Main St., Suite#302 118 West 6th St., Suite 200 Aspen, CO 81611 Glenwood Springs, CO 81601 The Provision of Wildin OS Bridge Scour Repairs (the "Contract") dated September 12, 2020 between the Board of County Commissioners of Pitkin County, Colorado (the "County") and SGM (the "Contractor"), is hereby amended as follows: Description of Change: Construction phase for Wildin OS Bridge Scour Repairs. A. Project Description This project consists of providing construction support during the construction of the Wildin OS Bridge near Redstone, CO. We have prepared the following scope of work based on the information provided, services requested, and previous experience on recent bridge projects similar in nature. This scope of this work is to support the Wildin OS Bridge Rehabilitation work through construction. This includes the following scope of work: • Deck Replacement Section Additional Analysis • Hydraulics Memorandum in Support of the Floodplain Permit • Nationwide 3 Permit (Including Archaeological Assessment) • Weekly One-hour Site visits for the Duration of Construction (6 Weeks Assumed) • Submittal Reviews B. Work Plan Alternate Deck Replacement Section The Contractor has proposed value engineer alternatives for the deck replacement. SGM will provide engineering necessary to determine the structural adequacy of proposed deck replacement. Hydraulics Memorandum in Support of Floodplain Permit SGM will provide a memorandum summarizing work done to analyze the hydraulics and size *Increases over$25,000 and/or 10%(single increase or accumulative)of original contract must have County Manager signature. 1 Contract#: 002.2020 Rev. 2018.06.13 btf Budget Line Item#: 11836360.544500.10098 the scour protection. This memorandum will be included in the floodplain permit application. Nationwide 3 Permit SGM will complete the Nationwide 3 Permit. SGM will bring on Flattops Archaeological Consultants to do the cultural assessment report. Site Visits During Construction SGM will provide construction observation during the duration of construction. Five site visits are assumed. Submittal Reviews SGM will review any submittals provided by the Contractor for conformance with the Construction Documents. C. Deliverables o Nationwide 3 Permit o Hydraulics Memorandum o Alternative Deck Detail Plan Sheet o Submittal Reviews o Construction Observation Daily Diaries D. Schedule • Work will be delivered as needed on the Contractors construction timeline. E. Exclusions and Limitations • Six site visits are assumed in this scope. • Two submittals are assumed. To request paper or electronic copies from Pitkin County To request delivery from us of paper or electronic copies of the notices and disclosures previously provided by us to you electronically, you should request those documents from Pitkin County under the Colorado Open Records Act by contacting the Pitkin County custodian who sent you the document for signature. To withdraw your consent with Pitkin County To inform us that you no longer want to receive future notices and disclosures in electronic format you may: i. decline to sign a document from within your DocuSign account, and on the subsequent page, select the check-box indicating you wish to withdraw your consent, or you may; ii. send us an e-mail to Helpdesk@provelocity.com and in the body of such a request, you must state your e-mail, full name, Postal Address, telephone number, and account number.
