Septic permit approved per compliance with the engineer design and specifications dated June 27, 2006 and letter
dated July 25, 2006. Any changes must be approved by this department and the design engineer prior to them
being made. Minimum horizontal distances between components of the system and physical features shall conform
to the Pitkin County ISDS regulations. Documentation that the well has been properly abandoned must be
submitted before final approval.
The existing 1,000 gallon tank can be used if the engineer documents that the tank has good integrity. A 1,000
gallon, two compartment concrete tank with an effluent pumping system is to be added. An automatic distributing
valve will pressure distribute to three new sections of 4 ft deep sand filter material, each section 15 ft X 40 ft,
placed 2 ft below the surface. The material from the existing field may be used on site or must be properly
disposed of at a landfill.
The sand must conform to the intent of Section 6.28.670 A. of the Pitkin County ISDS regulations or
conform to ISDS Policy 2005-001. An invoice to document the type of sand material must be submitted.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.
*Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly

SEPTIC PERMIT APPLICATIONS CANNOT BE ACCEPTED UNTIL ALL OF THE
DOCUMENTS AND INFORMATION BELOW HAVE BEEN RECIEVED.
Each box should be checked to indicate the following have been completed, or submitted as part of the
application. Only one copy of each document is necessary.
Completed application form.
Engineer design stamped by a Registered Professional Engineer (RPE) registered in the State of Colorado.
o Design must be based on the Pitkin County ISDS Regulation, Revised 2001. This design should be
on a site map (1" to 20' scale) that must include locations of all existing and proposed wells,
location of perc tests, contour intervals, all proposed and existing buildings, property lines, ditches,
water lines, sprinkler systems, springs, suction (irrigation) lines, drinking water cisterns, drain tiles,
all water courses, floodplains, floodways, dry gulches and existing septic systems. The map must
include all such features on neighboring properties within setbacks.
o An individual may provide ISDS engineering services for themselves provided the dwelling the ISDS
serves is his/her primary residence, but an Owner/Builder Acknowledgement must be signed at
submittal. See Pitkin County ISDS Regulation Article 4, Section 6.28.110.

time of soil testing.

'Fee. The ISDS Permit fee is $600; the flat fee covers the first 6 hours of processing. An hourly rate of $100
will be charged beyond the first 6 hours, up to $1,000. The "tank only" fee is $150. Please make check

A WAIVER IS AVAILABLE FOR SUBMITTING A BUILDING PERMIT APPLICATION PRIOR
TO OBTAINING AN INDIVIDUAL SEWAGE DISPOSAL SYSTEM PERMIT.

This letter presents a summary of the installation observations of the Onsite Wastewater System
(OWS) at the subject site. The Installation Observations were performed from May through June
2007. The installation was based on the design of June 27, 2006 and July 14, 2006,under Job No.
C244.
The OWS is a sand filter because of shallow bedrock. The existing drainfield installed in 1983 was
removed. The OWS includes the existing 1000 -gallon septic tank, a new 1000 -gallon tank with a
Biotube pump vault in the second chamber, a pump line to an automatic distributing valve (ADV) and
hence to three 600 square foot sections of drainfield. An As -Built is attached.
On May 30 and June 12, 2007 the site was visited to observe the excavation of the prior field and
observe the bottom of the excavation to confirm there were fractures in the bottom of the
excavation. A gradation of concrete sand was submitted by Walters Company, Inc. The proposed
concrete sand was suitable as sand filter material. It was requested that the two northern sections of
the field be constructed as 12X50 feet rather than 15X40 feet. The area is equivalent and the revised
configuration suitably fits the topography.
June 21, 2007 there were discussions with the installer regarding the pump installation, wiring and
field construction.
June 26 and 27, 2007 there was a conversation with Ted Davis with respect to the wiring and alarms.
Met onsite observed the pumps performing properly.
The components of the OWS appeared to be installed in general conformance with our plans and
specifications. Our installation observations do not imply a guarantee or warranty of materials or
workmanship.
A suggestion is to construct a cover or roof over the control panel. The cover would protect the panel
during the winter.

CHURCH Onsite Wastewater Consultants (COWC) is submitting additional information as request by you
from Pitkin County Environmental Health. The base design was on June 27, 2006 under Job No. 244. This
letter provides a 1" =20' Figure 2 and discussion of the existing septic tank.
As indicated above Location of Proposed OWS, Figure 2 is attached as in an 11X17 inch format.
The existing septic tank was observed during our site visit and no apparent problems were observed.
The tank was installed in 1982 and no problems are anticipated. At the time of construction the tank
will be further exposed and evaluated.
There is an existing well, which will have to be abandoned with the upgrade of the OWS. Prior to the
Pitkin County Environmental Health being able to finalize the OWS permit, the well must be
abandoned.
Our investigation, layout, design, and recommendations are based on data submitted. If conditions
different from those described in this report are encountered, COWC should be called to observe the

from Pitkin County Environmental Health. The base design was on June 27, 2006 under Job No. 244. This
letter provides a 1" =20' Figure 2 and discussion of the existing septic tank.
As indicated above Location of Proposed OWS, Figure 2 is attached as in an11X17 inch format.
The existing septic tank was observed during our site visit and no apparent problems were observed.
The tank was installed in 1982 and no problems are anticipated. At the time of construction the tank
will be further exposed and evaluated.
Our investigation, layout, design, and recommendations are based on data submitted. If conditions
considerably different from those described in this report are encountered, we should be called to
observe the conditions. If proposed construction is changed, we should be notified to evaluate the
effect of the changes on the wastewater system. All construction is to be in accordance with the

