This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This permit is to replace an existing metal septic tank. The existing metal tank shall be properly abandoned by pumping and
crushing/filling/removing. The new septic tank shall be a 1250 -gallon two-compartment septic tank with an effluent filter
installed in the second chamber. The tank shall be a minimum of 10 feet to the property line and 50 feet to any well or stream.
Do not install the tank in the driveway unless it is a traffic -rated septic tank.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Issuance of this
permit does not imply compliance with Pitkin County building and/or land use regulations, nor guarantee issuance of building and/or land use permits.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby
given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved by EH before final approval of the system will be issued.

I went out to 388 Handy Drive today and they were in the process of setting the tank. Attached are photos from when I
was there. They are connecting the new tank to the existing PVC pipe that runs to the existing field.

Dear Kurt,
Thank -you for meeting with me the other day. It was very helpful and your office has much of the need information.
My daughter or son-in-law will be dropping off my application and check. I presume if you are not there, they will slide
under the door or leave at your door.
,Things that are forth coming is a report from Carla. She is working on that now.
I have asked our subdivision secretary to state that the subdivision provides water to me. I will just email that to you, if
that is okay. I have included a water bill from the subdivision in the meantime.
I think those are the only two items that are missing. Could you please keep me posted as to your progress such that I
can keep B & R, Carla, and Pifco in the timing loop.
Again, thank -you for all your help. It made a big difference in my comfort level with this project.

Onsite Wastewater Treatment System (OWTS) Absorption Area Inspection
388 Handy Drive

As requested, CBO Inc. performed an evaluation of the absorption area that serves the single-family
residence located at 388 Handy Drive, Carbondale, Colorado on March 31, 2014.
The permit and record drawing was provided by Pitkin County Environmental Health Department
(Parcel ID # 2463-133-01-004). The subject OWTS consists of one 1000 -gallon, metal aeration tank
followed by an 18' x 98' lined Evapo-transpiration (ET) Bed. The aeration device in the septic tank
has not been operational for many years.
The metal septic tank will be replaced with a 1250 -gallon, two compartment concrete septic tank.
PifCo (Matt Piffer) is scheduled to replace the septic tank in April, 2014. An effluent filter should be
installed on the outlet tee.
At the time of installation, this office should be contacted to inspect the line from the tank to the field
(to the extent it is visible).
According to Pitkin County records, the absorption area is a lined ET bed. No observation ports are
present. There is grass growing over the absorption area with a dog run and a children's play area.
There are no signs of saturation, and no indication of failure. Some snow cover was present on the
property, but the approximate location of the absorption area was free of snow. The hillside to the
north of the absorption area drops steeply toward Prince Creek. The hillside was closely inspected
and did not appear to have any evidence of erosion. The hillside is well vegetated with mature scrub
oak.
This evaluation is not a guarantee of future system performance; only that the system is not visibly
failing at this time. It is not possible to determine the condition of the absorption area without digging
into it, which may cause damage. It is also not possible to determine how long the absorption area
will last. We recommend allowing native grasses to grow over the field area and minimizing or
discontinuing irrigation. You may consider installing an aeration device in the septic tank. This may
enhance the life of your system.
