The Provision of Engineering Services for Pitkin County Healthy Rivers Whitewater Park
Streamside Improvements Phase I - Structural and Civil under Task Order# 051.17 F, as
amended, (the "Task Order") entered into on June 20, 2019 pursuant to the Master Service
Agreement for Provision of On-Call Engineering Services (the "Contract") dated January 18,
2017 between the Board of County Commissioners of Pitkin County, Colorado (the "County")
and SGM, Inc. (the "Contractor"), is hereby further amended as follows:
Description of Chafe:
1. As further described in the attached Contractor Design Fee Estimate ("Attachment A"),
the Contractor's Obligations are hereby amended to add: (i) design modification services
based on a revised scope of work for the Whitewater Park Streamside Improvements
plan; (ii) completion of construction documents; (iii) continued consultation,public
outreach and permitting services; and(iv) construction management services through the
Construction Phase of the Whitewater Park Improvements Project.
2. Allocation of additional funds in the amount of One Hundred Sixteen Thousand Two
Hundred Sixty-One dollars and No cents ($116,261.00)to complete the additional
Contractor Obligations.
3. Extension of Task Order End Date from 6/13/2022 to 6/13/2023 to allow for services
through end of construction.
Reason for Change: Following COVID-19 related delays to the Project, work has resumed on
revised Project design and other construction related documents for the streamside improvement
plans at the Healthy River Whitewater Park with anticipated construction this year. High
construction cost estimates led to the need for modifications in the designs and scope reductions
in order for construction to occur. Contractor fees now also include construction management
services through to completion of the Project.

Getting paper or electronic copies

Consequences of changing your mind

DocuSign account. This will indicate to us that you have withdrawn your consent to receive
required notices and disclosures electronically from us and you will no longer be able to use your
DocuSign user account to receive required notices and consents electronically from us or to sign
electronically documents from us.
All notices and disclosures will be sent to you electronically
Unless you tell us otherwise in accordance with the procedures described herein, we will provide
electronically to you through your DocuSign user account all required notices, disclosures,
authorizations, acknowledgments, and other documents that are required to be provided or made
available to you during the course of our relationship with you. To reduce the chance of you

To request paper or electronic copies from Pitkin County
To request delivery from us of paper or electronic copies of the notices and disclosures
previously provided by us to you electronically, you should request those documents from Pitkin
County under the Colorado Open Records Act by contacting the Pitkin County custodian who
sent you the document for signature.
To withdraw your consent with Pitkin County
To inform us that you no longer want to receive future notices and disclosures in electronic
format you may:
i. decline to sign a document from within your DocuSign account, and on the subsequent
page, select the check-box indicating you wish to withdraw your consent, or you may;
