1. PARTIES
This Amendment (the “Amendment”) to the Original Contract shown on the Signature and Cover
Page for this Amendment (the “Contract”) is entered into by and between the Contractor, and the
State.
2. TERMINOLOGY
Except as specifically modified by this Amendment, all terms used in this Amendment that are
defined in the Contract shall be construed and interpreted in accordance with the Contract.
3. AMENDMENT EFFECTIVE DATE AND TERM
A. Amendment Effective Date
This Amendment shall not be valid or enforceable until the Amendment Effective Date
shown under the State Controller Signature. The State shall not be bound by any provision of
this Amendment before that Amendment Effective Date, and shall have no obligation to pay
Contractor for any Work performed or expense incurred under this Amendment either before
or after of the Amendment term shown in §3.B of this Amendment.
B. Amendment Term
The Parties’ respective performances under this Amendment and the changes to the Contract
contained herein shall commence on the Amendment Effective Date shown under the State
Controller Signature or July 1, 2022, whichever is later, and shall terminate on the
termination of the Contract or June 30, 2023, whichever is earlier.
4. PURPOSE
The Parties entered into the agreement to reduce the harms associated with drug use and to
develop alternatives to the crinimal justice system for those who suffer from substance abuse
disorder
The Parties now desire to renew for an additional term and change current Contract Maximum
Total for the following reason: to continue the services described in the Original Contract for an
additional term.
5. MODIFICATIONS
The Contract and all prior amendments thereto, if any, are modified as follows:
A. The Contract Maximum Amount table is deleted and replaced with the Current Contract
Maximum Amount table shown on the Signature and Cover Page for this Amendment.
B. The Contract Initial Contract Expiration Date on the Contract’s Signature and Cover Page
is hereby deleted and replaced with the Current Contract Expiration Date shown on the
Signature and Cover Page for this Amendment.
C. The Parties now agree to modify Exhibit B, Statement of Work, of the agreement. Exhibit
B, Statement of Work, is deleted and replaced in its entirety with Exhibit B, Statement of

Work, attached to this Amendment for the following reason: to continue the services

is deleted and replaced in its entirety with Exhibit C, Budget, attached to this Amendment
for the following reason: to increase funding for the SFY23 term.
6. LIMITS OF EFFECT AND ORDER OF PRECEDENCE
This Amendment is incorporated by reference into the Contract, and the Contract and all prior
amendments or other modifications to the Contract, if any, remain in full force and effect except
as specifically modified in this Amendment. Except for the Special Provisions contained in the
Contract, in the event of any conflict, inconsistency, variance, or contradiction between the
provisions of this Amendment and any of the provisions of the Contract or any prior modification
to the Contract, the provisions of this Amendment shall in all respects supersede, govern, and
control. The provisions of this Amendment shall only supersede, govern, and control over the
Special Provisions contained in the Contract to the extent that this Amendment specifically
modifies those Special Provisions.

STATEMENT OF WORK
To Original Contract Routing Number 2021*0627
These provisions are to be read and interpreted in conjunction with the provisions of the contract specified
above.
I. Entity Name: Pitkin County Public Health
II. Project Description: This project serves to reduce the negative health effects of drug use; improve
communication between law enforcement, Pitkin County Public Health, and community-based
organizations in Pitkin County; and to develop alternatives to the criminal justice system for those who
suffer from substance use disorder. This is a health project that will benefit the residents of Pitkin County,
Colorado. The goals of the project will be achieved by the creation and expansion of a co-responder health
program that will pair law enforcement officers with behavioral health specialists to respond to calls and
divert people with substance use disorders to treatment or needed services rather than jail. Services
received will help people to access treatment and recovery support for their substance use disorder rather
than be incarcerated, which will save money for the county and result in improved health for the residents
of Pitkin County. This project is necessary and beneficial to the State because there has been an increase in
drug overdose deaths, drug related crime and lack of treatment services throughout Pitkin County.
III. Definitions:

IV. Work Plan:
Goal #1: Improve the health, well-being and equity of all Coloradans through health promotion, prevention and access to healthcare.
Objective #1: No later than the expiration date of this contract, reduce adverse health outcomes, health
complications and incarceration rates related to injection practices, and overdose injury and death among PWUD
in Pitkin County.
Primary Activity #1 The Contractor shall build program capacity to deliver harm reduction services.
Sub-Activities #1
1. The Contractor shall employ a BH clinician and Co-response BH case manager.
2. The Contractor shall develop referral criteria for the range of behavioral health
services provided to participants.
3. The Contractor shall use behavioral health assessment tools.
4. The Contractor shall develop individual goals for program participants who engage
in Case Management or Peer Support.
5. The Contractor shall provide harm reduction resources to program participants.
6. The Contractor shall refer program participants to community resources.
7. The Contractor shall provide direct support to clients including:
a. Peer support services
b. Case management
8. The Contractor shall maintain co-responder protocols with LE.
9. The Contractor shall develop guidance for responding to referrals sent to PACT by

