This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This system will replace a collapsed tank and failing field area. It will consist of a 1500 gallon, 2 compartment tank with a
retroFAST .375 unit installed in the second compartment. This secondary treatment unit will flow into a drywell because of
size constraints on the lot. The dry well will be 15'x15' and 12 feet deep for 945 sq ft of absorption area. Five foot diameter
perforated rings will be placed on 8" of gravel bedding and extended to the top of the dry well which will be accessible from
grade. The floor and walls of the dry well must be scarified prior to fill. The dry well will be filled with gravel. A slotted PVC
pipe will be use as an observation port and will extend from the bottom of the dry well and be accessible from grade.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built' letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satistactory. rermission is nereuy
given to the property ownerl;s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year froth the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved by EH before final approval of the system will be issued.

ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system
(OWTS) on May 8, 2012 for the subject property.
The installation included a new 1500 -gallon, two-compartment tank with a RetroFAST .375 installed
in the second compartment. Effluent gravity flows to a 15'x15' and 12' deep drywell for a total of
945 square feet of absorption area.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWTS.
LIMITS:
The observations were based on information submitted. If conditions are different from conditions
described in this letter, ALL SERVICE septic, LLC should be notified. Any additional OWTS
construction must be according to the county regulations. The installer should have documented and
demonstrated knowledge of the requirements and regulations of the county in which they are
working.

ALL SERVICE septic, LLC performed a subsurface investigation and an onsite wastewater system
(OWS) design for the subject residence. The property is located outside of Redstone Colorado, in an
area where OWS are necessary. The subdivision will provide water via a community water source to
the proposed residence.
r('� SITE CONDITIONS
A single-family, 3 -bedroom residence, having less than 6000 SF is being proposed. The�Ilotpresentl vy scant. The proposed drain field are�es 5 % to the sout z�east, ancivegetation conf

SUBSURFACE
The subsurface was investigated by digging a profile pit and collecting a sample to perform a
gradation analysis. The materials encountered in the profile pit consisted of 1 -foot of a sandy root
zone, underlain by clayey, silty, sand with sparse gravel and cobble to the maximum depth explored
of 8 feet. A 16 -foot profile pit must be dug to confirm soils prior to the Health Department visiting
the site. An LTAR of .72 gals/day will be used to design the OWS.
DESIGN SPECIFICATIONS
The OWS design is based on 3 -bedrooms and an average sewage load of 450 GPD. The installation
must include a 1500 -gallon, two-compartment, precast concrete or plastic septic tank, and a 0.375
RetroFAST unit. One `drywell' must be constructed as indicated on plans. The drywell must be 15 -ft
by 15 -ft, and must be 12 feet deep. Concrete perforated concrete rings must be used in the drywell
construction. The total proposed area is 945 SF.
Construction must be according to the county ISDS regulations, the septic permit provided by Pitkin
County Environmental Health Department, and this design. This design for the subject property, noted
above, is valid only in conjunction with the approved County Health Department permit.

OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should be placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWS.
For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWS. No hazardous wastes should be directed into the OWS.
Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.
