If approval takes longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second, before final approval of the ISDS permit. Application for an individual sewage disposal system is hereby submitted. He plans to apply for a remodel permit with no additional sq footage and no change to room count soon. Mr. Horwich states that the $52122 employee housing mitigation fee is the reason he is cancelling his permits. He also will be requesting refunds of any fees that may be available for refund. He specifically mentioned a "septic permit" fee. The testing was done in accordance with our agreement for professional services to you, dated October 13, 2005. We previously conducted a subsoil study and percolation testing on the property and presented our findings in a report dated April 29, 2002, Job No. 102 227. A profile pit and three percolation test holes were excavated on October 20, 2005 at the locations requested by High Country Engineering and shown on Figure 1. The subsoils exposed in the Profile Pit consisted of about 1 1/2 feet of topsoil overlying sandy gravelly clay to 3 feet and clayey silty sandy gravel with cobbles and boulders to the bottom pit depth of 8 feet. No free water was observed in the pit and the soils were slightly moist to moist. Percolation test holes were hand dug in Cite bottom of shallow backhoe pits and soaked with water on October 20, 2005. Percolation testing was conducted on October 21, 2005, by a representative of Hepworth - Pawlak Geotechnical, Inc. The test holes were based in the gravel soils. The percolation test results are summarized on Table 1. The overall average percolation rate of the three test holes was 26 minutes per inch. Based on the subsurface conditions encountered and the percolation test results, the tested area should be suitable for a conventional infiltration septic disposal system. Arnold and Judith Horwich ("Applicant') have applied to the Pitkin County Hearing Officer for 1041 Hazard Review, Conceptual Submission, Ridgeline Review to establish a building envelope around the existing improvements on the property, as well as the proposed 3 -bay garage, workspace and caretaker dwelling unit. The Hearing Officer heard this application at a continued public hearing on August 28, 2001 at which time additional evidence and testimony were presented. 6. The Hearing Officer finds that this application proposes development in the best possible location with the least impacts, given the hazards identified on the property. 7. The Hearing Officer has determined that the proposed caretaker dwelling unit meets thestandards established in sections 3-150-130(B) and 3-210 of the Land Use Code. The Applicant proposes to deed restrict the proposed caretaker dwelling unit. Structural Design and Construction Requirements all structures on the proper . 1. Roofs shall have anon -combustible roof covering on a Class A assembly roof system. Wood shake/shingle roof covering are prohibited in all wildfire hazard areas. Roofs with less than 3:12 pitch are not permitted in 1041 Wildfire Areas unless they comply with the following: a. All roof coverings shall be non-combustible materials as defined in the Uniform Building Code (UBC) 1997 Section 1504 and installed on a Class A roof assembly. b. All roof coverings shall have a surface that shall facilitate the natural process of clearing the roof. c. All roof design shall facilitate the natural process of clearing roof debris. Protrusions above the roofline, such as parapets, shall be prohibited d. Roofs shall be installed as required by UBC 1997 Chapter 15 and shall have a minimum slope of 1:48. e. All roof designs, coverings, or equivalent assemblies shall be specifically approved by the Fire Marshall prior to submittal of a building permit application. 2. All vents shall be screened with corrosive resistant wire mesh of one quarter inch maximum. D. Maintenance: 1. Roofs and gutters shall be kept clear of debris. 2. Yards shall be kept clear of all litter, slash, and flammable debris. 3. All flammable materials (including firewood/wood piles) shall be stored on a parallel contour a minimum of 15 feet away from any structure. Firewood/wood piles shall be stacked on a parallel contour a minimum of 15 feet away from all structures. E. Miscellaneous: 1. Swimming pools shall be accessible to Fire Department vehicles. 2. Fences shall be kept clear of brush and debris. 3. Wood fences shall not be connected to the structure. 4. Fuel tanks shall be installed underground with an approved container. 5. Addresses shall be clearly marked and visible with minimum 2" non-combustible letters and shall be clearly visible at the primary point of access from the public or common access road and installed on a non-combustible post. 7. Any outbuildings or additional buildings shall adhere to the same standards as structures. F. Utilities: 1. Utilitv lines shall be buried. 4. No development, including grading, excavation, fill placement, berming, landscaping, entry or ranch gates, and vegetation removal or disturbance shall occur outside of the approved building envelope, except for utility and driveway extension and maintenance, and as may be necessary to implement the wildfire mitigation measures required elsewhere in this document. 5. All areas disturbed by construction shall be revegetated with native shrubs and grasses to limit water consumption and additional runoff within one growing season of the project's completion. 6. All development must comply with the provisions of the Pitkin County's Noxious Weed Management Plan. 7. The Applicant shall comply with the following standards for development on slopes of greater than 15%: A. Adequate mechanical support shall be provided for cut slopes. B. Adding; water which may decrease slope stability shall be avoided. C. Adding; weight to the top of the slope shall be avoided. D. Disturbed slopes must be contoured so that they can be revegetated. E. Steepening of existing slopes shall be avoided. F. Confine cuts, fills, grading and excavation shall be limited to the minimum area needed for constriction. 8. Prior to the submittal of any building permit applications, the Applicant shall provide documentation to the Environmental Health Department that the well can service all new structures and uses. The well shall be located within the approved building envelope or accessory envelope. 9. Prior to the issuance of any building permits, the Applicant shall provide documentation to the Pitkin County Environmental Health Departmentthat the existing sewage disposal systems can accommodate additional structures and uses. Sewage disposal system shall be located within the approved building envelope or accessory envelope. 10. A fireplace/woodstove application must be filed and approved by the Community Development Department prior to the issuance of a building permit. 11.
