the profile and percolation pits are shown on Figures 2 through 4. Test Results The percolation pits were prepared for testing by our field representative on August 5, 2004 after excavation operations were completed. Our representative performed percolation testing in the pits on August 6, 2004. Test results are presented on Figures 5 through 16 and summarized on the table below. Our percolation test results indicate that the design percolation for the ISDS at this site rate should be between 15 and 30 min/inch. A� Maintenance Instructions 061 0`rdstems 1-800-348-9843 4" Biotube® Effluent Filter How to Clean Your Effluent Filter To ensure your effluent filter is functioning properly, it should be inspected every year. Under normal conditions, your effluent filter will function for several years before cleaning is necessary. The filter should be cleaned when it becomes clogged enough to restrict normal flows out of the septic tank. At a minimum, the filter should be cleaned whenever the tank is pumped. Most people prefer 'to have a septic tank service provider take care of filter maintenance and cleaning. You can find a septic tank service provider in the Yellow Pages, under "Septic Tanks & Systems." Or you can contact your county health department for a list. If you wish to inspect and/or clean your effluent filter yourself, be sure to dress properly. Wear full-length pants and shirt, shoes, gloves, and goggles or glasses. Then follow these instructions: Remove the access lid to your septic tank by unscrew- ing the stainless steel lid bolts with hex head wrench provided. If your lid is above ground, it will be easy to find. If it is buried below ground, find the marker that indicates its location. 2. Remove the filter cartridge by grasping the tee handle and lifting it out of its housing (see photo 1). 3. Spray the cartridge tubes with a hose to remove any material sticking to them (see photo 2). Ensure the three orifices in the optional flow modulation plate inside the filter are clear of any debris. Make sure the rinse water runs back into the tank, but do not allow solids material to fall into the open filter housing. 4. Firmly place the cartridge back into the housing. 5. Some effluent filters come with an alarm that activates when the filter needs cleaning. If you have an alarm, check to make sure it is working by lifting the float with a stick. An audible horn should sound. The alarm panel is normally mounted on the side of the house or in the garage. Note: If your effluent filter doesn't have an alarm system and you would like one, call your local septic system installer. 6. Record the date that you inspected and/or cleaned your filter on the form that follows. If you checked the alarm or made any other observations about the tank or system, include that information under "Notes." 7. Attach access lid by placing it on the riser, matching the openings in -the lid with the bolt catches. Insert lid bolts into catches and tighten with hex head wrench provided. Photo 1. If unsure of the installation requirements for a particular site, contact the local health department. Like conventional systems, the soil and site conditions must be approved prior to installation. Conduct a thorough site evaluation to determine the proper sizing and siting of the system before installation. Chambers require a 12 -inch minimum of compacted cover to support a wheel load rating of 16,000 lbs/axle or equivalent to an H-10 AASHTO load rating. ❑ Only drive across the trenches when necessary. Never drive down the length of the trenches. ❑ To avoid additional soil compaction, never drive heavy vehicles over the completed system. Excavating and Preparing the Site Note: As is the case with conventional systems, do not install the systems in wet conditions or in overly moist soils, as this causes machinery to smear the soil. 1. Stake out the location of all trenches and lines. Set the elevations of the tank, pipe, and trench bottom. 2. Install sedimentation and erosion control measures. Temporary drainage swales/berms may be installed to protect the site during rainfall events. 3. Excavate and level 3 -foot wide trenches with proper center -to -center separation. Verify that the trenches are level or have the prescribed slope. Page 2 LF 4. Rake the bottom and sides if smearing has occurred while excavating. Remove any large stones and other debris. Do not use the bucket teeth to rake the trench bottom. Note: Raking to eliminate smearing is not necessary in sandy soils. In fine textured soils (silts and clays), avoid walking in the trench to prevent compaction and loss of soil structure. 5. Verify that each trench is level using a level, transit, or laser. Attaching the PosiLock'" End Plates 1. With a hole -saw, cut an opening for the inlet pipe using one of the pre -marked circles on the end plate as a guide. Pre -marked circles allow for 4 -inch SDR35, 4 -inch SCH40 and 2 -inch pressure dosing pipe. 2. Attach the end plate to the inlet end of the chamber by lining up the locking hubs with the corresponding chamber end. Apply firm pressure to lock the hubs in place on one side of the chamber and then the other. Note: The end plate is clearly marked "INLET SIDE TOWARD CHAMBER". 3. In the inlet end of the end plate, insert the appropriate - diameter inlet pipe into the previously drilled hole. Fasten the pipe in place with a screw to secure it to the end plate. Note: The end plate is designed so that effluent will flow in through the pipe and corresponding inlet hole and spill out the opening on the other side. When inserting the inlet pipe, the pipe will only extend into the plate one inch before it reaches a stop. 4. Attach a closed end plate onto the outlet end of the chamber by snapping the end plate's locking hubs onto the chamber end. Do not cut an opening on the closed or outlet end plate. Note: The existing opening on the end plate must face outward when installed on the closed/outlet end of the chamber. The opening is designed to fill with soil during system installation. CJ CO Installing the Chambers 1. Level the header pipe 2. Set the invert at the appro- priate height from the bottom of the trench to the bottom of the inlet. Invert height will vary depending on which end plate is used. Precisely measure the invert on the end plate prior to setting the invert height. Note: For Standard cham- bers, set the pipe invert at 6% inches. When using High Capacity chambers, set the pipe invert at 10/, inches. 