Main St., Suite#302 5770 East 77th Avenue Aspen, CO 81611 Commerce City, CO 80022 The Pitkin County Contract for the Purchase of Goods for the Superior DT74J Road Broom Purchase (the "Contract") dated April 15, 2021 between the Board of County Commissioners of Pitkin County, Colorado (the "County") and Faris Machinery (the "Contractor"), is hereby amended as follows: Description of Change: 1. The County agrees to the terms of the attached September 1, 2021 Proposal("Attachment A") for the purchase of a Superior DT74J Road Broom with a John Deere 74 hp Tier 4F diesel engine, together with all options, warranties and shipping charges, to replace the road broom previously purchased and delivered under County Contract 082.21 A. 2. Additional funding of Sixteen Thousand Nine Hundred Forty dollars and Zero cents ($16,940.00) is allocated to the purchase. 3. The contract term is extended to October 31, 2021 to allow for new purchase order,trade- in of previously delivered equipment, and delivery of new equipment. Reason for Change: Upon receipt of the road broom purchased under County Contract 082.21 A, it was discovered that the wrong product had been delivered. By this Change Order, the County agrees to the revised Contractor Proposal quoting the terms for the purchase and delivery of the Superior DT74J Road Broom, net the trade-in value of the previously purchased Road Broom.
