The effluent filter was filled with sludge and cleaned and the inlet tee was cleaned. An extension handle was added to the effluent filter and risers to grade were added to the inlet and outlet manholes. Water was running from the house into the tank and a leaking toilet was discovered and repaired. Inspector Recommendations: Clean effluent filter annually, limit irrigation over soil treatment area. Department Recommendations: Limit irrigation over the soil treatment area and clean the effluent filter annually. Issuance of this OWTS Use Permit is based solely on the conditions observed and reported by the inspector to the Department on the date of the inspection(s) and on Department records at the time of permitting. The issuance of this permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail or that the system will not be subject to future enforcement action to correct non-compliant conditions. Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible failure. The subject OWTS consists of one I 000-galton, two-compartment concrete septic tank and an absorption area consisting of a gravelless chamber bad with 15 `Standard' Infiltrators chambers per row for a total of 45 chambers and approximately 6198 square feet of infiltrative area - The septic tank manhole lids were buried 2 to 3 -feet below grade, The septic tank appeared to be in good condition with both inlet and outlet toes present. An effluent filter was installed on the outlet of the septic tank- At the time of the inspection, the tank was slightly over full- This was a result of the effluent filter needing to be cleaned- Accumulation of sludge and scum in the tank warranted pumping - The absorption area showed no signs of saturation, and had no indication of failure. Observation ports were not present. The absorption area is located under a lawn area. The septic tank was pumped by B&R Septic. The inlet tee and effluent filter were cleaned. A handle extension was added to the Effluent fitter to be accessible from grade- Risers were added to the inlet and outlet side of the tank for accessibility from grade. The inlet side of the manhole lid was also replaced- During the repairs, B&R Septic personnel noticed water running into the tank even though the house was vacant- This would indicate either a running toilet or leaky fixture of some kind or discharge from the water softener into the septic tank On December 2, 2013, you confirmed a leaky toilet was repaired- We recommend water softeners be plumbed into a separate drywell and not the OWTS- This evaluation is not a guarantee of future system pertormance. This inspection is good for one year. Recommendations: • Clean effluent filler annually, or as needed- • Minimize irrigation over absorption area.
