Changes must be approved by this Department and the engineer prior to construction. This permit is for the replacement of a damaged tank with a 1500 gallon, two-compartment, septic tank with a Bio -tube pump in the second chamber. The tank must be installed level and have risers to grade. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE. Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. The residence currently has a septic tank that appears to be leaking and must be replaced. A 1500 -gallon, two-compartment septic tank, with an inlet tee and a BiotubeTM pump vault may be installed at the subject property. The Biotube pump vault may be removed from the existing tank and utilized in new construction. The two existing risers may be removed and utilized in new construction, although two 1 -foot riser extensions must be used to raise lids to current grade. One riser is 30" in diameter and one riser is 24" in diameter. The septic tank has a small rock wall surrounding tank to provide access. This rock wall should be re- built after placement of new tank. The septic tank should be pumped every two to three years, and the tees inspected. If required, an effluent filter should be placed on the outlet of the septic tank. The effluent filter should be cleaned as needed. LIMITS: The design is based on information submitted. If conditions on site are different from conditions assumed in report, ALL SERVICE septic, LLC should be notified. All OWS construction must be according to the county regulations. Requirements not specified in this report must follow county regulations. The installer should have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. This design for the subject property, noted above, is valid only in conjunction with the approved County Health Department permit. Please call with questions. The original septic tank experiences seasonal high ground water, and was reported to be leaking. On June 8th 2010 we observed a 1500 -gallon, two-compartment concrete Valley PrecastTM septic tank with a duplex BiotubeTM pump vault. There was an OrencoTM effluent filter in the pump vault. The tank was exposed during our observation. A water -tight septic tank and water -tight risers were installed to grade. A 2 -inch pure -core pump line was connected to direct effluent to the drain field. The pumps, alarm, and control panel were tested, and were working at the time of our observation. The septic tank was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials.
