Re: Zwart Sewage Disposal System
Dear Clint:
A cncnT . P[TV W
ENVIRONMENTAL HEALTH DEPARTMENT
I have attached a copy of the septic application for the Zwart
property for your records to show that we have received the $150.00
fee.
As we discussed at the site visit on Tuesday, November 29, a new
septic system will be required for the additional 2 bedrooms. A new
percolation test will have to be performed and a profile hole dug
in the proposed area of the absorption fieldin the spring after
the ground has sufficiently thawed. Once the results are received
by this Department, the .system size and design criteria will be
forwarded to you along with a permit number.
The building plans have been released from this Department. Should
you have any questions, please feel free to contact me.
Sincerely,
N

Date November 30, 1994
Re Zwart Residence
Those Present: Betsy Kipp, Environmental Health Department; Bob Nelson, Nelson Environmental &
Wastewater Solutions, Ltd.; Clint Watkins, Theodore K Guy Associates PC
The following items were discussed:
ITEM 1: Septic System Expansion
A. There are concerns about the existing system's tank being located under the driveway,
or any part that may be under the driveway.
B. Betsy doesn't think there will be a problem with leaving the existing system in place.
C. A second system will be designed to service the three new bathrooms above the new
garage.
L _ D. The proximity of the old system to the new is such that the Perc. Test from the
., original will be used for designing the new system.
E. The design will be contingent upon a Perc. Test in the spring and a profile core
44 sampling.
The above represents our record of events noted and items discussed and will be relied upon as true
and correct by all parties unless notified within five working days of receipt.
