Comments
Septic permit approved per compliance with the engineer design and specifications dated, November 16, 2004.
Any changes must be approved by this department and the design engineer prior to them being made. Minimum
horizontal distances between components of the system and physical features shall conform to the Pitkin County
ISDS regulations.
The design is for one 2000 gallon single compartment concrete septic tank followed by one 2000 gallon concrete
two compartment tank with an effluent pump in a Biotube pump vault. The drain field is 9120 sq ft of drip irrigation,
twelve 8ftX 95ft sections.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.
*Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly

The fee for a ISDS application is $600 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.

AND PERTINENT PHYSICAL FEATURES
NOTE: The Department may require greater distances where warranted. For repair or upgrading of existing systems where the size of the lot prevents meeting these distances, the repaired
facility may not be closer to water supply components than the existing facility.
(1) Crossings from these distances may be permitted if the drinking water or wastewater pipe is encased for at least ten (10) feet on each side of the crossing or place where components are
closer than specified in the table. Pipe of Schedule 40 rating or better must be used, of sufficient diameter to easily slide over and completely encase the line. Ridged end caps of at least
Schedule 40 rating must be glued or secured in a watertight fashion to the ends of the encasement pipe. There must be a hole of sufficient size to accommodate the pipe in the lowermost
section of the ridged cap so that the conveyance pipe rests on the bottom of the encasement pipe. The area in which the pipe passes through the endcaps must be sealed with an
approved underground sealant compatible with the piping used.
(2) Add 8 feet additional distance for each 100 gallons per day of design flow over 1000 gallons per day as specified in the table, unless it can be demonstrated by an RPE or geologist that a
mechanical or natural barrier will prevent contamination.

We received your letter regarding the Individual Sewage Disposal System Permit
for Parcel ID # 2643-354-01-005, 150 Larson Drive, Starwood Subdivision. We
are aware that you need an "As Built" letter for our engineer. We have spoken
with Ed Church from Church OWC, LLC and he has scheduled the final septic
inspection for this site during the second week of March. Unfortunately, this is
past the deadline of February 20, 2006 that is mentioned in your letter. Please
be aware that we are working on this issue, and it will be taken care of as soon
as Mr. Church can do the final inspection.

As requested, CHURCH Onsite Wastewater Consultants, LLC (COWC) performed a site visit to observe
the installation of the onsite wastewater system (OWS) at the subject site. CHURCH & Associates,
Inc. prepared an OWS design report under Project No. 10778 dated May 16, 20041.
Three site visits were performed in July and August 2005 during installation of the field. The drip
irrigation fields were installed with a small excavator. The field was installed similar to the plan
presented on Figure 1.
A site visit was performed on March 9, 2006 to observe the pump system. At the time of the site visit,
the sewer pipe, septic tank with biotube effluent pump, were installed. The control panel alarm will
be under the stairs at the southwest side of the home, allowing visual contact with the pump.
The observed components of the OWS appeared to be installed in general conformance with the
CHURCH/InTEC OWS design, plans and specifications. Our installation observation does not imply a
guarantee or warranty of materials or workmanship. The OWS design figures have been revised to
reflect the final location of the OWS at the subject site.

As requested we have prepared a revised onsite wastewater system (OWS) design for the subject property.
The revisions are based on a change to the number of bedrooms from our latest revised design, dated May
30, 2002, under Job No. 10778. All previous designs should be discarded to avoid confusion. CHURCH &
Associates, Inc. (CHURCH) performed subsurface investigations on April 22, 1999 and April 16, 2002.
SITE CONDITIONS
The site is a 2.4 -acre lot located in Starwood Village, Pitkin County, Colorado. The lot is presently vacant.
The slope at the proposed field area is 40% to the southwest. The ground has a full cover of scrub oak and
small evergreens. Water is provided to the site by the local water district. The proposed drain field site is not
in a confined drainage area and has moderate solar and wind exposure. The site is suitable for use of a drip
irrigation or evapotranspiration -absorption (ETA) OWS.
PROPOSED CONSTRUCTION
A six -bedroom residence with a living area greater than 6000 ft2 is proposed at the location indicated on
Figures 1 and 2. The OWS was sized based on 6 bedrooms and 1560 gallons per day (GPD) using
Pitkin County Environmental Health Department (PC� ) loading of 130 GPD per person. The
sewage loading with a peaking factor of 1.75 is 2730 GPD. Loading includes washing mach es and
garbage grinders.
If additional bedrooms in unfinished areas are possible, it is suggested that the OWS be constructed to
handle the additional loading. The installation of a properly engineered OWS to serve future build -out can
be cost effective.

