Permit Conditions:
This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This system will replace an existing cesspool which will be filled in and abandoned. The new system will consist of
one 1250 gallon plastic (Roth or Infiltrator) two compartment tank with a screened dosing siphon. The effluent will
be dosed to a baffled distribution box which must be accessible at grade. The field will consist of 4 rows of 13
Quick 4 chambers for a total of 52 Quick 4 chambers 9R4 ARC -36 units are used, the engineer should be
contacted to assure the correct number of chambers are installed.
The field must be at least 10' from the property line, at least 25' from any water line, and at least 20' from the
house.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satistactory. Permission Is nereoy
given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This

ALL SERVICE septic, LLC reviewed and performed installation observations for the onsite
wastewater treatment system (OWTS) on November 10th and 17th, 2010 for the subject property.
The installation included a ROTHTM (Fralo) plastic 1500 -gallon, two-compartment septic tank with a
screened dosing siphon in the second compartment, followed by a gravelless chamber drain field. The
drain field consisted of 4 rows of 13 `Quick -4' chambers for a total of 52 chambers, equaling 514 SF
of infiltrative area.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWTS.
LIMITS:
The observation was based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWTS construction must be according to the county regulations. The installer should have
documented and demonstrated knowledge of the requirements and regulations of the county in which
they are working.

ALL SERVICE septic, LLC performed a subsurface investigation and a`Tepair onsite wastewater
treatment system (OWTS) design for the subject residence. The property is located outside of
Redstone, in an area where OWTS are necessary. The community water service provides water to the
residence.
SITE CONDITIONS
A single-family residence presently exists and is utilizing an OWTS which is experiencing problems.
The current OWTS should be abandoned. The residence has 3 -bedrooms. The proposed drain field is
down gradient of the existing residence, and vegetation consists of a moderate cover shrubs and
grasses.
SUBSURFACE
The subsurface was investigated by digging one profile pit and three percolation holes, as indicated. The
materials encountered in the profile pit consisted of 1 -foot of a sandy clay root zone, underlain by sandy
clay to clayey sand with scattered boulders to the maximum depth explored of 8 feet. Groundwater or
bedrock was not encountered. Three percolation holes were dug, in the area of the proposed drain field,
as indicated. Percolation rates ranged from 10 to 20 minutes per inch (MPI). A 13 MPI rate will be
used to design the OWTS.
DESIGN SPECIFICATIONS
The OWTS design is based on 3 -bedrooms and an average sewage load of 450 GPD. The installation
must include a 1250 -gallon, two-compartment, plastic ROTH or INFILTRATOR septic tank with a
screened dosing siphon in the second compartment. One gravelless chamber bed, using `Quick -4'
Onsite Wastewater Systems

InfiltratorTM chambers or `Arc -36' ADS chambers, must be installed as indicated. A distribution box,
brought to grade, or a level manifold must be used to distribute effluent.
Construction must be according to the Pitkin County Environmental Health and Natural Resources
Department OWTS regulations, the OWTS Construction Permit provided by Pitkin County
Environmental Health and Natural Resources Department, and this design.
OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should be placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS.
For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWS. The OWTS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWTS during construction. The OWTS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.

The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.
