This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This permit is to replace a failed septic tank for the main residence. The existing septic tank shall be abandoned properly and
evidence of proper abandonment shall be submitted to this office as part of the final approval of this permit. Sewage will flow
via gravity to a new, minimum 1000-gallon two -compartment septic tank with an effluent filter installed in the second
compartment. Effluent will then flow to the existing seepage pits. The contractor shall confirm the existing distribution valve is
operating as designed and replace it if it is not working correctly and confirm the seepage pits are not saturated and still
receiving effluent. The new tank shall be a minimum of 5-feet to the residence and 50-feet to any well.
This Permit must be kept on -site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Issuance of this
permit does not imply compliance with Pitkin County building and/or land use regulations, nor guarantee issuance of building and/or land use permits.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is nereoy
given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved by EH before final approval of the system will be issued.

Septic Tank Installation Observations

CBO Inc. observed the installation of the septic tank on June 19, 2017 for the subject property. You, as
the homeowner, installed the septic tank.
The existing onsite wastewater treatment system (OWTS) consisted of one 1000-gallon, single-
compartment concrete septic tank and a seepage pit (aka drywell) which were both original. In 1987 a
new seepage pit was constructed and connected to the existing system with a gate valve. The new
seepage pit is described as 11’ x 12’ x “3 rings” deep. Individual Sewage Disposal System Permit 87030
documents the new seepage pit. The system was sized to accommodate 3 bedrooms. The permit
received final approval on July 2, 1987.
The sewer line from the house to the septic tank was replaced. The existing septic tank was pumped
and the sides and top collapsed and the remaining void filled with onsite materials from the new septic
tank installation.
A new 1500-gallon, two-compartment Infiltrator® poly septic tank with an Orenco® Effluent Filter on the
outlet tee was installed. This filter must be cleaned annually, or as needed. A new Bull Run Valve
was also installed, which will allow alternating between the seepage pits. When alternating systems
are used, the property owner is responsible for rotation and tracking of the rotation. We
recommend alternating on an annual basis.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.
