Operational Status: According to the inspector’s observations on site the system appeared to be functioning as
designed. Tanks were pumped as part of the inspection. Both tanks appeared to be watertight and in good working
condition. The pump was functioning properly. The field area did not show signs of failure.
Inspector Recommendations: Bring outlet side of 3000 gallon tank to grade for maintenance, clean effluent filter
annually or as needed, location distribution box and bring access to grade.
Department Recommendations: Continue maintenance as needed.
Issuance of this OWTS Use Permit is based solely on the conditions observed and reported by the inspector to the Department on the date of the inspection(s) and
on Department records at the time of permitting. The issuance of this permit does not constitute a guarantee, warranty, or representation by the Department
that the system will operate properly or will not fail or that the system will not be subject to future enforcement action to correct non-compliant conditions.
Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible
failure.

As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit
inspection at 456 Kessler Drive, Aspen, Colorado on November 20, 2019. The legal description of the
3.83-acre property is Lot R-16, Original Starwood, Starwood Subdivision.
The permit and record drawing were provided by Pitkin County Environmental Health Department (Parcel
ID #2643-262-01-004). The subject OWTS consists of one 3000-gallon, single-compartment concrete
septic tank followed by a 2000-gallon, two-compartment septic tank with a pumping system in the second
compartment. Effluent is pumped to a distribution box and then to four gravelless chamber beds, each
with 6 rows of 8 ‘Quick 4’ Infiltrator® chambers for a total of 192 chambers and 2304 square feet of
infiltrative area. Individual Sewage Disposal System Permit 07077 documents this system. The system
was sized to accommodate 8 bedrooms. The permit received final approval on November 25, 2008.
The septic tanks are located north of the residence. The 3000-gallon single compartment septic tank has
only access to the inlet side. We recommend bringing the outlet side manhole lid to grade for future
maintenance. The inlet tee was present and angled sideways. This should not affect performance of the
system. Levels in the septic tank appeared normal at the time of our inspection.
Both inlet and outlet lids were present on the 2000-gallon septic tank. The inlet tee was present and
angled downward slightly. An Orenco® pumping system was present in the second compartment. The
effluent filter was dirty, and cleaning was recommended. We also recommended the septic tanks be
pumped. Pumping was conducted by Altitude Septic on December 7, 2019. The effluent filter was also
cleaned.
Both septic tanks have a mid-seam below the liquid level. At the time of our inspection, there were no
issues with the seams.
The distribution box was not accessible from grade and therefore could not be inspected. We
recommend the distribution box be located and access brought to grade for future inspection and
maintenance. We observed one inspection port. It was dry. There was snow cover over the STA
limiting our observations. We walked the STA and found no evidence of surface saturation or failure at
the time of our inspection. This evaluation is not a guarantee of future system performance. This
inspection is good for one year.
Recommendations:
• Bring outlet side of 3000-gallon septic tank to grade for future maintenance and inspection. The
septic tank is 4-feet below grade.
• Clean effluent filter annually, or as needed.
• Locate distribution box and bring access to grade for future inspection and maintenance.
A completed OWTS Use Permit Application must be submitted to the Pitkin County Environmental
Health Department. Applications must be submitted to Pitkin County Environmental Health Department

Within the application, you must attach the Inspection Form where “Pitkin County Inspection Form” is
requested, the OWTS Inspection Packet where “Site Plan” is requested, and Floor Plans where “Floor Plans”
are requested. Floor plans may be hand drawn and must represent number of bedrooms or potential
bedrooms in the residence.

Disclaimer: This inspection is intended only as an evaluation of the present condition of the OWTS
based upon what was observed and the Licensed Inspector’s expertise in onsite wastewater technology.
The Inspector has not been retained to warrant or guarantee the proper functioning of the system for any
period of time in the future. Because of numerous factors which may affect the operation of an OWTS,
as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this
report shall not be construed as a warranty by the Inspector that the system will function properly for any
particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied,
arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.

Septic permit approved per compliance with the engineer design and specifications dated August 16 , 2007,
Project # 1359. Any changes must be approved by this department and the design engineer prior to them being made.
The existing tank and field are to be abandoned. Minimum horizontal distances between components of the system and
physical features shall conform to the Pitkin County ISDS regulations.
Design is for one 3000 gallon single compartment concrete septic tank followed by one 2000 gallon two compartment
concrete septic tank with a screened dosing siphon. Effluent is dosed to a concrete, baffled distribution box with equal
distribution to four gravelless chamber beds, each 6 rows of 8 chambers, for a total of 192 Quick4 chambers. Bed

This permit is expressly conditioned on the property owner providing for the regular inspection, cleaning and
maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations.
This department does not endorse any brand of products. This permit must be kept on-site during installation. The
engineer must do a final inspection of the installation and submit an as -built letter to this department. This department
must also be called for an inspection with a minimum of 48 hours advanced notice.
Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly
encouraged

The fee for an ISDS application is $623 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $623 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cistems, drain
tiles, irrigation ditches, lakes, water courses, streams, floodplains, dry gulches, and existing septic systems. I hereby certify that
any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of any
other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all approvals

ALL SERVICE septic, LLC performed installation observations on November 2°d and December 5"'
2007, and on March 27t` 2008 for the subject property. This revision letter is based on corrected GPS
coordinates for the OWTS.
The installation 'included a 3000 -gallon, single -compartment concrete septic tank, followed by a
2000 -gallon, two-compartment septic tank with the addition of a Biotube pump vault in the second
compartment.
The drain field consisted of 4 chamber -beds, each bed having 6 rows of 8 `Quick -4' chambers for a
total of 192 `Quick -4' chambers. A concrete distribution box served effluent to the chamber -beds.
The pump, alarml, and control panel are wired and working.
The OWTS was installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWTS.
LIMITS:
The observations were based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWTS construction must be according to the county regulations. The installer should have
documented and demonstrated knowledge of the requirements and regulations of the county in which
they are workinj

2007, and on March 27'" 2008 for the subject property.
The installation included a 3000 -gallon, single -compartment concrete septic tank, followed by a
2000 -gallon, two-compartment septic tank with the addition of a Riotube pump vault in the second
compartment.
The drain field (consisted of 4 chamber -beds, each bed having 6 rows of 8 `Quick4' chambers for a
total of 192 `Quick -4' chambers. A concrete distribution box served effluent to the chamber -beds.
The pump, alarm, and control panel are wired and working.
The OWS was installed according to specifications. This observation is not a guarantee of
workmanship apdfor parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to theJOWS.
LIMITS:
The observations were based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWS construction must be according to the county regulations. The installer should have
documented an demonstrated knowledge of the requirements and regulations of the county in which
they are working.

ALL SERVICE septic, LLC performed a subsurface investigation and an onsite wastewater system
OWS) design for the subject residence. The property is located outside of Aspen Colorado, in an
area where OWS are necessary. Water is provided by the local water district.
Due to the existing sewer line elevations a pump is required to dose effluent to the proposed drain
field. A P-30 Biotube Pump Vault with control panel is recommended.
The pump vault must be placed in the second compartment of the 2000 -gallon, two-compartment
concrete septic t4nk. A 2 -inch PVC or Purecore discharge line must be installed from the pump to the
distribution box, A 20 - amp circuit is necessary for the pump and floats and a 15 -amp circuit is
necessary for the control panel. The pump must be capable of the specifications on the pump curve
and detail provided. All other recommendations from the Pitkin County Permit (#07077), and from
our design dated August 16th 2007 must be followed.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.
