RESOLUTION OF THE BOARD OF COUNTY
COMMISSIONERS ("BOCC")OF PITKIN COUNTY,
COLORADO MEMORANDUM OF UNDERSTANDING
BETWEEN PITKIN COUNTY AND GARFIELD
COUNTY REGARDING UTILIZATION OF THE
HEALTHY COMMUNITIES/EARLY AND PERIODIC
SCREENING,DIAGNOSTIC AND TREATMENT

RECITALS:
1. Pursuant to Section 2.8 3 (Actions) of the Pitkin County Home Rule Charter
("HRC')official action by formal resolution shall be required for all actions of the
Board not requiring ordinance power on matters of significant importance affecting
citizens
2. The intent of this Memorandum of Understanding is to set forth the conditions of
services provided by Garfield County Public Health to Pitkin County Public Health
by the Healthy Communities/Early and Periodic Screening,Diagnostic and
Treatment Program Family Health Coordinator.
3. The BOCC finds that it is in the best interests of the citizens of Pitkin County to
approve this Resolution.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Pitkin County, Colorado that it hereby adopts a Memorandum Of Understanding
between Pitkin County and Garfield County Regarding Utilization of the Healthy
Communities/Early and Periodic Screening,Diagnostic and Treatment Program Family
Health Coordinator and authorizes the Chair to sign the Resolution and upon the
satisfaction of the County Attorney as to form, execute any other associated documents
necessary to complete this matter.

BETWEEN PITKIN COUNTY AND GARFIELD COUNTY
REGARDING UTILIZATION OF THE HEALTHY COMMUNITIES/EARLY
AND PERIODIC SCREENING,DIAGNOSTIC AND TREATMENT
PROGRAM FAMILY HEALTH COORDINATOR

RECITALS
1 Garfield County Public Health(GCPH) is contracted with Health Care Policy and
Financing(HCPF) to provide the Healthy Communities and Early and Periodic
Screening, Diagnostic and Treatment Program(EPSDT) services on behalf of HCPF to
children,pregnant women, and families with Medicaid or Child Health Plan Plus(CHP+)
in both Garfield and Pitkin Counties
2 The Healthy Communities Outreach and Case Management Program focuses on
providing comprehensive outreach and case management services to all pregnant women
and children and youth aged 20 and under for the Early Periodic Screening Diagnosis and
Treatment (EPSDT)Medicaid program, clients enrolled into the Child Health Plan Plus
(CHP+)program and the Eligible But Not Enrolled(EBNE)populations. These services
are provided through the Family Health Coordinator employed by Garfield County Public
Health.
3. The purpose of this Memorandum of Understanding(MOU)is to identify the
terms by which the FHC will perform such services in Pitkin County.
NOW,THEREFORE,the parties agree as follows:
AGREEMENT
1. Garfield County Public Health. GCPH will fulfill its contractual commitments to
HCPF in Pitkin County through its Family Health Coordinator(FHC)as follows:
a The FHC will visit Pitkin County Public Health (PCHHS) a minimum of
quarterly to meet with clients on a designated day of the month agreed upon
by both PCHHS and the FHC.
b The FHC deliverables to PCHHS will include outreach by phone or in person
to Pitkin County clients as well as outreach to providers via attendance at a

regular(POD)networking, meetings or other provider meeting, as arranged by
PCHHS and defined in paragraph 2(b)
c. The FHC will provide quarterly reports to PCHIIS of"Newly Enrolled"
clients residing in Pitkin County and the number of Pitkin County clients
contacted during the preceding quarter("Client Interactions")
d. The FHC will provide quarterly reports to PCHIIS of the eligible but not
enrolled clients (EBNEs) contacted and the EBNEs that became enrolled from
those interactions.
e. The FHC will pros ide an annual report to PCHI IS showing trends, challenges
and goals for PCHIIS.
2. Pitkin County Health and Human Services. In exchange for the foregoing.
PCHHS will:
a Provide office space, phone, Internet connection, and access to a fax and copy
machine to the FHC while the FHC is physically in Pitkin County;
b. Arrange for outreach opportunities in Pitkin County at least once annually far the
FHC to present information on the HC program to provider partners.
3. Services offered by the FHS. The following list defines the types of services that
are offered by the FITC
• Initial Inform/Newly Enrolled-Contact families from Newly Enrolled list to do
Initial Inform of BC,explain benefits their plan offers,and verify family contact
information. This contact maybe made via phone call, home visit, face to face visit
and or mail ont.
• Acid a Baby/Newborn for family-When a mom delivers a new baby,the FHC can
assist her in getting the baby added to the family's Medicaid case,when necessary.
• Benefits/Pre Authorization Request(PAR) Issues—Explain to client/family when
a PAR is necessary and how to go through that process.
• Benefits Utilization Notification (reminders) -Set ticklers in Salesforce to remind
clients to renew benefits, keep scheduled appointments, etc.

