Septic permit approved per compliance with the engineer design and specifications dated February 1, 2007, Project
#2006-469. Any changes must be approved by this department and the design engineer prior to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin County
ISDS regulations. The well must be at least 100 feet from the absorption field.
This system calls for one 1000 gallon two compartment tank with a biotube pump vault in the second compartment. The
effluent will be pumped to a series of trenches. The first two trenches will have 9 Quick 4 chambers, the third trench 7
chambers, and the final trench 4 chambers. The system will be pressurized and a pump test at the time of
installation must be conducted to assure even distribution throughout the system.
This permit is expressly conditioned on the property owner providing for the regular inspection, cleaning and
maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations.
This department does not endorse any brand of products. This permit must be kept on-site during installation. The
engineer must do a final inspection of the installation and submit an as -built letter to this department. This department
must also be called for an inspection with a minimum of 48 hours advanced notice.
Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly
encouraged.

Plans and specifications of the proposed individual sewage disposal system have been reviewed and are considered
satisfactory. Permission is hereby granted to the owner or the owner's agent to perform the work indicated in accordance with
the Pitkin County ISDS Regulation in effect at the time of issue. This permit becomes invalid 6 months from the date that the
permit was issued unless system construction has commenced, or an extension has been approved in writing by the
Department. As -built drawings must be included with this permit before the final approval will be issued.

The fee for a ISDS application is $600 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.

Dear Carla:
This letter represents a certification of the construction of the Patrick and Alicia Smith residence
Onsite Wastewater Treatment System (OWTS) under Pitkin County Environmental Health
Department Permit for an Individual Sewage Disposal System No. 07036. The construction of the
on-site OWTS has been completed pursuant to plans and specifications prepared by Schmueser
Gordon Meyer, Inc. dated February 1, 2007 (stamped 02-28-2007). The OWTS design for the 3 -
bedroom Smith residence, at 795 Elk Lane near Redstone, was configured as four gravel -less
chamber seepage trenches fed by treated wastewater routed through a septic tank totaling 1,000
gallons in capacity.
I inspected the installation of the septic tank, collection system, pump system and gravel -less
chamber seepage trenches pursuant to our original plans and specifications. My inspection during
the construction process indicated that the system was installed in a manner generally consistent
with our planset. with only minor revisions to the original design. I did note, on a field inspection on
May 16, 2008, that the uppermost trenches had been revised with 6 Quick -4 chambers in the third
trench (it was designed with 7) and 5 chambers in the uppermost fourth trench (it was designed with
4). This was done to accommodate an existing tree in place near the end of trench 3 and results in
the same number of total chambers as in the original design. This change has no impact on the
function of the system as designed.
I took a few photographs of the work in progress at the time of my inspection on May 16th. As of my
site visit, the septic tank was in place and the pump and filter system was installed but not wired and
operational. A PVC tee was needed on the inlet side of the tank. All four seepage trenches had
been completed and, with the minor revision noted above, the trenches appeared to be level and
showing the appropriate total number of chambers per our design. Since my inspection on May 16tH
a member of our staff was back on-site to observe that the pump system had been wired and is now
operational, demonstrating good flow to the far end of the system.

Subject: Subsoil Study for Foundation Design and Percolation Test, Proposed
Residence, Lot A-7, Redstone Ranch Acres, 795 Elk Lane, Pitkin County,
Colorado
Dear Mr. and Mrs. Smith:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study and
percolation test for foundation and septic disposal designs at the subject site. The study
was conducted in accordance with our agreement for geotechnical engineering services to
you dated August 1, 2006. The data obtained and our recommendations based on the
proposed construction and subsurface conditions encountered are presented in this report.
Evaluation of potential geologic hazard impacts on the site are beyond the scope of this
study.
Proposed Construction: The proposed residence will be a one and two story wood
frame construction above a crawlspace and located on the site as shown on Figure 1. Cut
depths are expected to range between about 3 to 4 feet. Foundation loadings for this type
of construction are assumed to be relatively light and typical of the proposed type of
construction. The septic disposal system is proposed to be located slightly uphill and east
of the residence.
If building conditions or foundation loadings are significantly different from those
described above, we should be notified to re-evaluate the recommendations presented in
this report.
Site Conditions: The building area is located in a small clearing on the south side of Elk
Drive. Vegetation consists of an evergreen forest with scattered aspen trees and
understory of grass, weeds and brush. The ground surface in the building area slopes
moderately down to the west. A very steep hillside is located just south of the proposed
residence.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
excavating two exploratory pits in the building and in the septic disposal areas at the
approximate locations shown on Figure 1. The logs of the pits are presented on Figure 2.
The subsoils encountered, below about one foot of topsoil, consist of slightly silty sandy
gravel with cobbles and boulders. Results of a gradation analysis perf6rmed on a sample
of sandy gravel (minus 3 inch fraction) obtained from the site are presented on Figure 3.

