System Records: Permit #: 94038 Date of Issuance: 2/3/94 Date of Final Approval: 10/14/94 # of Bedrooms or fixtures served by OWTS: The system is designed to serve 3 bedrooms, the system serves a 2 bedrooms in the main house. Operational Status: According to the inspector's observations, the system was functioning as designed at the time of inspection. Tank lids were replaced because they had deteriorated. Pump chamber had some infiltration at the inlet pipe. Pump and alarm v✓ere tested and worked properly during inspection. There were no signs of failure in the soil treatment area. Inspector Recommendations: Electrical outlet supplying power to the pump should be made water tight, install an effluent filter to the outlet of the tank, seal inlet pipe to the pump chamber to prevent infiltration of water. Department Recommendations: N/A Issuance of this OWTS Use Permit is based solely on the conditions observed and reported by the inspector to the Department on the date of the inspection(s) and on Department records at the time of permitting. The issuance of this permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail or that the system will not be subject to future enforcement action to correct non-compliant conditions. Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible failure. Sopris Creek Road (1994 OWTS) Pitkin County, Colorado Mr. Wilke, As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit inspection at 7035 E. Sopris Creek Road, Snowmass, Colorado on June 30, 2015. The legal description of the property is Lot 2, Capitol and Elk Creek Subdivision. There are two residences on the property. The subject OWTS serves two bedrooms in the main residence. The permit and record drawing was provided by Pitkin County Environmental Health Department (Parcel ID #2645-032-02-002). The subject OWTS consists of one 1000 -gallon, two-compartment concrete septic tank followed by a concrete pump chamber (5 -feet in diameter, 8 -feet deep) with a pump. Effluent is dosed through a 2 -inch diameter pipe to a manifold and then to two gravelless chamber trenches, each consisting of four'Standard' Infiltrator® chambers. The septic tanks are located on the southwestern corner of the main house. Both manhole lids of the 1000 -gallon septic tank were accessible from grade. Both lids of the 1000 -gallon septic tank were in disrepair and were replaced by D&D Septic Services. Both the inlet and outlet tees were present. The outlet tee is angled badly at a downward angle. This could be an indication of settling. We typically recommend the installation of an effluent filter. The outlet tee would need to be adjusted and extended for the installation of a filter. If a filter were to be installed, this filter would need to be cleaned annually, or as needed. A cast iron lid covers access to the pump chamber. Accumulation of sludge measured approximately 12 - inches in the purnp chamber at the time of the inspection. Both the 1000 -gallon tank and the pump chamber were pumped by B&R Septic during the inspection. The concrete surrounding the inlet pipe appears to be deteriorating. A trickle of water was observed around the pipe during the inspection. We recommend efforts to minimize infiltration into the pump chamber by sealing the area between the concrete and pipe. Electrical connections for the pumps are made through a traditional electrical outlet. This outlet is not water -tight and is not recommended. If this pump were to be replaced in the future, we recommend it be replaced with an Orenco® pumping system, which would include a water -tight electrical splice box and control box allowing manual control of the pump and a visible and audible alarm. During the inspection, the pump was tested and was found to be functional. The alarm in the garage sounded when the high water float was lifted. The approximate STA had a covering of tall, native grasses. We walked the area and no inspection ports were observed. The area appeared to be dry with no signs of surface saturation or failure at the time of the inspection. This evaluation is not a guarantee of future system performance. This inspection is good for one year. Recommendations: • Install effluent filter on outlet tee of 1000 -gallon septic tank (will require extension and re- alignment of pipe). If installed, this filter will need to be cleaned annually, or as needed. • Seal around inlet pipe of pump chamber to eliminate the chance of infiltration into the pump chamber. • If pump is replaced in the future, replace with Orenco® pumping system with water -tight splice box for electrical connections. This report should be submitted to the Pitkin County Environmental Health Department. The following links are the required application and application checklist. Don't: • Pour cooking grease or oil down the sink or toilet. • Rinse coffee grounds into the sink. • Pour household chemicals down the sink or flush them. Don't: • Flush non -degradable products or chemicals, such as feminine hygiene products, condoms, dental floss, diapers, cigarette butts, cat litter, paper towels, pharmaceuticals. Don't: • Park or drive on your dra infield. The weight can damage the drain lines. • Plant trees or shrubs too close to your drainfield, roots can grow into your system and clog it. Don't • Concentrate your water use by using your dishwasher, shower, washing machine, and toilet at the same time. All that extra water can really strain your septic system. Do: • Eliminate or limit the use of a garbage disposal. • Properly dispose of coffee grounds & food. • Put grease in a container to harden before discarding in the trash. Do: • Dispose of these items in the trash can! Do: • Consult a septic service professional to advise you of the proper distance for planting trees and shrubs, depending on your septic tank location. Do: • Stagger the use of water -generating appliances. This can be helpful especially if your system has not been pumped in a long time. • Become more water efficien by fixing plumbing leaks and consider installing bathroom and kitchen faucet aerators and water -efficient products. x# LIGEND AND NOT{• `\` '\ O auNa IFIaJ•el N aaaCJlMe RAs r• ` • • ateaeY oL.NK \ T SO Pit.ua .JJI.Irwu RO AD e1NcUvi &.R`:iiPi », wla 8 i 772 ` 0 4.
