At 200 Ute I used (2112" inch risers and one 6" per each side. The titer was cleaned and the handle
replaced with one that was the proper length for the additional risers. The inlet T was cleaned and a top cover
was discovered on the T. The lid on the inlet side of the tank was replaced.
Note: The tank had filed with water after pumping. I observed water running out of the outlet while working on
the titer. I would guess a water softener or toilet is leaking in the home.
The work stated above was completed on Wednesday 11-27-2013. Please see attached photos. I also have
additional photos and 2 short video of the water lowing out of the outlet if needed.

Onsite Wastewater Treatment System (OWTS) Use Permit Inspection
201 Ute Trail

As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit
inspection at 201 Ute Trail, Carbondale, Colorado on February 23, 2016. The legal description of the
0.360 -acre property is Lot 31, Swiss Village Tracts. While the address listed on Pitkin County Assessor's
Website is 201 Ute Trail, it is posted at the entrance to the property as 200 Ute Trail.
The original permit and record drawing was provided by Pitkin County Environmental Health
Department (Parcel ID # 2649-291-02-043). The subject OWTS consists of one 1000 -gallon, two-
compartment concrete septic tank and a soil treatment area (STA) consisting of a gravelless
chamber bed with 15 'Standard' Infiltrator® chambers per row for a total of 45 chambers and
approximately 698 square feet of infiltrative area. Pitkin County Individual Sewage Disposal System
Permit 04100, finalized on May 2, 2005, documents this system. Pitkin County also issued OWTS
Use Permit 0056.2013.powu on December 4, 2013.
The septic tank manhole lids were both accessible from grade. The septic tank appeared to be in
good condition with both inlet and outlet tees present. The outlet side of the septic tank (risers) was
showing signs of deterioration due to gasses. The upper opening of the inlet tee is capped.
Sometimes, this is done to prevent odors from migrating back into the house. There are no issues
with this cap remaining; however, it prevents visual inspection of flow coming into the septic tank.
An effluent filter was installed on the outlet of the septic tank. This filter must be cleaned annually,
or as needed. Accumulation of sludge and scum in the tank warrant pumping as part of routine
maintenance. The septic tank must be pumped prior to obtaining the OWTS Use Permit.
The STA had some snow cover; however, we were able to locate some inspection ports. The ports
we observed were dry at the time of the inspection. The distribution box was not accessible from
grade and we were not able to inspect this component. There was a trailer stored on the STA at the
time of our inspection. We recommended it be removed without driving over the STA (pulled out by
hand or with a winch). Weeds had overgrown the surface over the STA. The weeds were tilled out
and we recommend plating a native grass seed mix needing minimal irrigation. The STA showed no
signs of saturation, and had no indication of failure.
This evaluation is not a guarantee of future system performance. This inspection is good for one
year.

based upon what was observed and the Licensed Inspector's expertise in onsite wastewater technology.

period of time in the future. Because of numerous factors which may affect the operation of an OWTS,
as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this
report shall not be construed as a warranty by the Inspector that the system will function properly for any
particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied,
arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.