The laboratory test results are summarized in Table 1. No free water was observed in the
pits at the time of excavation and the soils were slightly moist to moist.
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature of the proposed construction, we recommend spread
footings placed on the undisturbed natural soil, below topsoil, designed for an allowable
soil bearing pressure of 2,000 psf for support of the proposed residence. The soils tend to
compress after wetting and there could be some post -construction foundation settlement.
FootinLys should be a minimum width of 16 inches for continuous walls and 2 feet for
columns. Loose and disturbed soils encountered at the foundation bearing level within
the excavation should be removed and the footing bearing level extended down to the
undisturbed natural soils. Exterior footings should be provided with adequate cover
above their bearing elevations for frost protection. Placement of footings at least 42
inches below the exterior grade is typically used in this area. Continuous foundation
walls should be reinforced top and bottom to span local anomalies such as by assuming
an unsupported length of at least 10 feet. Foundation walls acting as retaining structures
should be designed to resist a lateral earth pressure based on an equivalent fluid unit
weight of at least 45 pcf for the on-site soil as backfill.
Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support lightly
loaded slab -on -grade construction. To reduce the effects of some differential movement,
floor slabs should be separated from all bearing walls and columns with expansion joints
which allow unrestrained vertical movement. Floor slab control joints should be used to
reduce damage due to shrinkage cracking. The requirements for joint spacing and slab
reinforcement should be established by the designer based on experience and the intended
slab use. A minimum 4 inch layer of free -draining gravel should be placed beneath
basement level slabs to facilitate drainage. This material should consist of minus 2 inch
aggregate with less than 50% passing the No. 4 sieve and less than 2% passing the No.
200 sieve.
All fill materials for support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site soils devoid of vegetation, topsoil and oversized rock.
Underdrain System: Although free water was not encountered during our exploration, it
has been our experience in mountainous areas that local perched groundwater can develop
during times of heavy precipitation or seasonal runoff. Frozen ground during spring
runoff can also create a perched condition. We recommend below -grade construction,
such as retaining walls and crawlspace areas, be protected from wetting and hydrostatic
pressure buildup by an underdrain system.
The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free -draining granular material. The drain should
be placed at each level of excavation and at least 1 foot below lowest adjacent finish
grade and sloped at a minimum 1 % to a suitable gravity outlet. Free -draining granular
material used in the underdrain system should contain less than 2% passing the No. 200

sieve, less than 50% passing the No. 4 sieve and have a maximum size of 2 inches. The
drain gravel backfill should be at least 1'/ feet deep.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times after the residence has been completed:
1) Inundation of the foundation excavations and underslab areas should be
avoided during construction.
2) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas. Free -draining wall backfill should be
capped with about 2 feet of the on-site, finer graded soils to reduce surface
water infiltration.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We
recommend a minimum slope of 12 inches in the first 10 feet in unpaved
areas and a minimum slope of 3 inches in the first 10 feet in pavement and
walkway areas. A swale may be needed uphill to direct surface runoff
around the residence.
4) Roof downspouts and drains should discharge well beyond the limits of all
backfill.
Percolation Testing: Percolation tests were conducted on August 4, 2006 to evaluate the
feasibility of an infiltration septic disposal system at the site. One profile pit and three
percolation holes were dug at the locations shown on Figure 1. The test holes (nominal
12 inch diameter by 12 inch deep) were hand dug at the bottom of shallow backhoe pits
and were soaked with water one day prior to testing. The soils exposed in the percolation
holes are similar to those exposed in the Profile Pit shown on Figure 2 and consist of
slightly silty sandy gravel with cobbles and small boulders. The percolation test results
are presented in Table 1. Based on the subsurface conditions encountered and the
percolation test results, the tested area should be suitable for a conventional infiltration
septic disposal system. Pitkin County requires that a civil engineer design the septic
disposal system.
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either expressed or implied. The conclusions and recommendations submitted
in this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Figure 1, the proposed type of construction and our experience in
the area. Our services do not include determining the presence, prevention or possibility
of mold or other biological contaminants (MOBC) developing in the future. If the client
is concerned about MOBC, then a professional in this special field of practice should be
consulted. Our findings include interpolation and extrapolation of the subsurface
conditions identified at the exploratory pits and variations in the subsurface conditions
may not become evident until excavation is performed. If conditions encountered during
construction appear different from those described in this report, we should be notified at
once so re-evaluation of the recommendations may be made.

