This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This permit is for the replacement of the existing septic tanks and sewage pipe to a new location due to construction of a
swimming pool. The sewage shall flow via gravity to a 1500 -gallon septic tank. Effluent from the first tank shall flow to a
second 1250 -gallon pump tank. Effluent will then be pumped to the existing soil treatment area. If the existing septic tanks are
to be reused, a water tight test will be required. The new sewer line shall not have any 90 degree or greater bends. The new
tank and pipe locations must be a minimum of 10 feet to the property line and 10 feet to any water line. Proper functioning of
the audible and visual alarms must be verified and evidence submitted to this office prior to final approval of the permit.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Issuance of this
permit does not imply compliance with Pitkin County building and/or land use regulations, nor guarantee issuance of building and/or land use permits.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.

Pitkin County Environmental Health and Natural Resources Department:
This letter is intended to fulfill the Pitkin County requirements for the certification of construction by
a CO PE of the Onsite Wastewater Treatment System (OWTS) improvements at 220 Owl Creek
Road, Pitkin County, CO. This work was completed under Pitkin County Environmental Health and
Natural Resources Department Permit for Parcel ID #273504102001 Permit #0004.2014. powt. The
observations made by SGM for the OWTS improvements have been completed per Pitkin County
requirements and are pursuant to plans and specifications prepared by SGM, Inc.
SGM conducted site visits on 3/27/14 to observe the installation of a 1,500 septic tank, a 1,250
gallon pumping tank, and distribution line realignment and reconnection. These items had been
replaced and installed per plan as shown on the attached As Built drawing. Due to the age of the
pump system, the owner elected to replace the existing pump system with a new duplex pump
system and alarm. The alarm was installed on the solar field panel between the leach field and
septic tanks and is visible and audible from the septic tanks. The contractor, GF Woods, and
OWTS installer tested the tank for watertightness successfully.
The attached as built drawings show the general layout of the completed improvements. All items
have been installed per setback requirements. The as -built drawing was provided by GF Woods.
GPS coordinates have been provided on the drawing.
The intent of this letter, along with the attached drawing, is to provide adequate information to
convey that the installation of the OWTS improvements for the residence at 220 Owl Creek Road
were completed pursuant to Pitkin County regulations and the construction plans and specifications
prepared by SGM.

2. Project Introduction
2.1. Approximately ten thousand (10,000) square feet of site disturbance will occur.
This includes the demolition area of the existing spa and water feature,
modifying the existing SE patio, excavation for a new pool and spa, installation
of approx 2,685 SF of snow -melted stone paver hardscape, installation of
concrete site walls covered in stone veneer, installation of heavy timber post
and beam design element, installation of outdoor gas fire pit and outdoor
kitchen/prep area with sink & mini fridge. Material staging area to be on the
shoulder of the temporary access road from the driveway circle to the
excavation site.
2.2. Vicinity Map:
2.3. The property contains an existing SFH which was constructed approx 20yrs ago.
The home is encircled with a timber fence on all sides with primarily sod
landscaping. Scope of this remodel will be completed in 1 phase. The
excavation for the pool will encroach on the existing septic holding tank which
will be permitted for relocation. We anticipate starting the project in mid Feb
with a septic relocation permit and a general excavation permit followed shortly
after by our overall building permit. This house is serviced by public utilities.
The gas and electric services will be upgraded in order to accommodate the
demands for the new mechanical systems for the pool, spa and snowmelt. A
solar array will be install on the NW area of the property to assist with offsetting
the utility demands on mechanical/electrical system, approx size of the array is
estimated to be 24'x20'.
3. Project Documentation
3.1. Adjacent properties will be notified of work and provided contact information via
the posting of the project sign. This sign will include a brief scope of work and
list contractor's contact information. Because impacts beyond the licensed ROW
area are not anticipated, specific notification and on-site meeting should not be
necessary.
3.2. A project sign will be posted on construction fencing at entry to property with
contractor's contact information.

4. Project Implementation
4.1. Dates of Construction: Construction is expected to last 5 months (Feb 10, 2014
— July 2, 2014) and will only occur on approved dates Monday through
Saturday.
4.2. Hours of Construction: Construction activity will only occur during the allowed
hours of 7:30AM and 5:30PM Mon - Friday and 9AM to 5PM on Saturdays.
4.3. Excavation will not encroach within 5' of the property line.
4.4. Project is already fenced on all sides so job site fencing will not be installed
other that an access gate into the dig area and tree protection fencing.
4.5. Project shall uphold utmost respect for public health and welfare.
4.6. Map of tree protection fencing is attached as Exhibit A.
5. Project Management
5.1. Parking for subcontractors and GF Woods Construction employees will be
located along the shoulder of the private approach road/driveway. Material
staging, temporary facilities, and waste and recycling will be located on currently
sodded areas of the property. These areas are represented on the Construction
Management Plan (Exhibit D). Contractor is requesting zero Parking Permits as
the property and parking for the property is all located on private property.
5.2. Limited parking at the site will be available. Carpooling and public transit will be
encouraged and utilized to the greatest extent practicable.
5.3. Map of Construction Management Plan is attached as Exhibit A.
6. Traffic Control
6.1. Traffic Control Supervisor will manage operations, if and when necessary.
6.2. Designated haul routes will be followed.
6.3. Maximum vehicle weight shall not exceed 10,000lbs.
6.4. Any traffic control required for deliveries will be coordinated with the Pitkin
County Engineering Department. Deliveries will not be scheduled outside of the
approved hours of construction. No more than one delivery truck can be
accommodated at any time.
6.5. A Traffic Control Plan is not included as road blockages or traffic stoppages are
not scheduled or anticipated due to the limited scope and nature of the project.
7. Pedestrian Protection
7.1. Pedestrian safety will be maintained per City Code Section 21.04.060, MUTCD
Chapter 6D, the Americans with Disability Act, and IBC Chapter 33.
8. Sediment and Erosion Control — The majority of excavated spoils will be exported
from the job site during excavation. Top soil will be stock piled near the dig site and
covered until final grading. Dump trucks will access the dig site via a temp access
road through the NW side yard. All disrupted lawn and plantings will be
repaired/replaced prior to project completion.
9. Fugitive Dust Control — N/A. Due to the fact that the dig site will be covered in a
temporary winter conditions tent we expect very little if any dust emissions to leave
the property. Should dust mitigation be necessary during the time of excavation or
backfill the site will be periodically wet with existing on-site water sources for control.
10. Emissions —All construction vehicles will be maintained and in good running order.
11. Noise Suppression
11.1. Construction noise will be limited to 80 decibels at the property line whenever
possible. Activity in excess of this limit will be governed by Item 11.2 below.
11.2. Any loud or obtrusive work will be completed during the approved hours of
9AM to 5PM (Monday through Friday only). Neighbors will be notified three
days ahead of construction initiation when demolition or other loud or obtrusive
activity is anticipated, with descriptions of the activity and equipment.
