COMMISSIONERS (“BOCC”) OF PITKIN COUNTY, COLORADO APPROVING, AN INDIVIDUAL GRANT AGREEMENT WITH ROCKY MOUNTAIN HEALTH PLANS IN THE AMOUNT OF $10,000
RESOLUTION NO. 029-2023
RECITALS
WHEREAS, Pursuant to Section 2.8.3 (Actions) of the Pitkin County Home Rule Charter (“HRC”) official action by formal resolution shall be required for all actions of the Board not requiring ordinance power on matters of significant importance affecting citizens, and;
WHEREAS, Pitkin County Human Services believes it is in the best interest of the residents
of Pitkin County to promote the health of local communities and populations that are Medicaid eligible by helping improve their health and life outcomes, and;
WHEREAS, by accepting these grant funds, Pitkin County Human Services will provide sporting and recreational equipment, the cost of membership and admission fees for
Medicaid eligible individuals.
WHEREAS, The BOCC finds that it is in the best interests of the citizens of Pitkin County to approve this Resolution.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Pitkin County, Colorado that it hereby adopts a Resolution of the Board of County Commissioners authorizing the Pitkin County Department of Human Services to accept grant funds in the amount of $10,000.00 from Rocky Mountain Health plans and authorizes
the Chair to sign the Resolution and upon the satisfaction of the County Attorney as to form, execute any other associated documents necessary to complete this matter.

Hello Samuel,
Rocky Mountain Health Plans is grateful for all that your organization is doing to address Social
Determinants of Health. Community Investment funding supplements organizations that focus
on such disparities within their own community. Upon review of your proposal, Rocky Mountain
Health Plans is offering $10,000 to assist the work you do in your community. Through your
organization's commitment to address social determinants of health, we will strive to make a
change together.
Payments for this funding will be processed in June 2023. If further information is needed, feel
