This is a re -design based on the new March 2008 OWTS Regulation. The design is for one 1500 gallon 2 -co ` partment
septic tank followed by a 1000 gallon 1 -compartment tank followed by a 500 gallon dosing tank with a dual alternating
effluent pumping systern. Pump One will direct effluent to the West drainfield and Pump Two will direct effluent to the
South drainfield for equal distribution. Each drainfield will have an automatic distributing valve and center feed to four
trenches each with suspended laterals with 24 Quick4 gravelless chambers in each trench. Each drainfield will have a
total of 96 chambers for a grand total of 192 chambers.

This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -build" letter to this Department
with in 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection
with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation.
This permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacture's recommendations and the requirements of the Pitkin County OWTS Regulation.
THIS PERMIT IS EXPRESSELY CONDITIONED UPON COMPLIANCE WITHY ALL REQUIREMENTS OF THE PITKIN COUNTY
OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given

Septic permit approved per compliance with the engineer design and specifications dated September 27, 2007, Project #
C560. Any changes must be approved by this department and the design engineer prior to them being made. Minimum
horizontal distances between components of the system and physical features shall conform to the Pitkin County ISDS
regulations. The existing system is to be properly abandoned.
Design is for two single compartment 2,000 gallon tanks in series followed by a 500 gallon dosing tank with a dual
alternating effluent pumping system in a Biotube pump vault. There are two equally sized drainfields identified as West and
East, each with 8 trenches with 13 chambers (104 units) for a total of 208 chambers.
This permit is expressly conditioned on the property owner providing for the regular inspection, cleaning and
maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations.
This department does not endorse any brand of products. This permit must be kept on-site during installation. The
engineer must do a final inspection of the installation and submit an as -built letter to this department. This department
must also be called for an inspection with a minimum of 48 hours advanced notice.
Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be

The fee for an ISDS application is $623 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $623 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.

As requested, CHURCH Onsite Wastewater Consultants, LLC (COWC) conducted installation
observations of the onsite wastewater system for the subject lot. The design was created by
COWC under job number C560 in a report dated September 27, 2007 and revised for the updated
Pitkin County OWTS Regulations on July 3, 2008.
At least four independent installation observations were performed by COWC personnel in July of
2008: July 2, July 11, July 16 and July 25.
The first site visit on July 2, 2008, two 2,000 gallon Copeland Concrete septic tanks were in place
and a Copeland Concrete pump tank was installed. At the time of this visit, there was a 2 -inch
pipe from the house to the tanks that was not identified, the wrong type of chambers were being
used and the installer was using an old design. The tanks were determined not to be level; the
first tank was a single compartment tank, not a 2 -compartment tank as specified. The source of
the 2 -inch pipe from the house and what it drains to the septic tank has not been identified at the
writing of this report.
On July 9, 2008, Jered, installation foreman contacted Mr. Bob Wright with COWC to request that
the lower drainfield be allowed to be angled in the middle to better fit the contours. Jered was
informed that a sketch of the proposed final configuration be faxed to COWC for approval.
The second installation observation visit occurred on July 11, 2008. During this visit, the tanks
were being watertightness tested in accordance with the Pitkin County ISDS regulations. The
COWC representative, Ms. Katie Kangas observed water draining from the mid -seam and called
this to the attention of the installation contractor's foreman. The tanks were noted to be level at
this observation.
The third installation observation on July 16, 2008 noted that risers were installed on the second
septic tank and dosing tank, but not on the first septic tank. PVC discharge piping was installed to
the chambered drainfields and through the ADV.

