Per the terms of the contract it states that, "At the expiration of the initial term, the contract may be extended for an additional term of two (2) additional one 1-year terms by the express written consent of both parties." If the renewal of this contract is agreeable to Jen Schumacher, M.A.,please sign this letter in the space provided below and return it, along with a current copy of your certificate of insurance, naming Pitkin County as additionally insured on the Certificate as required. Pitkin County hereby authorized the renewal of Contract#285.2016 with Jen Schumacher, M.A., for a period of twelve (12) months from contract expiration date; new expiration date is now December 31, 2017. All rates will remain the same for the extension ending December 31, 2017.