3. Place the first chamber with the open end plate at the beginning of the trench. 4. Insert the inlet pipe into the end of the chamber. The pipe will only go into the unit one inch before it reaches a stop. 5. Check the first -installed C) chamber to be sure it is level or has the prescribed slope. 6. Secure the inlet pipe to the end plate with a screw at the 12 o'clock position. G 7. Lift and place the end of the next chamber onto the previous one at a 450 angle. Line up the notches on the center end of the chamber and lower it to the ground to engage the interlocks. 8. Continue interlocking the chambers until the trench is com- pleted. The last chamber in the trench should have a closed end plate. As the chambers are installed, verify that they are level or have the prescribed slope. 9. To fill the sidewall area, start at the joints where one chamber interlocks with another, and pull soil from the sides of the trench with a shovel. Continue backfilling the area making sure the fill covers the louvers 1-2 inches. 10. Pack down the fill by walking along the edges of the trench and chambers. This step is important for assuring structural support. Only compact enough to match the nat- ural in-place density of the surrounding native soils. In wet conditions, silty or clay soil, do not walk in the sidewalls. 11. Proceed to the next trench and begin with step one. Installing Optional Inspection Ports 1. Using a hole saw or router, cut an opening in the pre -marked area located in the center of the chamber. The hole saw should match the size and type of pipe that is being installed. 2. Glue a 6 -inch long piece of PVC pipe into a 2 -inch pipe coupling. 3. Insert the 6 -inch piece of pipe into the opening at the top of the chamber so the coupling sits on top of the chamber. 4. Insert another piece of pipe into the coupling and cut it at or above grade. 5. Attach a cap or threaded cleanout assembly onto the protruding pipe. 6. A small valve -cover box may be used if the inspection port is below the desired grade. Note: Inspection ports may also be used for venting. Covering the System Before backfilling, the system must be inspected by a health officer or other official as required by state and local codes. Create an as -built drawing at this time for future records. 1. Backfill the trench by pushing fill material over the units. Keep a minimum of 12 inches of compacted cover over the chambers before driving over the system. Note: Do not drive over the system while backfilling in sand, since sand does not give adequate support. 2. It is best to mound several extra inches of soil over the finish grade to allow for settling. This also ensures that runoff water is diverted away from the system. After the system is covered, the site should be seeded or sodded to prevent erosion. Note: A minimum cover of 12 inches is required to attain H -10 -equivalent loading. If allowed by code, chambers can be installed with a minimum of 6 inches of cover in a non -traffic area. Note: If the system is for new home construction, it is impor- tant to leave marking stakes along the boundary of the system. This will notify contractors of the site location so they will not cross it with equipment or vehicles. Page 3 C . ca Infiltrator Systems, Inc. Limited Warranty 4D(a) The structural integrity of each chamber and end plate manufactured by Infiltrator (collectively referred to as 'Units'), when installed and operated in a new, residential leachfield of an onsite septic system in accordance with Infiltrator's installation instructions, is warranted to the original purchaser ('Holder) against defective materials and workmanship for one year from the date upon which a septic permit is issued for the septic system containing the Units; provided, however, that if a septic permit is not required for the septic system by applicable law, the one (1) year warranty period will begin upon the date that installation of the septic system commences. In order to exercise its warranty rights, Holder must notify Infiltrator in writing at its corporate headquarters in Old Saybrook, Connecticut within fifteen (15) days of the alleged defect. Infiltrator will supply replacement Units for those Units determined by Infiltrator to be defective and covered by this Limited Warranty. Infiltrator's liabil- ity specifically excludes the cost of removal and/or installation of the Units. (b) THE LIMITED WARRANTY AND REMEDIES IN SUBPARAGRAPH (a) ARE EXCLUSIVE. THERE ARE NO OTHER WARRANTIES WITH RESPECT TO THE UNITS, INCLUD- ING NO IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. Specifically excluded from Limited Warranty coverage are damage to the Units due to ordinary wear and tear, alteration, accident, misuse, abuse or neglect of the Units; the Units being subjected to vehicle traffic or other conditions which are not permitted by the installation instructions; failure to maintain the minimum ground covers set forth in the installation instruc- tions; the placement of improper materials into the system containing the Units; failure of the Units or the septic system due to improper siting, improper sizing, excessive water usage, improper grease disposal, or improper operation; or any other event not caused by Infiltrator. This Limited Warranty shall be void if the Holder fails to comply with all of the terms set forth in this Limited Warranty.Further, in no event shall Infiltrator be responsible for any loss or damage to the Holder, the Units, or any third party result- ing from installation or shipment, or from any product liability claims of Holder or any third party. For this Limited Warranty to apply, the Units must be installed in accordance with all site conditions required by state and local codes; all other applicable laws; and Infiltrator's installation instructions. (d) No representative of Infiltrator has the authority to change this Limited Warranty in any manner whatsoever, or to extend this Limited Warranty.