• Billing Issues -Assist families with billing issues they may have with providers
• Community Resources-Refer families to any and all community resources that may
be beneficial to them Look for resources in the community for families, as needed
• Education/Awareness-Educate families on current health issues and importance of
regular well child check-ups, immunizations and/or prenatal care.
• Eligibility Issues-Assist families who are having trouble obtaining information
about their eligibility on a Medicaid/CHP+case.
• Eligibility Verification -Assist families to verify if they currently have coverage and
if it is Medicaid and/or CHP+
• Find a Specialist-Find specialty providers that accept the family's plan, when
necessary
• Follow-up-Follow-up with families as often as necessary to assist them with their
issue(s.)
• Outreach/Eligible But Not Enrolled (EBNE) -Attend various community
functions, such as fairs, Connect for Health CO enrollment, etc to reach out to
families that are potentially ERNE.
• Provider Issue-Assist families with any provider issuesls) they may be
experiencing
• Regional Care Collaborative Organization (RCCO)-Refer clients to the RCCO
when necessary Inform clients of what the RCCO is and how to obtain assistance
from the RCCO.
• Redetermination Issues-Assist families to understand and submit their
redetermination paperwork for Medicaid/CHP+ Submit requested documentation to
appropriate office, as necessary to complete redetermination.
• Research -Conduct research on behalf of families to assist with various. May
contact providers,DHS technician, state, etc.
• Returned mail- Document in Salesforce when an attempt has been made to contact a
family by mail and mail is returned.

• Transportation -Explain what transportation benefits are available to families in
their respective counties. Explain how and where to access these benefits.
4. Quarterly Evaluation. Representatives of GCPH and PCHHS will confer
quarterly to ensure that client needs are being met
5 No Financial Obligations. No money will be exchanged between GCPH and
PCHHS for the services described m this MOU
6 Term This MOU shall remain in effect as long as GCPH is contracted with
HCPF.
7. Miscellaneous Provisions. To the extent that any of the following is applicable to
this MOU,the Parties agree to handle them as follows
a. Assignability. This MOU is not assignable by either party.
b. Modification Any modifications to the agreement between GCPH and HCPF
will automatically apply to this MOU and any provision in conflict therewith will
be superseded. Further modifications between GCPH and PCHHS may be made
only in writing by an agreement approved by the respective parties and signed by
authorized officers of each party.
c Entire Agreement. This MOU is dictated by GCPH's contractual commitments to
HCPF constitutes the entire agreement between the parties No other obligations,
promises or agreements not expressly stated above, whether oral or written, are
merged or included
d. Severability. Should any one or more sections or provisions of this agreement be
judicially adjudged invalid or unenforceable, such judgment shall not affect,
impair, or invalidate the remaining provisions of this agreement, the intention
being that the various sections and provisions hereof are severable
8. Notices Any notice required or permitted under this agreement shall be in
writing and shall be hand-delivered or sent by registered or certified regular mail, postage
pre-paid and via e-mail(electronic delivery)to the mailing and e-mail addresses set forth
below.Each party by notice sent under this paragraph may change the address to which
future notices should be sent Electronic delivery of notices shall be considered delivered
upon receipt of confirmation of delivery on the part of the sender. Nothing contained
herein shall be construed to preclude personal service of any notice in the manner
prescribed for personal service of a summons or other legal process

9. Government Immunity The parties agree and understand that each is relying on,
and does not waive,the immunities and protections provided by the Colorado
Governmental Immunity Act,C.R.S. 24-10-101, et seq., as amended, to the extent
applicable to this MOU, or any officer, agent, or employee.
10 Agreement made in Colorado This MOU shall be construed according to the laws
of the State of Colorado, and venue for any action shall be in the District Court in and for
Garfield County, Colorado.
11. No Waiver. The waiver by any party to this agreement of any term or condition
of this Agreement shall not operate or be construed as a waiver of any subsequent breach
by any party
