Page 3 CBO Inc. 33 Four Wheel Drive Road Carbondale, CO 81623 cell) 970-309-5259 office) 970-704-0484 carla.ostberg@gmail.com June 30, 2018 Hallie Johnson johnsonhallie@hotmail.com Onsite Wastewater Treatment System (OWTS) Use Permit Inspection 77 Avalanche Trail Pitkin County, Colorado Hallie, As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit inspection at 77 Avalanche Trail, Carbondale, Colorado on June 19, 2018. The legal description of the 0.69-acre property is Lot 17, Swiss Village Tracts. The permit and record drawing were provided by Pitkin County Environmental Health Department (Parcel ID #2649-291-02-022). The subject OWTS consists of one 1000-gallon, two-compartment Copeland concrete septic tank. Effluent gravity flows to an 18’ x 55’ pipe and gravel bed. Pitkin County Health Department Permit 77120 documents this system. The system was sized to accommodate 3 bedrooms. The permit received final approval on May 3, 1978. The septic tank is located approximate 25’ east of the residence. We did not observe a clean out between the house and septic tank. Both septic tank manhole lids were accessible from grade at the time of our inspection. Liquid levels in the septic tank were normal at the time of our inspection. Both inlet and outlet tees were present. The septic tank was pumped by All Valley Resource at the time of our inspection. We typically recommend adding an effluent filter to the outlet tee; however, the tee is tucked under the lid of the tank which would make the addition of a filter not feasible unless the tee was extended. The septic tank appeared to be aging but in good condition. It should be noted that the tank has a mid- seam below the liquid level (see photo below). This seam is prone to leaking; however, at the time of our inspection the seam appeared to be holding properly. Effluent gravity flows to an 18’ x 55’ pipe and gravel soil treatment area (STA) that begins 6-feet north of the septic tank. We walked the approximate STA and found no evidence of surface saturation or failure at the time of our inspection. No inspection ports were observed. This evaluation is not a guarantee of future system performance. This inspection is good for one year. This report and a completed OWTS Use Permit Application must be submitted to the Pitkin County Environmental Health Department. Because of numerous factors which may affect the operation of an OWTS, as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this report shall not be construed as a warranty by the Inspector that the system will function properly for any particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied, arising from the inspection of the OWTS or this report.
