Mike has installed this elbow with screws, so that it could be removed for cleaning, if necessary. He has also drilled a hole in the top of the elbow, to help prevent a siphon developing. He has sent me a photo, showing the installation and hole. I have sent the photo on to you by email, and have made a hard copy for your files, which I enclose here. They would like to get the final permit issued as soon as possible. When you are able to send it out, would you also copy me please? If you have any questions, please call. The approximate depths to rock are shown on the drawing, and noted on some of the photos. Only the first sheet of the three -sheet set of plans has been revised to any real extent. The original (revised) second and third sheets will be adequate for the as -built plans for this project. This system was installed in substantial compliance to the approved (revised) design plans, and Pitkin County regulations. A note on the 90° Elbow in the distribution box, which was, I believe the only item remaining: the photograph, which shows the elbow, was taken by me, today. I also "installed" this elbow. I put this in quotes because the inlet pipe into the distribution box is sloping downwards (as it should) and was cut off at a non -90° angle, and I could not fully seat the elbow on the pipe. My preference would be for the elbow to be only a slip -fit on the pipe, or at most attached with a short stainless steel screw on the very top of the fitting. This was not possible, because of the downward angle of the pipe where it entered the distribution box as well as the angle at which the pipe was cut. rady/Patch ISDS — As -Built Plans and photos transmittal. 'ednesday, November 9, 2005 7ge 1 of 2 I am forwarding this letter to Mike Patch to have him resolve the issue. The distribution box is very accessible, in that it has been installed in a large irrigation valve box. With the proper tools, it would be the work of an hour at most to resolve this minor problem. It is mandatory that our office receives an "As Built" letter from the engineer who designed the system. The letter will confirm that they observed the installation of the system as designed. Any changes to the system must be noted in the letter or by a new set of plans. Included in the letter we will need a confirmation of the baffle in the D -box in either written form or picture form. The Building Department will not issue a Certificate of Occupancy for this property until they are notified that the ISDS permit process is complete. This office must receive the "As Built" letter from the engineer no later than November 17, 2005. Please contact me with any questions. I have revised the design for a three-bedroom residence. I have also made a few minor changes to the details, such as changing to a Tuf-Tite distribution box, over the concrete one, as I anticipate that the plastic will hold up better than the concrete, and comes with built-in gaskets, and levelers. The access is easier, and more secure also. I have shown the cut-off wall to extend around the entire lower boundary of the system. My on-site inspection of the prepared field surface indicates that the cut- off wall may not need to extend this far. On the north end of the field, there is plenty of room to accommodate a 4:1 fill slope. This may also prove to be true for a small segment of the western edge of the field. I next intend to inspect and photograph the system when the septic tank, and the Class 6 and the entire Infiltrator® Quick4 chambers are in place. Hopefully, the distribution box and piping will also be installed at that time. I will forward the photographs to you. If you have any questions, please call. Sincerely, Gamba & Associates, Inc. The soils test and system design must be done by a Registered Professional Engineer. You should plan to meet with the RPE at your site to discuss where the soils tests should be done to meet all of the County regulations and where you want to put your house. The RPE will be your contact for issues you want to discuss about your system's size and location. Each box should be checked to indicate the following have been submitted before the application can be accepted. X Environmental Health has been called to look at profile hole. X Completed application form. X Engineer design stamped by a RPE registered in the State of Colorado. (One set). Design must be based on the Pitkin County ISDS Regulation, Revised 2001. X Site map roust include locations of all existing and proposed wells within setback distances (including on surrounding properties), contour intervals, all proposed and existing buildings, property lines, ditches, slopes of greater than 15% in the area of the drain field, water lines, sprinkler systems, springs, suction (irrigation) lines, drinking water cisterns, drain tiles, irrigation ditches, lakes, ponds, water courses, streams, floodplains, floodways, dry gulches, or existing septic systems. This must include all such features on neighboring properties within setbacks. ❑ Complete building floor plans. Floor plans should be 11" x17". (One set) X Proof of adequate water supply: (Well must be drilled before septic permit can be issued.): For private wells, recent well driller's pump test showing gpm actually produced, well permit showing legal supply. The minimum yield from the well or water storage shall be .07 gallons/minute/person, which is 100 gallons/person/day. There must be storage or yield capable of a 24-hour supply for domestic purposes only (not including fire protection or irrigation). For community systems, proof there is adequate water for all houses served by the system. X Proof of adequate water quality with bacteriology test: Aspen Consolidated Sanitation District and Snowmass Water & Sanitation District perform this test locally. X Percolation tests. X All ISDS Permit fees are $600 to cover first six hours of processing. Hourly rate of $100 will be charged beyond first six hours, up to $1,000. "Tank only" fee $150. Please make check payable to: Pitkin County Environmental Health Department. X Copy of county approval (1041 approval, subdivision approval, etc.) You must obtain written approval from Community Development before submitting this application if your septic system is outside the building envelope and any part of the system is more than 30" above grade. A WAIVER IS AVAILABLE FOR SUBMITTING A BUILDING PERMIT APPLICATION PRIOR TO OBTAINING AN INDIVIDUAL SEWAGE DISPOSAL SYSTEM PERMIT. Please ask the Community Development Dept. or Environmental Health Dept. for further information. H:\04498\ISDS DesignWpplicalion Checklist - Grady-Patch.doc Created on 02/22/2003 3:24 PM May 7, 2001 Colleen & Kevin Grady 3728 Queen Mary Drive Olney, Maryland 20832 Job No. 101 337 Subject: Percolation Testing, Proposed Residence, Lots 27 and 28, Crystal River Drive, Crystal River Park Subdivision, Redstone, Pitkin County, Colorado Dear Mr. and Mrs. Grady: As requested, Hepworth - Pawlak Geotechnical, Inc., performed percolation testing at the subject. site. The work was done in accordance with our agreement for Professional Services to you, dated May 3, 2001. A Profile :pit and three shallow percolation pits were excavated on May 3, 2001 in the northwest portion of Lot 28. Percolation test holes were hand dug through the topsoil adjacent to the shallow backhoe pits and soaked with water by Duane Piffer on May 3, 2001. The: subsoils exposed in the Profile Pit consisted of about 74 foot of organic topsoil overlying clayey sand with gravel. Hard sandstone bedrock was encountered in the pit at 3 feet below the ground surface and refusal to digging occurred at 31/2 feet. No free water was observed in the pit and the soils were moist. Percolation testing was conducted on May 4, 2001, by a representative of Hepworth - Pawlak Geotechnical, Inc. The percolation test results are summarized on Table I. The percolation test results indicate and infiltration rate of 15 to 30 minutes per inch with an average of 20 minutes per inch for the native soil overlying the bedrock. The percolation rates were based on the last three readings of each test. Based on the shallow bedrock encountered, a "mounded" system may be needed. Pitkin County requires that a professional civil engineer design the septic disposal system. If you have any questions or need further assistance, please call our office.