Subsurface conditions were investigated by drilling two profile borings and 15 percolation holes. Subsurface
materials encountered in the Profile -1 consisted of sandy, gravelly clay, with sparse large boulders, to the
maximum depth explored of 9 feet where refusal was encountered. Subsurface materials in Profile -2
consisted of 9 feet of sandy, gravelly clay, with sparse large boulders, underlain by sandstone bedrock to the
maximum depth explored of 10 feet. The percolation tests indicated rates of 20 to 120 minutes per inch.
The OWS design is based on the percolation rate of 71 NW1, which has a corresponding application rate of
0.30 gal ft /day. We are designing OWS based on percolation holes 1 through 6 and 12 through 15, which
are in the area of the proposed drain field. Percolation and subsurface investigation results are presented as
Table 1.
RECOMNWNDATIONS
The OWS should be installed as indicated on Figures 2 through 4. The OWS includes two septic tanks with
an effluent pump and a dosed shallow trench drip irrigation drain field. The OWS is designed on an
application rate of 0.30 gal/A2/day and a sewage loading of 1560 GPD. The OWS installation must include
one 2000 -gallon, single -compartment, concrete septic tank followed by one 2000 -gallon, two-compartment,
concrete septic tank with an effluent pump in a Biotube® pump vault in the second compartment. Pump
specifications are presented as Figure 5. The drain field installation must include 9120 ft2 of drip -irrigation
drain field in twelve 8 -foot by 95 -foot sections as presented on Figures 2 and 3. Configuration of drip
irrigation fields can be modified to fit the topography and preserve existing vegetation where
possible.
The trenches should be backfilled to reestablish the original slope. Grading and/or retaining structures may
be needed in drain field areas to achieve required drain field depths. If this occurs, the topsoil should be
removed prior to grading and replaced to promote revegetation. The surface of the drip irrigation field
should be continuously vegetated after installation of the system. Added hydraulic loading from an
automatic irrigation system may reduce the life of the drain field.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer service. The owner must assume responsi-
bility for maintenance of the system. The system is relatively maintenance free, but the owner must have the
septic tanks pumped and filters cleaned (hosed off). It is recommended the tank be pumped and filter
cleaned every two years. There are daily considerations, such as not putting plastic or other non -bio-
degradable material into the system. Water use must be monitored so toilets are not allowed to run when
seals malfunction. To illustrate the point, a running toilet will consume in excess of 1000 gallons per day if
allowed to flow. An excess 1000 GPD loading could flood and irreparably harm the field.
We caution against installation of a water softener. The loading from the backwash of a water softener can
be detrimental to OWS and a separate drywell should be constructed for the backwash waste, if a water
softener is installed. No plastic should be used over the field, which will reduce evapotranspiration from the
field. Chemically treated water from a swimming pool or spas should not be introduced into the OWS.
LBMATIONS
A drip irrigation design requires installation by a contractor who is an experienced installer. Our
investigation, layout, design and recommendations are based on data submitted. If subsurface conditions

® different from those described are encountered, we should evaluate the effect of the changes on the
proposed OWS. If the health department makes modifications to this design, CHURCH should be
contacted to evaluate the impact on the performance of our OWS design.
All construction is to be in accordance with the ISDS regulations. Pipe type and size, burial requirements,
septic tank construction, and other specifications, which are not depicted in our report, are to conform to
the ISDS regulations. The installer of the system is to have demonstrated knowledge of the ISDS
regulations and requirements.