Alicia Sm' frantically calling to be able to get her well drilled tomorrow. She said you needed to know if there
re septic systems on the surrounding lots. Our records show that the surrounding lots A-6, A-8 and A-9 (parcel
#'s ending in -012, -011, -007) have very old septic systems some place on each property. These are from the
1970's so we just have sketches of some of them, nothing to scale on a plat. So I could not state that a well on
this lot would be a minimum of 100 ft from a neighboring septic field.
The other concern is that since this is such a small lot, the neighboring wells should be located to insure that the
future location of Lot A -7's septic can be 100 ft from them.
The engineer designing their septic system is supposed to check on these well setbacks when designing the
septic and should b e consulted on the well location. If I had info from the engineer showing the surrounding lots
and well and septic locations, I could give you a more definite answer (maybe).

GENERAL NOTES
1. ALL MATERIALS, INSTALLATION PRACTICES, SETBACK REQUIREMENTS, ETC., SHALL COMPLY
WITH THE PITKIN COUNTY INDIVIDUAL SEWAGE DISPOSAL SYSTEMS REGULATIONS.
2. LOCATION OF THE TRENCH MAY BE ADJUSTED IN THE FIELD TO MINIMIZE CONFLICTS WITH
EXISTING VEGETATION, PROVIDED ALL SETBACKS ARE MET. FILL AREAS AND AREAS REQUIRING
RE—GRADING SHALL BE AVOIDED. CONTRACTOR SHALL VERIFY THAT LAYOUT ACCOMMODATES
SITE SLOPE AND STRUCTURAL/PLUMBING ELEVATIONS.
3. PIPING IN THE ISDS SYSTEM SHALL BE STANDARD SEWER PIPE ASTM D3034, EXCEPT AS

7. AN OSI BIOTUBE EFFLUENT PUMP WITH FILTER SHALL BE PART OF THIS PACKAGE. THE
CONTRACTOR SHOULD INCLUDE ALL PIPING, WIRING AND ELECTRICAL/CONTROLS FOR THIS
SYSTEM.
8. THE CONTRACTOR AND OWNER SHALL TAKE WHATEVER MEASURES ARE NECESSARY TO
ASSURE THAT (a) SEPTIC TANK AND SEWER LINES ARE COMPLETELY WATER TIGHT, AND (b)
THE SYSTEM IS INSTALLED TO PREVENT FREEZING OF ALL GRAVITY SEWER LINES.
9. THE PITKIN COUNTY ENVIRONMENTAL HEALTH DEPARTMENT AND THE ENGINEER SHALL BE
NOTIFIED WHEN CONSTRUCTION COMMENCES AND KEPT ABREAST OF THE CONSTRUCTION
PROGRESS SO THAT SUFFICIENT INSPECTION CAN BE PERFORMED TO ASSURE CONFORMANCE
WITH THESE PLANS. PROVIDE A MINIMUM OF 48 HOURS NOTICE TO EACH.
10. THE FOLLOWING PREVENTATIVE MAINTENANCE SCHEDULE IS RECOMMENDED FOR THE LIFE
OF THE ISDS SYSTEM: A. PUMP THE SEPTIC TANK AT LEAST ONCE EVERY TWO YEARS. B.
CHECK PLUMBING FIXTURES ON A REGULAR BASIS AND REPAIR ALL LEAKS WHICH CAN ADD
SUBSTANTIAL AMOUNTS OF WATER TO THE SYSTEM.
11. NO VEHICULAR TRAFFIC SHALL BE ALLOWED ON THE PROPOSED TRENCH AREA
12. IT IS THE CONTRACTORS RESPONSIBOJTY TO VERIFY ALL UTILITY LOCATIONS THAT MAY
EFFECT THE LOCATIONS OF THE ISDS FACILITIES DRAWN. VERIFICATION IS TO BE MADE WITH
THE OWNER AND THE AFFECTED UTILITY COMPANIES.
13. SUBSTITUTION OF MATERIALS (I.E. PIPE, TANKS, ETC.) IS ACCEPTABLE PROVIDED
VERIFICATION AND ACCEPTANCE BY THE ENGINEER AND ENVIRONMENTAL HEALTH OFFICER IS
OBTAINED.
14. THE APPROXIMATE LOCATION OF ALL KNOWN UTILITIES (WATER, SEWER, GAS, PHONE,
ELECTRIC, CABLE, ETC.) ARE SHOWN ON THE DRAWINGS. THE CONTRACTOR SHALL CONTACT
ALL UTILITY OWNERS TO VERIFY BOTH LOCATION AND DEPTH OF UTILITIES BEFORE ANY WORK
BEGINS. CONTRACTOR SHALL BEAR THE RESPONSIBILITY FOR THE PROTECTION OF UTILITIES
