Operational Status: After repairs were made during the inspection the system appeared to functioning as designed.
Both tanks with in good working order, the outlet tee and filter were repaired as part of the inspection. The distribution
box was found damaged and unlevel so it was also replaced as part of the inspection. The field area did not show any
signs of failure such as surfacing effluent.

Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible
failure.

To Whom It May Concern,
Altitude Septic was hired to perform a septic inspection at 6067 Woody Creek Rd. The septic
system consists of a 2000-gallon single compartment concrete septic tank, a 1000-gallon two
compartment concrete tank, and a distribution box followed by four tranches of 22 infiltrators
in each trench. The first tank is the 2000-gallon and is in good condition with the inlet and
outlet tees in place. The tank has two feet of risers to bring the lids to the surface. The second
tank is the 1000-gallon septic tank and is in good condition with both the inlet and outlet tees
in place. This tank has two feet of risers to bring the lids to the surface and new lids were
needed, so we installed them. At the time of this inspection, both tanks were pumped, and the
system appears to be functioning as it should.
There were some repairs made to this system. The outlet filter was missing its handle and was
also “cocked” up so that it was not able to be removed for proper cleaning. When we dug up
the outlet line to fix this settling issue, we found that the distribution box was only one foot
away from the tank and it had also settled. Upon inspection of the d-box, we found that the
end of the box was rotted out and it needed to be replaced. We returned with a new
distribution box, installed it, installed risers to bring the lid to the surface, brought the settled
line back into position, and installed the new filter.
