The fee for a ISDS application is $330 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $55 per hour will be charged. The maximum fee is $1000. The basic fee of $330 is due at the time
of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second, before
final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain
tiles, irrigation ditches, lakes, water courses, streams, floodplains, dry gulches, and existing septic systems. I hereby certify
that any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of
any other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all
approvals and permits have been obtained.
The Aspen/Pitkin Environmental Health Department, Pitkin County and employees of these agencies will be held harmless
should the individual sewage disposal system fail or malfunction. The permit to construct is issued on information submitted by
the applicant or his/her representatives. The owner assumes full responsibility in case of failure of the system.

On June 28, 2002, High Country Engineering personnel observed the construction of the
ISDS for the Zanin Residence, 5084 Snowmass Creek Road in Pitkin County, Colorado.
One 2000 -gallon septic tank with baffle, a distribution box and 51 standard Infiltrator units
had been installed in a parallel trench configuration. Minimal backfilling had taken place.
The installation of the system was in conformance with the intent of the design and I have
enclosed pictures of the system for your convenience. If you have any questions, or need
additional information, please contact us.

Subject: Percolation Testing, 5084 Snowmass Creek Road, Pitldn County,

Dear Mr. Zanin:
As requested by you, Hepworth - Pawlak Geotechnical, Inc., performed percolation
testing at the subject site. The work was done in accordance with our agreement for
Professional Services to Joe Zanin Construction, dated April 19, 2002.
A Profile Pit and three percolation test holes were excavated on April 21, 2002 at the
locations shown on Fig. 1. The subsoils exposed in the Profile Pit consisted of about 2
feet of topsoil overlying silty clayey sandy gravel with cobbles and small boulders to the
bottom pit depth of 8 feet. No free water was observed in the pit and the soils were
slightly moist to moist. Percolation test holes were hand dug in the bottom of shallow
backhoe pits and soaked with water on April 21, 2002 by Joe Zanin Construction.
Percolation testing was conducted on April 22, 2002, by a representative of Hepworth -
Pawlak Geotechnical, Inc. The percolation test results are summarized on Table I.
Based on the subsurface conditions encountered and the percolation test results, the
tested area, should be suitable for a conventional infiltration septic disposal system.
Pitkin County requires that a professional civil engineer design the septic disposal
system.

A MINOR PLAT AMENDMENT

RECITALS
1. Brad Zanin ("Applicant") has applied to the Community Development Director of Pitkin County
("Director") pursuant to Section 3-190-080 of the Pitkin County Land Use Code ("Code"), for a subdivision
exemption fora minor plat amendment to designate an accessory building envelope to accommodate a barn.
2. The property is located at 5084 Snowmass Creek Road, and is described as Parcel B, Snowmass
Trust Subdivision.
The lot is zoned RS-30/PUD and contains 8.207 acres.
4. The Director finds that the request is consistent with the requirements of the Land Use Code.
APPROVED by the Director, subject to the following conditions:
I . The Applicant shall prepare an amended plat in accordance with Section 5-40 of the Land Use
Code, which shall designate the accessory building envelope. The plat shall be titled "First Amended Plat of
