At the time of issuance of this permit you are required to post $ as financial security to ensure successful
completion of your revegetation plan. These monies will be held for two growing seasons from the date of completion of the project
and will only be released at that time if a site visit by Pitkin County Zoning confirms that successful revegetation has been
accomplished. Applicant is obligated to contact Pitkin County Zoning (970-429-2799) to schedule the required two-year site visit. Th
monies will be forfeited if successful revegetation is not confirmed after two growing seasons, or if Applicant fails to timely schedule
the required site visit.
Documentation of State Engineer approval is required to be submitted prior to issuance of this permit (in connection with
co nstructionlrepairlalteration of a jurisdictional size dam).
Documentation of filing of a Notice of Intent to Construct a Non -Jurisdictional Water Impoundment Structure is required to b
submitted prior to issuance of this permit (in connection with a non -jurisdictional size dam).
Documentation of decreed water rights is required.

RE: Aspen Valley Ranch — Debris Flow Structure As-builts
This memo is being provided to address the constructed Debris Flow Basin and
immediate surrounding area at Aspen Valley Ranch. The Debris Flow basin consists of
the concrete debris flow catchment structure, a concrete box culvert, and the run -out area
at the end of the debris flow structure. It is our opinion that all of the aforementioned
structures/areas have been constructed in substantial conformance with our construction
drawings.
Accompanying this memo is a set of record drawings of the Debris Flow basin. The set
consists of structural details as well as civil grading and drainage plans based on record
survey data. We have also received documentation from HP Geotech that states that the
structure, as -constructed, meets the design intent of their debris flow modeling. That
memo is also attached to this document.

Subject: As -Built Review of Debris Flow and Flood Mitigation Facilities for Fan F1,
Ranch Center Area, Aspen Valley Ranch, Dry Woody Creek, Pitkin County,
Colorado.
Gentlemen:
As requested by Nathan Torres of SGM, we performed an as -built field review of the
debris flow and flood mitigation facilities for Fan F1 at the ranch center area on October
12, 2015. At that time, the construction mitigation facility was essentially complete
except for the revegetation of the transition between the mitigation facilities and the
natural channel at the fan head.
Based on our field observations and discussions with SGM, it appears that the mitigation
facility was built according to our previous design recommendations (Hepworth -Paw I ak
Geotechnical, 2014). It is our understanding that Aspen Valley Ranch AH, LLC is aware
that maintenance of the mitigation facility will be needed to keep it serviceable in the
event of heavy flow events.
