Permit Conditions:
This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This system will consist of a 1250 gallon 2 -compartment tank with an effluent filter on the outlet side. This tank will gravity flow
into a distribution box which will evenly distribute the effluent across the bed using 4 headers. The distribution box must be
level and accessible from grade. The bed will consist of 9 rows of 8 chambers for a total of 72 chambers, making the total
absorption area of 711 square feet. The bed must maintain minimum setbacks to the property line of 10 feet and 20 feet from
the existing or future dwelling.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby
given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved byjEH/NR before final approval of the system will be issued.

ALL SERVICE septic, LLC performed installation observations for the onsite wastewater treatment
system (OWTS) on August 10th 2010 for the subject property.
The installation included a 1250 -gallon, two-compartment, concrete septic tank with an effluent fitler
on the outlet, follow by one gravelless chamber bed. The bed has 9 row of 8 `quick -4' chambers, for
a total of 72 chambers, equaling 711 SF of area.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWTS.
LIMITS:
The observation was based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWTS construction must be according to the county regulations. The installer should have
documented and demonstrated knowledge of the requirements and regulations of the county in which
they are working.

ALL SERVICE septic, LLC evaluated the subsurface investigation prepared by HP Geo -tech, Job No
101 595, and prepared a repair onsite wastewater treatment system (OWTS) design for the subject
residence. The property is located in Pitkin County, in an area where OWTS are necessary. This letter
is to be used in conjunction with plans provided. Community water serves the property.
SITE CONDITIONS
A residence exists on the subject property. The existing OWTS consists of a metal septic tank and a
drain field of unknown dimensions. The area of the drain field slopes approximately 3% to the east
and vegetation consists of native grasses, and conifer trees.
SUBSURFACE
The subsurface was investigated by HP Geo -tech by excavating one Profile Pit and three percolation
holes as indicated on attached plans. HP Geo -tech indicated the test has a 1 foot topsoil layer, underlain
by silty sand with scattered cobbles and boulders to the maximum depth explored of 8 feet.
Groundwater or bedrock was not encountered. Percolation rates ranged from 5 to 10 minutes per
inch (MPI). An 8 MPI rate will be used to design the OWTS.
DESIGN SPECIFICATIONS
The OWTS design is based on 4 -bedrooms and an average sewage load of 600 GPD. The installation
must include a 1250 -gallon, two-compartment, precast concrete septic tank, with the addition of an
effluent filter on the outlet, followed by one gravelless chamber bed. Nine rows of 8 `Quick 4'
chambers, for a total of 72 chambers, and 711 square feet (SF) of infiltrative area, are required. A
distribution box, brought to grade, should be used to distribute effluent.
Construction must be according to the county OWTS regulations, the septic permit provided by Pitkin
County, and this design. This design for the subject property, noted above, is valid only in conjunction
with the approved County Health Department permit.

OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced during and prior to re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should be placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS.
For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Septic tank risers should extend to grade. Access to all
tank compartments and distribution devices is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWTS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS
construction must be according to the county regulations. Requirements not specified in this report
must follow county regulations. The installer should have documented and demonstrated knowledge
of the requirements and regulations of the county in which they are working.