The last installation observation was conducted on July 25, 2008 by Mr. Bob Wright or COWC.
During this observation trip, the tanks had been sealed at the mid -seam, but no retest of the
tanks had been called for. Electrical conduit was run from the house to the riser splice box and
the first septic tank was not completely backfilled, nor did it have risers in place. Risers were not
sealed to the tank lid. The drainfield was backfilled and monitoring pipes were in place. The
flushing valve assemblies projected above the surface of the ground, however no finished,
grading, topsoil and seeding or landscaping was in place. COWC did not observe in the field how
the flushing valves have been finished and whether valve boxes have been installed over them.
The final configuration of the lower drainfield could not be verified.
COWC has received photos from Mr. Kevin McClure, construction manager for the project,
verifying that the flushing valve assemblies have been placed in vaults. In addition, an explanation
of the 2 -inch pipe into the initial septic tank has been provided indicating that this pipe is from a
floor drain, and not a water softener. A sketch indicating how the south drainfield was laid out
during installation has been provided and is attached in 8.5 X 11 format with this letter. While no
observation of the tanks watertightness retesting was called for, Mr. Bob Wright did see the repair
of the seam to both the outside and inside of the tanks with what appears to be a bitumastic
compound. Mr„ Kevin McClure and the installer indicated that they refilled the tanks, and that they
observed the tanks not to leak. The tanks are in use at this time.
LIMITATIONS
This letter furnishes information collected from observations made during 4 -site visits to the
subject property. COWC has observed installed components of the OWS as called for by the
installer; full time inspection of the construction was not provided. COWC does not have the
authority to control the installer's methods, protocols, schedule or the equipment that has been
selected by the installer and/or client. Neither the observations made nor the contents of this
letter imply any warrantee or guarantee of materials or workmanship. It is possible that
problems with the system may occur in the future due to installation procedures that were not
able to be observed by COWC.

As requested, CHURCH Onsite Wastewater Consultants, LLC reviewed existing conditions and
investigated subsurface conditions at the subject site and designed an upgrade of the onsite
wastewater system (OWS) for the residence, which is being remodeled. There are two prior ISDS
permits: 1986 Permit 86050 for the original construction and a Repair ISDS Permit 93040 in 1993.
Each permit was for a 5 bedroom residence.
SITE CONDITIONS
The property is located in a rural area in Pitkin County, where OWS are required. The proposed
drainfield areas have a range of slope from 10 to 12% to the west. Vegetation varies lush grasses
at the existing drainfield to scattered shrub oak good cover of native grasses at the proposed
second drainfield area. The residence is served water by community water system.
EXISTING CONSTRUCTION
The existing residence is a 6000 plus square foot (SF) structure and has 5 -bedrooms. It is
proposed to have 7`bedroomralions/day/person
'the remodeled residence. The flow rate for a residence with
greater than 6000 SF is 100- for an average daily flow of 1400 gallons per
day (GPD) and peak load of 2100 GPD with a 1.5 peaking factor.
The existing system consists of a 1500 -gallon 2 -compartment septic tank followed by a 1000
gallon 1 -compartment tank and a drainfield consisting of three 90 foot long ABS chambers
trenches as indicated on Figure 1. The 1000 gallon septic tank is a replacement tank from 1993.
The drainfield trenches have settled into the ground and are not suitable for including in the
upgrade design. Both tanks, as well as the chambers, distribution box and piping are to be
removed. The original drainfield was sized based on a percolation rate of 15 minutes per inch
(MPI).
There is an existing landscape irrigation system above the existing drainfield area, which should
be significantly reduced to allow proper function of the upgraded drainfield. Irrigation must be
removed from the area of the trenches and drip irrigation drainfield.
SUBSURFACE CONDITIONS
A subsurface investigation was investigated by excavating profile pits and percolation holes at the
two locations indicated on Figure 1. Percolation tests were performed by COWC personnel as
indicated on Tables 1 and 2. Subsurface materials encountered in the profile pits consisted of 1 to

