Changes must be approved by this Department and the engineer prior to construction. This system has failed and the field area needs to be replaced. The existing tank with RETROFAST unit will remain and flow to a new 18" external basin with effluent filter and then gravity flow to a level manifold to evenly distribute effluent to a gravelless chamber bed. The bed will be 5 "Quick -4" units wide by 8 long. Observation ports must be installed at each corner of the field. Field must be at least 10' from the property line. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Issuance of this permit does not imply compliance with Pitkin County building and/or land use regulations, nor guarantee issuance of building and/or land use permits. Mountain Works installed the system. The existing 1000 -gallon, two-compartment septic tank with the BioMicrobics0 RetroFAST remained in use. An Orenco0 external filter basin was installed outside of the outlet side of the septic tank. This filter must be cleaned annually, or as needed. Effluent gravity flows to a gravelless chamber bed, connected with a level manifold, with five rows of 8 `Quick 4' InfiltratorO chambers for a total of 40 chambers and 480 square feet of infiltrative area. Inspection ports were installed on each corner of the bed. We recommend an annual maintenance contract for service of the BioMicrobics0 RetroFAST unit; however, no reductions in infiltrative area square footage or setback distances were provided with the use of this unit, making use of this technology not a requirement or condition of the OWTS Construction Permit. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The property is located outside of Basalt, in an area where OWTSs are necessary. The legal description of the 0.68 -acre property is Lot 26, Holland Hills at Basalt, Tract: 55 Section: 17 Township: 8 Range: 86 Parcel of Land in Tract 55 Sec 17-8-86 BK 245 PG 198. SITE CONDITIONS A single-family residence presently exists. The existing OWTS, documented by Pitkin County Health Department Permit Number 76053 is experiencing problems. The pipe exiting the septic tank is clay and was extremely soft and almost entirely clogged. The status of the existing soil treatment area (STA) is not known; however, it is the desire of the homeowner to upgrade the STA at this time. Pitkin County OWTS Construction Permit 003.2012.pow2 documents the installation of a RetroFAST® added to the second compartment of the septic tank. The residence is served by a community water system. A ditch runs to the northeast of the property. This ditch is greater than 50 -feet from the proposed STA and has been lined. A gas line runs south of the area of the proposed STA. This line will need to be located and the STA placed in an area that will not interfere with this gas line. Source Gas must be contacted prior to work commencing on the STA. The area of the proposed STA slopes slightly to the southwest toward the Roaring Fork River and is located in an irrigated area of the yard. aMSV�,�y' aA}� } 7: Clay inlet pipe Page 2 Pipe exiting side of septic tank / clay / very soft and clogged SUBSURFACE Soils in Holland Hills have consistently been classified as Loamy Sand with percolation rates ranging from 10 to 30 minutes per inch (MPI). Permit 76053 documents the percolation rate (performed 8/16/76) at 15 MPI with a soil classification of "Red Sandy Loam and some Clay". At that time, no groundwater or bedrock was encountered within 8 -feet of grade. An 8 -foot deep profile pit must be excavated prior to installation of the STA to verify soil type. This office and Pitkin County Environmental Health Department must be contacted upon digging the test pit. A long term acceptance rate (LTAR) of 0.8 gallons per square foot will be used to design the OWTS, in accordance with Pitkin County OWTS Regulation, Table 6.28-7. DESIGN SPECIFICATIONS The OWTS design is based on 3 -bedrooms. A design flow of 450 GPD will be used. Design Calculations: 3 bedrooms x 75 gallons/person/day x 2 people/bedroom = 450 gallons/day Septic Tank Capacity = 450 gallons/day x 48 hour retention time = 900 gallons minimum STA Calculations = 450 gallons/day / 0.80 SF/gallon (Loamy Sand Type 1) = 565.5 SF 565.5 SF x 1.2 (gravity bed) x 0.7 (chambers) = 472.5 SF 472.5 SF / 12 SF/chamber = minimum 39 'Quick 4' Infiltrator® chambers We recommend exploring the sewer pipe from the house to the septic tank to assure that line is in good condition. If found to be in disrepair, this line should be replaced and a double sweep clean out installed within 5 -feet of the house, if one does not already exist. The clay inlet pipe should be replaced with PVC pipe. The existing septic tank will continue to be utilized. The BioMicrobics® RetroFAST may remain in the septic tank, if desired by the owner. An Orenco® external filter basin should be installed outside of the outlet side of the septic tank. Effluent will gravity flow to a gravelless chamber bed, connected with a level manifold, with five rows of 8 'Quick 4' Infiltrator® chambers for a total of 40 chambers and 480 square feet of infiltrative area. Inspection ports must be installed on each corner of the bed, and ports may be cut to grade and placed in a sprinkler box. Over -excavation of the existing STA will be necessary due to the location of the gas line. If materials encountered in the existing STA are saturated, they must be staged and dried before disposal at the landfill. If the existing STA material is found to be dry, it can remain and excavation of the new bed can Page 3 continue. Soil replacement may be necessary if the extent of the excavation is greater than 4 -feet. Onsite (sandy loam) soils or ASTM C33 concrete sand may be used as soil replacement. If other soils are imported, this office must be contacted for review and approval of the soil. The component manufacturers are typical of applications used by contractors and engineers in this area. Alternatives may be considered or recommended by contacting our office. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo -fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high water conditions and shall be connected to a control breaker separate from the high water alarm breaker and from any other control system circuits. The pump system shall have a switch so the pump can be manually operated. Excavation equipment must not drive in excavation of the STA due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3 -inch. Page 4 INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be according to the county regulations. Requirements not specified in this report must follow applicable county regulations.
