The Provision of Standby Power Generator Installation at Health and Human Services
Building (the "Contract") dated July 9, 2021 between the Board of County Commissioners of
Pitkin County, Colorado (the "County") and Smith Power Products, Inc. (the "Contractor"), is
hereby amended as follows:
Description of Change:
1. Extension of original Contract term through April 30, 2022 for additional scope (see
attached) and to allow for re-seeding of disturbed landscaping within project area once
snow has melted in Spring of 2022 as required by the original Contract document.
2. Increase to project funding in the amount not to exceed Nine-Thousand Seven-Hundred
and Forty-Four dollars and Zero cents ($9,744.00), billable only as incurred and as
approved by Pitkin County Project Lead and/or Project Engineer, for work that was not
anticipated in the original scope of the project. Contractor's cost estimate for repairs to
the sidewalk areas undermined during project are included as Attachment A, inclusive of
contingency funding in the amount not to exceed Five-Hundred dollars and Zero cents
($500.00), billable only if approved in advance by Project Lead. Flow-fill is anticipated to
begin, as scheduled, on December 17th, 2021. Preparation for flow fill must be observed
by SGM, Inc. and substantial completion will not be established until SGM, Inc. has
signed off on all project deliverables for acceptance.
Contractor and Project Lead will coordinate reseeding and any outstanding items once
snow recedes in Spring of 2022.
Project shall not be considered complete until Pitkin County has accepted all work,
including reseeding of the project area. A closeout punch list has been requested from
Contractor and shall be made part of this Contract upon receipt and approval of contents.

Change (add or delete) the following work to the original contract:
Install flow fill concrete backfilling under undermined portion of sidewalk at the HHS as per
recommendations from Project Engineer. Flow fill concrete will be pumped into the void under the
Slab and on top of compacted gravel backfill. Earth surrounding the fill area will be thawed and
Kept above freezing for a minimum of two days after pour.
Change the original contract amount by: $9,744.00

To request paper or electronic copies from Pitkin County
To request delivery from us of paper or electronic copies of the notices and disclosures
previously provided by us to you electronically, you should request those documents from Pitkin
County under the Colorado Open Records Act by contacting the Pitkin County custodian who
sent you the document for signature.
To withdraw your consent with Pitkin County
To inform us that you no longer want to receive future notices and disclosures in electronic
format you may:
i. decline to sign a document from within your DocuSign account, and on the subsequent
page, select the check-box indicating you wish to withdraw your consent, or you may;