LE and Community partners.
10. The Contractor shall identify PACT clients with support from APCHA, SVHA,
AVH, and community agencies.
11. The Contractor shall refer PACT clients to participating community agencies.
Primary Activity #2 The Contractor shall provide harm reduction training and supplies to reduce recidivism
and the harm associated with drug use.
Sub-Activities #2 1. The Contractor shall use evidence based harm reduction training.
2. The Contractor shall implement training processes for training participants.
3. The Contractor shall provide harm reduction supplies to local organizations and/or
individuals.
4. The Contractor shall provide training on overdose prevention.
5. The Contractor shall collect training participant data.
6. The Contractor shall track training completion metrics for each identified training.
7. The Contractor shall facilitate a minimum of one (1) behavioral health training for
PACT LE and BH.
8. The Contractor shall facilitate a minimum of one (1) mental health first aid training
for LE and BH staff.
Primary Activity #3 The Contractor shall implement data collection practices to identify substance use hot
spots in Pitkin County.
Sub-Activities #3
1. The Contractor shall work with a data consultant to improve data collection
measurements.
2. The Contractor shall refine data management procedures.
3. The Contractor shall perform data sharing between law enforcement, behavioral
health and harm reduction stakeholders in Pitkin County.
4. The Contractor shall track data sharing between law enforcement, behavioral
health, and harm reduction stakeholders in Pitkin County.
5. The Contractor shall work with a data consultant to monitor quality assurance
metrics.
6. The Contractor shall inform co-responders on substance use data findings to inform
local efforts.
Primary Activity #4 The Contractor shall mobilize a multidisciplinary committee of stakeholders to support
project efforts.

1. The Contractor shall coordinate a multidisciplinary committee of stakeholders
quarterly.
2. The Contractor shall facilitate meetings of a multidisciplinary committee of
stakeholders.
3. The Contractor shall refine and implement a communication plan with input from a
multidisciplinary committee of stakeholders.
4. The Contractor shall develop data sharing agreements with the multidisciplinary
committee of stakeholders.
Primary Activity #5 The Contractor shall develop an evaluation strategy that monitors changes in substance
use behaviors and associated health outcomes.
Sub-Activities #5
1. The Contractor shall implement data collection measurements to monitor health
outcomes.
2. The Contractor shall collect data on program referrals and the program referrals’
efficacy where possible.
3. The Contractor shall conduct quality assurance activities to review services on a
quarterly basis.

4. The Contractor shall engage with PWUD to elicit feedback in order to improve
service delivery.
5. The Contractor shall use mental health first aid evaluation tools.
6. The Contractor shall use behavioral health training evaluation tools.
7. The Contractor shall obtain feedback from program participants to guide
improvements in service delivery.
8. The Contractor shall disseminate evaluation results to community stakeholders.
Primary Activity #6 The Contractor shall complete a CDPHE approved progress report.

Standards and
Requirements
1. The content of electronic documents located on CDPHE and non-CDPHE websites
and information contained on CDPHE and non-CDPHE websites may be updated
periodically during the contract term. The Contractor shall monitor documents and
website content for updates and comply with all updates.
2. CDPHE will provide a monthly and annual progress report template which shall
include:
a. Primary and sub- activity progress summary
b. The most current data available including the demographics and number of
people participating in interventions
c. Process and outcome evaluation data as specified in the grantee’s evaluation
plan
d. Documentation of any activities between law enforcement, LPHAs and
community-based organizations
e. Challenges and successes encountered
f. Any requests for technical assistance or training.
3. The Contractor shall follow information at
https://www.colorado.gov/pacific/cdphe/opioid-prevention. This website is
incorporated and made part of this contract by reference.
4. The Contractor shall facilitate regular meetings with stakeholders
throughout planning and implementation of the approved activities.
5. CDPHE will provide technical assistance to the Contractor regarding selected
overdose prevention activities which may include: provider education;
community education; community-level interventions; targeted naloxone
distribution; data; evaluating local efforts to implement effective strategies
6. The Contractor shall work with all stakeholders to enhance community capacity
to implement approved activities.
7. The Contractor shall attend CDPHE approved training and meetings.
8. CDPHE will provide the Contractor with technical assistance for evaluation
planning and implementation.
9. The Contractor shall submit all deliverables to the Harm Reduction Grant Program
Coordinator via email.
Expected Results of

1. Reduce health risks associated with drug use for individuals at risk of entering
the criminal justice system or individuals in the criminal justice system through
implementation of harm reduction strategies.
2. Increase harm reduction efforts between law enforcement agencies, public
health agencies, and community-based organizations.

Measurement of

V. Monitoring:
CDPHE’s monitoring of this contract for compliance with performance requirements will be conducted
throughout the contract period by the Harm Reduction Grant Program Coordinator. Methods used will
include a review of documentation determined by CDPHE to be reflective of performance to include
progress reports and other fiscal and programmatic documentation as applicable. The Contractor’s
performance will be evaluated at set intervals and communicated to the Contractor. A Final Contractor
Performance Evaluation will be conducted at the end of the life of the contract.
VI. Resolution of Non-Compliance:
The Contractor will be notified in writing within 15 calendar days of discovery of a compliance issue.
Within 30 calendar days of discovery, the Contractor and the State will collaborate, when appropriate, to
determine the action(s) necessary to rectify the compliance issue and determine when the action(s) must be
completed. The action(s) and timeline for completion will be documented in writing and agreed to by both
parties. If extenuating circumstances arise that requires an extension to the timeline, the Contractor must
email a request to the Harm Reduction Grant Program Coordinator and receive approval for a new due
date. The State will oversee the completion/implementation of the action(s) to ensure timelines are met and
the issue(s) is resolved. If the Contractor demonstrates inaction or disregard for the agreed upon
compliance resolution plan, the State may exercise its rights under the provisions of this contract.

Funds to support a Law enforcement officer assigned to the Pitkin Area
Co-responder team, facilitate trainings for law enforcement, support
co-responder program and provide coordination among local law
enforcement.
One Full time Mental Health Clinician (Salary including fringe: $88,392)
1 full time Case Manager (Salary including fringe: $80,772) , 10% time
for Supervisor (Salary including fringe: $13,462). Incidentals for the
purpose of grant activity completion (professional liability, minor
equipment)= $3,212.00. Full time staff will provide behavioral health
services for program participants through the Pitkin Area Co-responder
team programs. Full time staff will support program participants,
provide referrals and data collection measurements.
