The Provision of the Co-Responder Program (the “Contract”) dated September 1, 2018 between
the Board of County Commissioners of Pitkin County, Colorado (the “County”) and CO West
Regional Mental Health, Inc. dba Mind Springs Health, Inc. (the “Contractor”), is hereby
amended as follows:
Description of Change:
1. Original term correction: The current term, beginning September 1, 2018, is hereby
amended to expire on June 30, 2019. At the expiration of the initial term, the contract
may be extended for an additional term of one (1) year by the express written consent of
both parties.
2. Correction to term budgeted amount: Original contract value was not to exceed the sum
of one hundred seventy-two thousand ninety-seven dollars and zero cents ($172,097.00).
This change order removes Thirty-Six Thousand Two-Hundred dollars and Zero cents
($36,200.00) for a revised not to exceed sum in the amount of One-Hundred Thirty-Five
Thousand Eight-Hundred Ninety-Seven dollars and Zero cents ($135,897.00)
Reason for Change:
1. Original contract listed term expiration as 6/1/19; this should have been reflected as
6/30/19
2. Original not to exceed sum has been revised under this program
