This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
ISDS Permit #98072 was sized to accommodate 6 potential bedrooms. The buildings that were once served by this system
have been demolished and have been replaced by a one -bedroom modular unit. All of the tanks have been abandoned with
the exception of a 1250 gallon tank which will be relocated. An accurate as -built of the new tank location must be submitted
after installation. The pump must be re -installed and tested by the engineer prior to use.
The tank must meet setbacks to all pertinent physical features.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby
given to the property owne,(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved by EH/NR before final approval of the system will be issued.

Onsite Wastewater System Septic Tank and Drain Field Report

As requested, ALL SERVICE septic, LLC performed an onsite wastewater system (OWS) tank
installation observation and a drain field evaluation on October Oh 2009 for the subject property. The
subject property is located in Pitkin County, outside of the town of Snowmass, Colorado.
The OWS was permitted by Pitkin County under permit #980720 The septic system originally served
a total of 6 -bedrooms. The existing structures, and septic tanks, have been demolished and/or
abandoned, and a 1 -bedroom modular unit has been placed at the subject site.
A 1250 -gallon, two-compartment, concrete septic tank with Biotube pump vault and control panel
has been installed to accommodate the 1 -bedroom modular unit. The tank and pump vault have been
installed properly, and according to Pitkin County. The pump and floats are wired to the control
panel.
The 1250 -gallon septic tank will dose the existing drain fields having a total effective area of 1260
SF. Eight observation ports were visible in the area of the drain field. All ports were checked and
were dry. The area of the drain fields has adequate vegetation, and the drain field areas showed
no signs of surfacing effluent, and had no indication of failure.
RECOMMEDATIONS AND/OR CONCLUSIONS
The newly installed septic tank and the existing drain field will serve the 1 -bedroom
modular unit.
2. The septic tank and effluent filter should be pumped and cleaned every three years, or as
needed.
This evaluation is not a guarantee of any future drain field performance, in any way. ALL
SERVICE septic, LLC evaluated the OWS, and has no further knowledge of use and/or performance,
and/or any changes.

As requested, ALL SERVICE septic, LLC reviewed the current site conditions and is providing
recommendations for continued OWTS use.
As we understand the property structures have been demolished and a 1 -bedroom unit having
approximately 400 SF will be brought to the property.
The OWTS was sized and permitted for 6 total bedrooms in 1998 under permit number 98072. The
current OWTS consists of three septic tanks (two with effluent filters) and two pressurized chamber
bed drain fields, as indicated on figure 2. A pump was present in the last tank of the series, although
recently removed.
The drain field is sized appropriately to serve a 1 -bedroom unit, although a pump must be installed in
the last compartment of the tank series. The pump should be capable of 20 GPM, and a total dynamic
head of 35 feet. A tank cross section is included.
LIMITS:
The recommendations are based on information submitted. If conditions encountered are different
from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS
construction must be according to the county regulations. Requirements not specified in this report
must follow county regulations. The installer should have documented and demonstrated knowledge
of the requirements and regulations of the county in which they are working.
