f Nancy Mackenzie From: Nancy Mackenzie Sent: Wednesday, June 18, 2008 4:45 PM To: Jim Austin; Vicki Monge Subject: Cochener - 689 Elk Mt Drive NO SEPTIC PERMIT REQUIRED The existing system is sized for this remodel. Please issue the building permit. Nancy Nancy MacKenzie 970-920-5076 Pitkin County Environmental Health & Natural Resources Department Environmental Health Specialist May 29, 2008 U PO Box 2844 Glenwood Springs, CO 81602 Ph 970-618-5033 Fax 303.216-2796 Stewart Custom Builders Attn: Peter 734 Main Street Carbondale, CO 81623 Onsite Wastewater System Evaluation, Existing Residence 389 Elk Mountain Drive Pitkin County, Colorado Peter, JUN 0 n 2008 PITKIN COUNTY ENVIRONMENTAL HEALTH & NATURAL RESOURCES Project No. 1481 As requested, ALL SERVICE septic, LLC performed an onsite wastewater system (OWS) evaluation on May 15th and May 28th 2008 for the subject property. The subject property is located in Pitkin County, outside of the town of Redstone, Colorado. The OWS was permitted by Pitkin County under permit numbers #80027 and 490004. The subject OWS has a primary septic system (#80027) and a grey water system for the clothes washing machine (#90004). According to Pitkin County Environmental Health Department the residence has an OWS permitted for 3 -bedrooms, and consists of a 1250 -gallon concrete septic tank and a drywell for the primary system and a 500 -gallon fiberglass tank and absorption trench for the grey water clothes washer system. OBSERVATIONS Primary septic system - A 1250 -gallon, two-compartment concrete septic tank is located next to the residence. Both compartments of the septic tank were accessible. PVC tees were present on the inlet and outlet of the septic tank. The septic tank appears to have good integrity and has a sewage level to indicate the tank is working properly. The drain field consists of one drywell with 5 -foot perforated concrete rings. The observation port is accessible and was checked and is dry. The area of the drain fields has adequate vegetation, and the drain field areas showed no signs of surfacing effluent, and had no indication of failure. Grey water clothes washer s stem — The fiberglass tank is located as indicated on attached drawing. The observation pipe for the drain field is accessible and was checked and is dry. The area of the grey water trench drain field had adequate vegetation, and the drain field area showed no signs of surfacing liquid. There was no indication of failure. The septic tanks should be pumped and cleaned every three years, or as needed. An effluent filter may be placed on the outlet of the septic tanks (and should be cleaned during tank pumping) to help minimize the migration of solids to the drain fields. Onsite Wastewater Systems Page 2 LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction must be according to the county regulations. Requirements not specified in this report must follow county regulations. The installer should have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Please call with questions. Don't have to count the loft as long as they don't use/consider it a bedroom. -----Original Message ----- From: Tim Petz [mailto:tpetzl@comcast.net] Sent: Monday, May 19, 2008 10:29 AM To: Nancy Mackenzie Subject: No of Beds for 389 Elk Mountain Drive #87011 I am checking on this system for Richard Spenser and Duane Stewart. I just faxed you the remodel. It is clear that it is 3 beds, although there is a loft on the upper level. There is no bath up on that level, just a room. Would this be a bedroom? I have a 4 page fax sent with the floor plans. thanks a bunch. tim Timothy R. Petz, P.G. Onsite Wastewater Systems 970-618-5033 Glenwood Springs 303-908-7823 Golden
