Changes must be approved by this Department and the engineer prior to construction. This is an amendment to permit # 0024.2015.POWT and that permit is now void. This new field area will replace the existing pipe and gravel bed designed for 3 bedrooms with a new bed system designed for 4 bedrooms. The existing septic tank will be abandoned -in-place and evidence of proper abandonment shall be submitted to this office as part of the final approval of this permit. Sewage will flow via gravity to a new, 1250 -gallon, two-compartment septic tank with an effluent filter installed in the second compartment. Effluent will flow via gravity to a distribution box and then a "split bed" with 2 soil treatment beds with a minimum of 6 feet of undisturbed soils between the beds. Each STA bed will have 4 rows of 9 Quick -4 chambers for a total of 72 chambers in both beds. The STA beds shall be a minimum of 10 feet to the property line, not in the area of the abandoned STA, and 20 feet to any water line. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Avalanche Excavation installed the system. The existing septic tank was removed and the existing soil treatment area (STA) abandoned in place. The existing cast. iron line from the house to the septic tank was sleeved and a clean out installed. The installation included a new 1250 -gallon, two-compartment Roth® septic tank with an Orenco® Biotube effluent filter on the outlet tee. This filter must be cleaned annually, or as needed. Effluent will gravity flows to a distribution box, which is accessible from grade via a concrete risers and manhole lid. Speed levelers were installed to assure equal distribution between the beds. Two gravelless chamber beds were installed, each with four rows of 9 `Quick 4' Infiltrator® chambers for a total of 72 chambers and 864 square feet of infiltrative area. At least 6 -feet of undisturbed soil was present between each bed. Inspection ports were installed at each corner of both beds. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the 01NTS in the future. Any additional OWTS construction must be according to the county regulations. 11111111 Observations are limited to components that are visible at the time of the inspection. The system design packet dated June 20, 2015 depicted the existing septic tank and soil treatment area (STA) in an incorrect location. The drawings have been modified to more accurately represent the existing system location; however, measurements represented on Pitkin County's drawing do not appear to be accurate when transposed to a more recent site plan. The exact location of the STA is unknown. All documents dated June 20, 2015 must be discarded. The property is located in Old Snowmass, in an area where OWTSs are necessary. The legal description of the property is as follows: Subdivision: LITTLE ELK CREEK VILLAGE Block: 1 Lot: 11 FILING 1. SITE CONDITIONS A 3 -bedroom single-family residence presently exists and a remodel is proposed, which will include a 4th bedroom. The existing OWTS is documented by Pitkin County Health Department Permit Number 77113 and consists of -a 1250 -gallon, two-compartment Copeland Concrete septic tank and a 17' x 40' pipe and gravel STA. The existing OWTS is currently in use. The existing septic tank and STA will be abandoned. The new proposed soil treatment area (STA) to the south of the driveway is relatively flat and is vegetated with native grasses and wild flowers. The residence is currently served by a community water system. No component of the new OWTS will come within 25 -feet of any water line. Page 2 SUBSURFACE The subsurface was investigated on June 17, 2015 by digging two soil profile test pit excavations (Test Pits). A visual and tactile soil analysis was completed by Carla Ostberg at the time of excavation.' The materials encountered in Test Pit #1 consisted of dark brown, moist, organic topsoil to 10 -inches, underlain by medium brown, moist, sandy clay loam with blocky structure shape, weak structure grade, and firm consistence to 3 -feet, underlain by medium brown, moist, sandy loam with granular structure shape, strong structure grade, and loose to friable consistence to a maximum depth explored of 7.0 -feet. No bedrock or groundwater was encountered. The materials encountered in Test Pit #2 consisted of dark brown, moist, organic topsoil to 18 -inches, underlain by medium brown, moist, gravelly sandy loam with granular structure shape, strong structure grade, and loose to friable consistence to a maximum depth explored of 8 -feet. No bedrock or groundwater was encountered. A 4 -inch perforated PVC pipe was placed in the Test Pit #2 and re- checked on July 1, 2015 and found to be dry. Soil conditions in Test Pit #2 are preferable as the sandy clay loam horizon was not present. The proposed STA will be in the area closer to Test Pit #2. According to Pitkin County Health Department Permit Number 77113, groundwater was encountered at 94 -inches on July 5, 1978. Installation of the OWTS should remain as shallow as possible, in the area of Profile Pit #2, in the Sandy Loam horizon. A long term acceptance rate (LTAR) of 0.6 gallons per square foot will be used to design the OWTS, in accordance with Pitkin County OWTS Regulation, Table 6.28-7. A design flow of 600 GPD will be used Design Calculations: 4 bedrooms x 75 gallons/person/day x 2 people/bedroom = 600 gallons/day Septic Tank Capacity = 600 gallons/day x 48 hour retention time = 1250 gallons minimum STA Calculations = 600 gallons/day / 0.60 SF/gallon (Sandy Loam Soil Type 2) = 1000 SF Gravity, Bed = 1000 SF x 1.2 = 1200 SF Chambers 1200 x 0.7 = 840 SF 840 SF / 12 SF/chamber = minimum 70 'Quick 4' Infiltrator® chambers The existing septic tank will be abandoned in place or removed. A new minimum 1250 -gallon, two- compartment concrete or plastic septic tank with an effluent screen on the outlet tee of the septic tank will be installed. Effluent will gravity flow to a distribution box, which must be accessible from grade. Flow equalizers, or other similar product, must be utilized to assure equal distribution between the beds. The location of the existing STA has been estimated based on drawings provided by Pitkin County. The existing STA will be abandoned in place. If any portion of the STA is encountered within the proposed STA, this material must be removed. The distribution box will distribute effluent to two graveless chamber beds, each with four rows of 9 'Quick 4' Infiltrator® chambers for a total of 72 chambers and 864 square feet of infiltrative area. There must be at least 6 -feet of undisturbed soil between each bed. The infiltrative area must be installed in the sandy loam horizon, no more than 4 -feet below native grade, with shallower installation preferable. Inspection ports must be installed at each corner of both beds. Inspection ports may be cut to grade and covered with a sprinkler or valve box for access from grade. The component manufacturers are typical of applications used by contractors and engineers in this area. Alternatives may be considered or recommended by contacting our office. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo -fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high water conditions and shall be connected to a control breaker separate from the high water alarm breaker and from any other control system circuits. The pump system shall have a switch so the pump can be manually operated. Excavation equipment must not drive in excavation of the STA due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3 -inch. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be according to the county regulations. Requirements not specified in this report must follow applicable county regulations.