1.5 feet of sandy clay topsoil underlain to 8 feet, the maximum depth explored, by red silty clay
with some sand and gravel. No ground water was encountered. In the area of the existing drain -
field, the soil was moist to very moist due to the drainfield and landscape irrigation to the east. To
the south in the area of the second portion of the proposed drainfield, the soil is dry to moist.
RECOMMENDATIONS
Based on site conditions, two new drainfields utilizing chamber technology are proposed for the
drainfield. The OWS design is based on an average sewage load of 1400 GPD and a percolation
rate of A0 MPI. A septic tank capacity of 2625 gallons and drainfield area of (1860 SF is required.
The proposed OWS includes: 1) the replacement of the existing septic tanks with one 1500 -gallon
single -compartment tank followed by a 1000 gallon tank, followed by a 500 -gallon dosing tank
with a dual alternating effluent pumping system and 2) two areas of chamber trench drainfield, at
the locations presented on Figure 2. The dosing tank, a 500 -gallon one -compartment tank, will
have a dual alternating effluent pump in a Biotube° pump vault, as indicated on Figure 3. Pump
One will direct effluent to the west chamber trench drainfield and Pump Two will direct effluent to
the south chamber trenches, as presented on Figure 4. The drainfield at the west will consist of
four trenches with 24 chambers are proposed with a center feed distribution in the area of the
existing drainfield for a total of 96 chambers. The south drainfield will have the same
configuration as the west drainfield. The total number of trenches for the two drainfields is 8 with
192 chambers, where 190 chambers are required. _%z- ;
The surface of the fields should be seeded after installation of the trenches. A native seed mix
should be sown at the drainfield to re -vegetate the surface. These mixes do not require irrigation
and develop a growth 10 to 15 inches high. No sprinkler system should be installed over the field.
INSTALLATION OBSERVATIONS
The installation of the OWS is to be observed by the design engineer. The observations will
include drainfield construction observations at the following installation events:
1. After all existing drainfield chambers and piping have been removed from the existing
drainfield, the drainfield has been leveled and prepared, and the existing area is ready to
receive the new chambers.
2. When the south drainfield trenches are excavated and ready for chamber installation.
3. When the tanks, building sewer, discharge piping, bedding, ADV and control panel are
installed, and all chambers for both fields are installed, but not backfilled.
A final observation is required once the electrical connection has been made prior to placing the
OWS into service. Our office should be called at 970-948-5803 at least 48 hours in advance to
observe the installation. The observations, repeat observations, design revisions, staking of field

unit rate fee.
OPERATION AND PREVENTATIVE MAINTENANCE SCHEDULE
The goal of an operation and maintenance schedule is to observe the operation and perform
minor maintenance to the OWS to allow long term functioning of the system. An operation and
maintenance schedule or contract with a private maintenance company is recommended.
Septic tanks: The scum and sludge accumulation in the septic tanks should be
monitored yearly. Once the scum or sludge thickness reaches 25% of the chamber
depth, the entire chamber should be pumped. COWC anticipates a pumping frequency
of 1 to 3 years at design flows. An alternative is a regular pumping frequency of every

year. Frequent use of a garbage grinder will increase the pumping requirement.
Filter and Effluent Pumping System: The effluent filter at the septic tank discharge
should be cleaned (hosed off) at the time of pumping or as needed. The effluent pumps
should be checked semi-annually to ensure pumps are functioning properly. If the alarm
sounds, the pumps and floats should be checked and/or serviced immediately.
Drainfield: The surface of the fields is to be observed monthly for signs of failure such
as lush vegetation growth or effluent ponding. Liquid levels in each field section are to
be observed through the observation pipes and levels recorded.
General: System users must realize an OWS is different from public sewer service. There
are daily considerations such as not putting plastic or other non -biodegradable material
into the system. Water use should be monitored so toilets are not allowed to run when
seals malfunction. Allowing fixtures to flow continuously to prevent water lines from
freezing is not acceptable. Although the proposed OWS can accommodate variable
flows, spreading water use over several hours and eliminating peak flows is recom-
mended. To illustrate the point, a malfunctioning toilet can consume in excess of 1000
GPD. Excessive daily loading could flood and irreparably harm the OWS.
COWC cautions against installation of a water softener. The chemical and hydraulic
loading from the backwash of a water softener may be detrimental to the OWS and a
separate drywell should be constructed for the backwash waste, if a softener is install-
ed. No landscaping or plastic sheeting can be used over the field, which will reduce
performance of the field. The design of the OWS is based on the treatment of domestic
sewage only. Swimming pool or spa water is not to be discharged into the OWS. The
proposed OWS design is based on design flows in the report. Increased flows may
hydraulically or organically overload each respective OWS, causing premature failure.
LIMITATIONS
A contractor who is experienced in its installation must install an OWS design. Our investigation,
layout, design and recommendations are based on data submitted. If subsurface conditions
different from those described in this report are encountered, we should be notified to evaluate
the change on the proposed OWS. If the health department modifies this design, we should be
contacted to evaluate the impact on the performance of the OWS. The county and our firm must
be contacted upon completion of the OWS
All construction is to be in accordance with the ISDS regulations. Pipe type and size, burial
requirements, septic tank construction, and other specifications, which are not depicted in our
report, are to conform to the requirements of the ISDS regulations. The installer of the system is
to have demonstrated knowledge of the PCEHD ISDS regulations and requirement -
