System Records: Permit #: 88048 Date of Issuance: 8/18/88 Date of Final Approval: 9/23/88 # of Bedrooms or fixtures served by OWTS: System was originally sized for 2 bedrooms, there are currently 3 bedrooms in the home, change of ½ bath to full bath was approved by EH 1/25/89 Operational Status: According to the inspectors observation the system was functioning at the time of inspection. The system was missing original components of the dosing tank and dosing siphon. This removal was not approved by the department and may affect the ability of the system to function as designed. The field area did not show signs of failure such as surfacing effluent during the inspection. Inspector Recommendations: Continue annual maintenance or as needed Department Recommendations: Replace removed dosing tank and dosing siphon. Issuance of this OWTS Use Permit is based solely on the conditions observed and reported by the inspector to the Department on the date of the inspection(s) and on Department records at the time of permitting. The issuance of this permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail or that the system will not be subject to future enforcement action to correct non-compliant conditions. Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible failure. The legal description of the 2.5-acre property is Lot 8, Block 2, Prince Creek Subdivision. No record of the OWTS was available from Pitkin County Environmental Health Department (Parcel ID #2643-133-01-008). We believe the septic tank and soil treatment area (STA) are original, with construction of the house listed in the Pitkin County Assessor’s record as 1978. The 1000-gallon, two-compartment septic tank is located north of the new garage, on the north side of the driveway. A clean out is located approximately 10-feet west of the new garage in the landscaped railroad tie flower beds. Three feet of risers brought the manhole lids to the septic tank accessible from grade. These risers were added in September 2016. The septic tank was also pumped by B&R Septic in September 2016. There was no scum and 12”+ of suspended sludge in the first compartment of the septic tank and no scum or measurable sludge in the second compartment of the septic tank. It is not necessary to pump the septic tank at this time. We did feel something in the bottom of the septic tank when taking measurements. We were unable to tell what this item was with the septic tank full. Both the inlet and outlet tees were present. The inlet tee is PVC and the outlet tee is clay. We typically recommend that an effluent filter be added to the outlet tee of the septic tank; however, in this case the clay tee does not make installation feasible. The liquid levels in the septic tank were normal at the time of our inspection. We have no record of the STA location, size, or configuration. We believe it is likely a pipe and gravel STA located just beyond the septic tank, to the northeast. We walked the approximate STA and found no evidence of surface saturation or failure at the time of our inspection. No inspection ports were observed. This evaluation is not a guarantee of future system performance. This inspection is good for one year. This report and a completed OWTS Use Permit Application must be submitted to the Pitkin County Environmental Health Department. Because of numerous factors which may affect the operation of an OWTS, as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this report shall not be construed as a warranty by the Inspector that the system will function properly for any particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied, arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the system is having on groundwater. Please call with questions.