DETERMINATION OF THE HEARING OFFICER OF THE COMMUNITY DEVELOPMENT
DEPARTMENT OF PITKIN COUNTY, COLORADO, APPROVING THE OLANDER 1041
HAZARD REVIEW AND CONCEPTUAL SUBMISSION
Determination No. 10 -2002
RECITALS
1. Michael & Cheryl Olander ("Applicant") have applied to the Pitkin County Hearing Officer
("Hearing Officer") for 1041 Hazard Review and Conceptual Submission approval to establish a building
envelope for a new single family residence and accessory structures.
2. The parcel is located at 0009 Chair Mountain Drive, adjacent to the Crystal River Park Subdivision
and is described as Parcels 1 and 2, as shown on the Hunter/Colomo Boundary Line Adjustment Plat
recorded August 18, 1993 in Plat Book 32 at Page 41.
The property is zoned AFR 10 and contains 1.07 acres.
4. The parcel was legally created and configured prior to May 8, 1972 and has not merged with
adjacent properties, and, therefore, is exempt from growth management for up to 5,750 square feet of floor
area. The existing residence on the property was constructed in the 1950s.
5. The Hearing Officer considered this application at a duly noticed public hearing on April 16, 2002,
at which time evidence and testimony were presented with respect to this application.
6. The Hearing Officer finds that the building envelope contains slopes of less than 15%; the wildfire
hazard is rated as low to medium and can be mitigated; the envelope is within an alluvial fan where the
hazard can be mitigated; and the envelope is not within any critical wildlife habitat areas.
NOW THEREFORE BE IT RESOLVED by the Pitkin County Community Development
Hearing Officer that he does hereby approve the Olander 1041 Hazard Review and Conceptual Submission,
subject to the following conditions, which shall run with the landand be binding on all successors in interest.
Prior to submission of any earthmoving, access, or building permit applications for new
development, the Applicant shall submit a 1041 Hazard Review Site Plan to the Community
Development Department for approval and recording. The site plan shall also be submitted in digital
format for integration into the County's GIS system. The mylar copy of the site plan must be signed
by the owners prior to submittal for recording. The site plan shall be amended as folbws:
A. Include a note that the property boundary adjacent to the southern leg of Chair Mountain
Drive is considered the front lot line.
B. Designate an access envelope between the property line and the building envelope.
2. Prior to submission of a septic permit, the Applicant shall provide a letter of intent to serve from the
Redstone Water District to the Environmental Health Department.
Prior to submission of any building permit applications, the Applicant shall:
A. Obtain a site specific geotechnical study from a qualified professional engineer or

1. All roof coverings shall be noncombustible materials as defined in the Uniform
Building Code (UBC) 1997 Section 1504 and installed on a Class A roof assembly.
2. All roof coverings shall have a surface that shall facilitate the natural process of
clearing the roof.
3. All roof designs shall facilitate the natural process of clearing roof debris.
Protrusions above the roofline, such as parapets, shall be prohibited.
4. Roofs shall be installed as required by UBC I997 Chapter 15 and shall have a
minimum slope of 1:48.
5. All roof designs, coverings, or equivalent assemblies shall be specifically approved
by the Fire Marshal prior to submittal of a building permit application.
E. Vents shall be screened with corrosive resistant wire mesh with mesh I/, inch maximum.
F. Roofs and gutters shall be kept clear of debris.
G. Yards shall be kept clear of all litter, slash, and flammable debris.
H. All flammable materials and firewood/wood piles shall be stored on a parallel contour a
minimum of 15 feet away from any structure.
I. Swimming pools or ponds shall be accessible to Fire Department vehicles.
J. Fences shall be kept clear of brush and debris.
K. Wood fences shall not connect to the structure.
L. Fuel tanks shall be installed underground with an approved container.
M. Propane tanks shall be installed according to NFPA 48 standards and on a contour away
from the structure with standard defensible space vegetation mitigation around any abova
ground tank. Any wood enclosure around the tank shall be constructed with materials
approved for 2 hour fire -resistive construction on the exterior side of the walls.
N. Each structure shall have a minimum of one 10 pound approved ABC fire extinguisher
placed in a visible and accessible location.
O. Addresses shall be clearly marked with 2 inch notreombustible letters and shall be visible
and installed on a non-combustible post.
P. New utility lines shall be buried within the driveway.
Exterior lighting shall comply with the County's lighting regulations in effect at the time of
installation.
No development, including grading, excavation, fill placement, berming, landscaping, vegetation
removal or disturbance, or septic system shall occur outside of the approved building envelope.
Utility and driveway extension and maintenance shall occur within approved building or access
envelopes.
All areas disturbed by construction shall be revegetated with native shrubs and grasses to limit
water consumption and additional runoff within one growing season of the project's completion.
10, The floor area shall be limited to a maximum of 5,750 square feet exempt from growth
management.
11. The Applicant shall adhere to all material representations made in the applcation or in public
meetings or hearings and shall consider those representations to be conditions of approval, unless
amended by other conditions.
