ALL SERVICE septic, LLC performed an operation and maintenance visit on October 26th 2006 for
the subject property.
During our site visit we accessed the septic tanks, AX pods, and other applicable tanks. We observed
the working condition of the pumps, splitter valves, filters, and the control panel. We cleaned the
effluent filter. We cleaned the filter media, and flushed laterals, if it was necessary. We observed the
surface of the drain field, and checked observation ports.
OBSERVATIONS
All AdvantexTM and pumping components were functioning normally. There was no effluent
surfacing, and no indication of a failing drain field.
RECOMMEDATIONS AND/OR CONCLUSIONS
1. None at this time.
This evaluation is not a guarantee of any future OWS performance, in any way. ALL SERVICE
septic, LLC should be notified if changes are made to the OWS.
