OWTS Use Status:
® In use at the time of the inspection. ❑Not in use at the time of the inspection.*
*If the OWTS was not in use at the time of the inspection, it is recommended that the system be re-evaluated when it is in use for a more accurate
evaluation of the system.
System Records:
Permit #: 96041 Date of Issuance: 6/5/96 Date of Final Approval: 7/12/96
# of Bedrooms or fixtures served by OWTS: Designed to serve 3 bedrooms
Operational Status: The inspector indicates the system is functioning properly and there is no indication of failure. The
tank was pumped as part of the inspection and had both inlet and outlet tees in place. The risers on the outlet of the
tank were replaced because they had become no longer watertight. The dry well did not show any signs for failure.
Inspector Recommendations: N/A
Department Recommendations: We recommend adding an effluent filter to the outlet tee of the tank.
Issuance of this OWTS Use Permit is based solely on the conditions observed and reported by the inspector to the Department on the date of the inspection(s) and
on Department records at the time of permitting. The issuance of this permit does not constitute a guarantee, warranty, or representation by the Department
that the system will operate properly or will not fail or that the system will not be subject to future enforcement action to correct non-compliant conditions.
Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible
failure.

To whom it may concern,
The purpose of this letter is to show that Duane Piller, with Pifco,
at the request of Sherry Rubin with Coldwell Banker worked on
the
Chuck and Barbara Aulbin septic system in Crystal River Park Sub.
On September 30, 2013, 1 went to the Aulbins with small backhoe
and dug down and removend the concrete extension rings on
there
septic tank. I then laid in standard concrete sealant called rubber
and reasembled the concrete extension rings to make them water
tight.
