Effluent will flow via gravity to a new distribution box with flow equalizers and then to a new 5 trench STA. Each trench will have 15 low profile Quick -4 Chambers for a total of 75 Chambers (900 ft'). The tank shall be a minimum of 5 feet to the house. The STA shall be a minimum of 10 feet to the property line and 25 feet to the water line and 100 feet to any well. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Zamora Excavating, Inc. installed the system. A 1500 -gallon, two-compartment Copeland Concrete septic tank was installed under OWTS Construction Permit 0037.2016.POWT. The cast iron line from the house was replaced with PVC pipe and a double - sweep clean out was installed. An effluent filter was installed on the outlet tee of the septic tank. This filter must be cleaned annually, or as needed. The existing STA was abandoned in place. Effluent gravity flows from the septic tank to a distribution box, which is accessible from grade. Flow equalizers were installed on each pipe exiting the distribution box to assure equal distribution to each trench. The new soil treatment area (STA) consists of five rows of 15 `Quick 4' Low Profile Infiltrator® chambers for a total of 75 chambers and 900 square feet of infiltrative area. There were at least 6 -feet of undisturbed soil between each trench. Chambers were not installed deeper than 3 -feet below native grade. Inspection ports were installed at both the beginning and end of each trench. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The 5.2 -acre property is located outside of Carbondale, in an area where OWTSs are necessary. The legal description of the property is Lot 3, Prince Creek Village Tracts. Parcel ID: 2463-133-03-003 SITE CONDITIONS A 5 -bedroom, single-family residence presently exists. The existing septic tank was recently replaced under OWTS Construction Permit 0037.2016.POWT. The distribution pipe and soil treatment area (STA) were determined to also be inundated with roots by B&R Septic and Zamora Excavating, Inc. The existing STA will be abandoned in place. The new proposed STA slopes to the north at approximately 5% and is covered with native grasses. The residence is currently served by a community water system for the Prince Creek Subdivision. The water line enters the property from the east and is located greater than 25 -feet from any OWTS component. SUBSURFACE The subsurface was investigated on November 4, 2016 by digging two soil profile test pit excavations (Test Pits). A visual and tactile soil analysis was completed by Carla Ostberg at the time of excavation. The materials encountered in Test Pit #1 consisted of medium brown, topsoil to 1.5 -feet, underlain by tan, sandy loam (caliche) to 4.0 -feet, underlain by tan, sandy loam (caliche) with an increasing percentage of rock to a maximum depth explored of 8 -feet. No bedrock or groundwater was encountered. The materials encountered in Test Pit #2 consisted of medium brown, topsoil to 2.0 -feet, underlain by tan, sandy loam (caliche) to 6.0 -feet, underlain by tan, sandy loam (caliche) with an increasing percentage of rock to a maximum depth explored of 8 -feet. No bedrock or groundwater was encountered. �J 7] Page 2 A sample of the soil was taken from Test Pit #1 at approximately 2 -feet below grade. The sample had single -grain to blocky structure shape, moderate structure grade, and loose to friable consistence. The sample formed a ball and a ribbon less than 1 -inch before breaking. The texture of the soil was more gritty than smooth. The soil is classified as Soil Type 2, Sandy Loam. A long term acceptance rate (LTAR) of 0.6 gallons per square foot will be used to design the OWTS, in accordance with Pitkin County OWTS Regulation, Table 6.28-7. backfill Sieved sample • Page 3 DESIGN SPECIFICATIONS The OWTS design is based on 5 -bedrooms having less than 6000 square feet of living area. A design flow of 750 GPD will be used. The component manufacturers are typical of applications used by contractors and engineers in this area. Alternatives may be considered or recommended by contacting our office. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. PERMIT APPLICATION INSTRUCTIONS A completed Request for an Amendment to an OWTS Construction Permit must be submitted to Pitkin County Environmental Health Department, along with this OWTS Design Packet. W Page 4 REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo -fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA. If an effluent filter or screen has been installed in the OWTS, we recommend this filter or screen be cleaned annually, or as needed. If the OWTS consists of a pressurized pump system, we recommend the laterals be flushed annually, or as needed. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high water conditions and shall be connected to a control breaker separate from the high water alarm breaker and from any other control system circuits. The pump system shall have a switch so the pump can be manually operated. Excavation equipment must not drive in excavation of the STA due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3 -inch. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. Legal description of the 5.2 -acre property is Lot 3, Prince Creek Village Tracts Parcel ID: 2463-133-03-003 The permit and record drawing were provided by Pitkin County Environmental Health Department. The subject OWTS consists of a 1000 -gallon VCP concrete septic tank followed by an 18' x 54' pipe and gravel soil treatment area (STA). The system was permitted to accommodate "four or five" bedrooms; however, a letter dated May 3, 1978 from Evelyn Bowers may have allowed for a smaller septic tank. This OWTS is documented by Pitkin County Health Department Permit Number 78004. Final approval for the system installation is dated May 3, 1978. A record drawing was provided, and is enclosed. The residence is served by a community water system. B&R Septic was contacted to pump the septic tank, and upon pumping, significant root intrusion was discovered in the septic tank. We interviewed Roger Maynard from B&R Septic, and he noted that in addition to the root intrusion in the tank, the sewer line exiting the septic tank was also broken. Given the status of the plumbing exiting the septic tank, we were not able to test the STA to see if it is accepting water. The septic tank must be replaced. We recommend the sewer line from the house be inspected. If the sewer line material is cast iron or the sewer line is found to be in poor condition, this line should be replaced with minimum SDR 35 pipe at a minimum 2% fall from the house to the septic tank. A double -sweep clean out should also be installed within 5 -feet of the house foundation. We recommend the exiting septic tank be either removed or properly abandoned in place by pumping and crushing the sides and top and filling the void with soil or on-site materials. The new septic tank may be placed in a location near the existing septic tank. The septic tank must be a MINIMUM 1500 -gallon, two-compartment septic tank with an effluent filter on the outlet tee. The sewer line from the septic tank to the STA must be inspected. If broken or found to be in poor condition, this line must be replaced. The line must be uncovered all the way to the manifold of the STA for proper inspection. Approximately 100 -gallons of water must be run into the pipe leading to the STA to assure the STA is accepting water, or effluent.