As requested, CHURCH Onsite Wastewater Consultants (COWC) has prepared an onsite wastewater
system (OWS) design for the subject site. The design was pr ared for Individual Sewage Disposal
System (ISDS) permit submittal to Pitkin County Health Department (PCHD).
SITE CONDITIONS
The site is an approximate 2 -acre lot located as indicated on Figure 1. The site is in a rural area
where an OWS is required. The site is served by public water, although there is a well along the entry
driveway to the northwest. The area of the proposed drain field is at the location of the existing field.
There is a relatively flat lawn and playground in the proposed and existing field area. The slope is
steep away from the proposed field area.
The ISDS was installed under Permit No. 82044 in 1983 for a 2 -bedroom home. The existing ISDS
includes a 1000 -gallon, 2 -compartment septic tank and a 2500 square foot (SF) lined Evapo-
transpiration (ET) field. Three hand dug pits were excavated into the ET field and not ponding
effluent was observed. The line was observed and found to be a 10 mil liner, which appeared to be
cracked and broken.
PROPOSED CONSTRUCTION
A three-bedroom, single-family residence exists (although the 1982 ISDS Permit indicates 2 bed-
rooms), which is to be upgraded to a four bedroom residence. The residence will have less than 6000
SF as proposed. In accordance with the ISDS regulations, the sewage load for a four-bedroom
residence with between 2000 and 6000 SF is 800 gallons per day (GPD). The peak sewage loading is
1,400 GPD using a 1.75 peaking factor. A setback of 132 feet is required between the well and field.
It is proposed to construction the upgraded ISDS after completion of most of the addition because of
limited construction access at the site. The status of the well may become an issue. It is understood
that the well is for irrigation only. The well met the requirements of the 1982 permit, but will not
meet current setback requirements.
SUBSURFACE CONDITIONS
Subsurface conditions were investigated by hand digging percolation holes to the east and down
slope of the existing field. The percolation holes were excavated into weathered fracture shale, typical

of the Brush Creek area. The results of the percolation tests indicate percolation rates of 27 to 41
minutes per inch (MPI).
RECOMMENDATIONS
The OWS should be installed as indicated on Figures 1 through 4. The OWS includes an additional
septic tank/dosing tank and pressurized sand filter. The sequence of construction is important. It is
proposed that the addition be constructed while the existing ISDS is used and then the upgrade ISDS
be installed. This will allow construction access over the old field and protection of the new ISDS from
construction traffic. If surfacing occurs atthe old field during construction, the septic tankwill have to
per pumped to restrict surfacing.
A 1000 -gallon, two-compartment precast concrete septic tank with an effluent pumping system must
be added. A manufacturer diagram of the proposed septic tank is presented on Figure 4.
It is recommended the removal of the existing field and any excavation be done with track
equipment. Vehicular movement over the field area should be kept at a minimum to avoid
compaction of the receiving soils. Dump trucks and rubber -tire backhoes are to be kept off of the
proposed field area.
COWC personnel must observe the bottom of the excavation to confirm suitable fractured and jointed
shale is exposed. Soft or plastic clay soil must be removed. Typical drain field details are presented
on Figure 3. The chambers are to be installed a maximum of 12 inches deep and will have to have a
minimum of 6 inches of cover.
The existing drain field is to be removed. Sand and topsoil for the field can be stockpiled for use as
cover at the proposed field. It is proposed to remove two feet of the existing field, then place 4 feet
of sansL it r material, distribution gravel and soil cover. The end result will be the group surface will
be 3 feet higher than the existing ground surface. If not all three feet of elevation is desired,
additional—excavation to a lower elevation is possible, but must be coordinated with COWC, the
contractor and architect. The new field will consist of three segments of pressurized sand mound.
Each segment of the field is 15 feet wide for a total width of 45 feet. Details of the field are indicated
on Figure 2 and 3.
The surface of the field should be seeded after installation of the system. A seed mix, such as ""Pas-
ture or Prairie" should be used. These mixes do not require irrigation and develop a growth 10 to 15
inches high. No automatic sprinkler system should be installed over the field. Vehicular traffic and
small livestock pastures should be kept off the drain field.
EFFLUENT PUMPING SYSTEM
The installation of an OSI Biotube® Pump System is recommended. Ed Church is a non -participating
shareholder of SCG Enterprises, Inc., which is a local distributor for Orenco Systems, Inc. The OWS
contractor can propose alternative products.
The proposed pumping system and substitutions must meet the following minimum specifications:
Pumps - The pump selected for the pump vault should have a minimum capacity of 30
gallons per minute, a minimum Total Dynamic Head of 19 feet. A minimum power rating of
1/2 -Horsepower (230 -volt) is recommended for the pumps. The pumps shall be UL and CSA
listed as an effluent pump. OSI model P30 High Head pump (230 -volt) is recommended.

