Changes must be approved by this Department and the engineer prior to construction. This system will consist of a 1500 gallon 2 compartment tank followed by a 500 gallon dosing tank. All risers must be accessible from grade, and effluent filter will be installed in the outlet of the 1St tank. Inside the dosing tank alternating duplex pumps will be installed to pressure dose the field area. The field area will consist of 2 trenches, each will have a minimum of 3' of ASTM 33 sand. A total of 47 infiltrator units will be utilized. Pressure dosing will be done via 1 %2" schedule 40 pipe, drilled 3/16" holes facing upwards every 3.5' and holes drilled downwards at the ends for drainage. Observation ports must be installed at the ends of each trench. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Issuance of this permit does not imply compliance with Pitkin County building and/or land use regulations, nor guarantee issuance of building and/or land use permits. We previously conducted a subsoil study for design of foundations at the site for the residence and presented our findings in a report dated August 31, 2016, Project No. 16-7-264. The profile pits are located in the proposed septic disposal areas downhill, west and south of the residence as shown on Figure 1. The ground surface slope is steep down to the southwest and covered with thick scrub brush. The profile pits had been excavated with a backhoe prior to our site visit. Profile Pit 1 was identified as being in the west disposal area and Profile Pit 2 was identified as being in the south disposal area. The soil profiles exposed in Profile Pits 1 and 2, shown on Figure 2, below about 1 foot of topsoil and about 3 feet of red -brown sandy loam with gravel consisted of brown very gravelly sand with cobbles and small boulders to the pit depths of 7 and 8 feet. The CDPHE WQCC Regulation 43 Table 10.1 indicates the soil type is 0 due to the rock content being greater than 35%. The results of gradation analyses performed on samples of soils (minus 3 to 5 -inch fraction) obtained from the pits are presented on Figure 4. The legal description of the property is Lot R-101, Starwood Sixteen Block 16, Starwood Subdivision. The permit was provided by Pitkin County Environmental Health Department (Parcel ID #2643-354-01- 009), along with an illegible drawing. Individual Sewage Disposal Permit No. 82035 documents this system. An accurate record drawing was provided by you, in advance of the inspection. The subject OWTS consists of one 1200 -gallon, Jet® Aeration treatment plant, followed by an "upflow filter". Effluent gravity flows to a distribution box and then to two deep gravel trenches, each 90' long and 3 -feet wide with 3 -feet of gravel below the distribution laterals. Two of the Jet® tank manhole lids are located under the deck area. The lids are accessible once the deck boards are removed. We recommend creating hatches or another similar access to allow the lids to be easily located for future pumping. The inlet pipe coming into the septic tank is cast iron and the inlet tee was missing. The Jet® aeration device had been removed and is no longer a functioning part of the system. The outlet baffle had deteriorated and fallen off. The "tube settler" was still in the third compartment, but has been dislodged from its original position in the septic tank. The "upflow filter" had an inlet tee and an outlet tee that sits parallel to the top and bottom of the septic tank, rather than sitting perpendicular. We believe the "upflow filter" chamber is filled with "media" as part of the intended functioning, or filtering process. It is unclear if the piping was modified over time, as schematics we found online show influent coming into the bottom of the filter, which is not the case here. Repairs were made to the JetO tank by D&D Septic Services on July 10, 2015. Both inlet and outlet tees were installed. Photos of the repair are included below. The distribution box was not accessible from grade at the time of the inspection. We walked the very steep area down to where the trenches are located. There was one inspection port located at the end of the west trench. The port did not have a cap. We recommend replacing the slip-on cap. The entire area is covered with high grasses, and there was no evidence of surface saturation or failure at the time of the inspection. While the system is functioning and there are no evident signs of failure, this OWTS is aging and the cast iron pipe entering the septic tank is prone to deterioration. The Jet® aeration plant is no longer functioning as once intended, with the interior components either removed or not in their intended position. The "upflow filter" may have been modified, and this component is not typical of any we commonly see in Pitkin County. C 0 This evaluation is not a guarantee of future system performance. This inspection is good for one year. Recommendations: Create better accessibility for manhole lids under deck. Replace slip on cap on west inspection port. Subject: OWTS Construction Approval Letter 0026.2015.POWT The proposed OWTS field design for 602 Eppley drive in Pitkin County, Colorado has been installed per the approved and permitted plans permit number . The installation contractor was Dirt Devil Excavation, Inc. , 3270 County Road 113, Carbondale, CO. The chambers were trenched down to the tested soil elevation and installed on native material that was reviewed prior to the chambers being installed. The soils were consistent through the chamber trenches as per what was described within the geotechnical report prepared by HP Kumar, Inc. job no. 16-7-264. Some of the previous system was encountered and excavated to provide the required 3’ depth of sand. The fields are located as shown on the following page in the same location as the design. The two trenches equal to the required 47 units shown on the approved plans. The chamber 1.5” pipes were run through the upper port on the end caps and through the chambers with the piping being attached to the top of the chambers per dosing requirements. The inspection ports were installed on both ends of the chamber runs. The installed ports were brought to above grade as required. The septic tank (1769 gallon) meets the minimum sizing requirements of 1500 gallons. The dosing tank was 500 gallons and had a duplex pumping system installed. The control box is located in the crawl space and can be heard in several rooms when the alarm sounds but is also connected to a monitored alarm system. The dosing pump was found to be operational through a pumping test, which included testing of the floats. The alarm system was tripped and is connected with a monitoring company. Notification was witnessed on the monitored alarm system panels. The tank lids have been raised to the proper elevation above grade and backfilling of the trench and septic tank dig have been completed. Please see attached photos of the installation.
