The Provision of 3rd Party Commissioning for Pitkin County Courthouse (the “Contract”) dated
January 24, 2020 between the Board of County Commissioners of Pitkin County, Colorado (the
“County”) and SGM, Inc. (the “Contractor”), is hereby amended as follows:
Description of Change:
1. Added scope of work to perform the Phase 3 commissioning services for the Pitkin
County Courthouse Remodel described in the attached Contractor Task Order Scope of
Work Estimate (“Attachment A”);
2. Allocation of additional funding in the amount of $7,250.00; and
3. Extension of Task Order End Date to 12/31/2022.
Reason for Change: Additional services required for completion of Phase 3 of the Courthouse
Remodel.

1.Contractor’s Obligations.
A.Project Description
Pitkin County Courthouse Remodel Phase 3 - commissioning services for the new
HVAC equipment and controls, DHW, Lighting and lighting controls. Existing snowmelt
and controls for the West side of the building, and existing airside systems.
B.Work Plan
Commissioning Scope of Services
•Provide system prefunctional checklists (PFCs) for the contracting team to
supplement startup procedures.
•Conduct a minimum of two (2) site visits and meetings to verify equipment / systems
installations and project status.
•Review the startup documentation of all heating, ventilation and air conditioning
(HVAC), domestic hot water (DHW), and lighting and daylighting control equipment
as provided by the contractor(s).
•Review the final TAB Report for accuracy and completeness.
•Verify operation of the HVAC system and controls. The Commissioning Agent (CxA)
will conduct functional performance testing of scheduled equipment and control
systems with the project team to document operation and performance. Verification
will include confirming each sequence of operation is being met by enabling the
systems and noting how they function during each mode of operation.
•Verify operation of lighting. The CxA will perform functional performance testing of
the scheduled lighting and controls to document operation and performance, as
required.
•Verify operation of the DHW system and controls. The CxA will check temperatures
at all fixtures, mixing valve and pump operations, and recirculation system operation.

• Create a Cx Issues Log to document all deficiencies (including seasonal or deferred)
found during the MEP systems functional performance testing phase. The CxA will
document and track the status of all commissioning issues for Owner review.
• Submit a Preliminary (Preoccupancy) and/or Final (Post-occupancy) Commissioning
Report (CxR) that includes an Executive Summary, results of observed conditions,
recommended corrective actions, and all associated functional performance testing
Cx documentation for Owner review.
Items we require prior to testing:
• Building automation system (BAS) access.
• Current project schedule, to include MEP equipment installation milestones or
equivalent.
• Current MEP drawings and specifications, changes, ASIs, RFIs, etc. to reflect the
current state of the building systems.
• Project team member point-of-contact (POC) information as needed, to include
name, phone number and email address.
• Final Test and Balancing (TAB) Report after engineering review and approval.
Report should indicate all commissioned equipment and systems meet the design
configurations and are ready for functional performance testing.
C. Exclusions and Limitations
• Cx activities required by LEED® or other green-rating systems.
• Cx of the building thermal envelope.
• Cx of other systems and equipment not listed under Scope of Services above.
• Changes in Cx scope as the result of changes in project scope, change orders,
unscheduled project delays, etc.
• Additional retesting beyond the first retest.
2. Compensation and Expenses.
Total compensation and reimbursement to be paid hereunder shall not exceed the sum of:
Seven-thousand-two-hundred and fifty dollars ($7,250) for all services rendered.

Getting paper or electronic copies

To request paper or electronic copies from Pitkin County
To request delivery from us of paper or electronic copies of the notices and disclosures
previously provided by us to you electronically, you should request those documents from Pitkin
County under the Colorado Open Records Act by contacting the Pitkin County custodian who
sent you the document for signature.
To withdraw your consent with Pitkin County
To inform us that you no longer want to receive future notices and disclosures in electronic
format you may:
i. decline to sign a document from within your DocuSign account, and on the subsequent
page, select the check-box indicating you wish to withdraw your consent, or you may;
ii. send us an e-mail to Helpdesk@provelocity.com and in the body of such a request, you
must state your e-mail, full name, Postal Address, telephone number, and account
number.