Access Risers— Access risers that extend to final grade must be provided. The risers are to be
watertight and attached to the septic tanks such that a watertight seal is provided. Risers
should be rated for the appropriate load requirements.
DischargeAssemb/y— The pump discharge assembly (OSI cold weather version) shall have a
disconnect union accessible from the ground surface to allow for easy pump replacement.
The discharge assembly shall be plumbed with a weep hole (and air relief valve at the high
point near the ADV) to allow drainage between cycles to minimize freezing potential.
Float5witchAssemblies— The on/off and high water alarm float switches are to be accessible
for adjustment without removing the pumps or vault. Pump float switches should be set to
discharge approximately 100 to 150 gallons per dose to the drain field.
Electrical Sp/ice Box— The splice box shall be UL approved for wet locations. All wire splices
are to be completed with UL listed waterproof splice connectors.
Controls and Alarms— Controls and alarms shall be listed per UL 508. Controls are to include
a toggle switch for manual operation of the pumps; separate dedicated circuits for the pump
and the high water alarm; an audible and visual alarm; and an elapsed time meter. Panel is
to be NEMA 4X rated.
Location — The pump control panel is to be mounted within sight of the pump system. The
panel is to be located such that the alarm can be readily seen and heard (remote alarms to a
location within the building can be provided). The panels are to be mounted at convenient
heights to allow access and maintenance.
INSTALLATION OBSERVATIONS
The installation of the OWS is to be observed by the design engineer. These observations include
drain field construction observations. Other observations are prior to backfill. At those times, the
septic tank and pumping system are to be installed with risers on the tank, and inlet and outlet con-
nections properly bedded. The drain field shall have distribution pipes in place. A final observation is
required once the electrical connection has been made prior to placing the OWS into service. Our
office should be called at 720-898-3434 at least 48 hours in advance to observe the installation.
These observations, repeat observations, design revisions, staking of drain field or additional site visit
requirements are not included with this design and will be at an additional unit rate fee.
OPERATION AND PREVENTATIVE MAINTENANCE SCHEDULE
The goal of an operation and maintenance schedule is to observe the operation and perform minor
maintenance to the OWS to allow for proper, long term functioning of the system. We suggest an
operation and maintenance schedule or contract with a private maintenance company exist.
Septic tanks. The scum and sludge accumulation in the septic tanks should be monitored
yearly. Once the scum or sludge thickness reaches 25% of the chamber depth, the entire
chamber should be pumped. We anticipate a pumping frequency of 1 to 3 years at design
flows. An alternative is a regular pumping frequency of every year.
Filter and EftZuent Pumping System: Tl:e effluent filter at the septic tank discharge should be
cleaned (hosed off) at the time of pumping or as needed. The effluent pumps should be
checked semi-annually to ensure pumps are functioning properly. If the alarm sounds, the
pumps and floats should be checked and/or serviced immediately.

Drain Fie/d' The surface around the fields is to be observed monthly for signs of failure such
as lush vegetation growth or effluent ponding. Liquid levels in each field section are to be
observed through the observation pipes and levels recorded. Flow to each field is to be
controlled through the distribution valves and should be diverted if effluent is observed
ponding in any trench.
General. • System users must realize an onsite wastewater system is different from public
sewer service. There are daily considerations such as not putting plastic or other non -
biodegradable material into the system. Water use should be monitored so toilets are not
allowed to run when seals malfunction. Allowing fixtures to flow continuously to prevent water
lines from freezing is not acceptable. Although the proposed system can accommodate
variable flows, spreading water use over several hours and eliminating peak flows is
recommended. To illustrate the point, a malfunctioning toilet can consume in excess of 1000
GPD. Excessive daily loading could flood and irreparably harm the OWS.
We caution against installation of a water softener. The chemical and hydraulic loading from
the backwash of a water softener may be detrimental to the OWS and a separate drywell
should be constructed for the backwash waste, if a softener is installed. No landscaping or
plastic can be used over the field, which will reduce performance of the field. The design of
the OWS is based on the treatment of domestic sewage only. Swimming pool or spa water is
not to be discharged into the OWS.
The proposed system design is based on design flows in the report. Increased flows may
hydraulically or organically overload each respective OWS, causing premature failure.

I am presently looking at our septic field inspection on the 23rd at
3:30pm. It is my understanding that Ed Walters (septic contractor)
and will be present for the inspections and I believe a representa-
tive from Ed Church's office as well. I will. check on that. Ed
Walters will be bringing photos of the construction of the field that
he took at the given stages as requested by Ed Church.
