Onsite Wastewater Treatment System (OWTS) Use Permit Inspection

As requested, CBO Inc. performed an Onsite W astewater Treatment System (OWTS) Use Permit
inspection at 580 Johnson Drive (aka 678 Johnson Drive), Aspen, Colorado on December 1, 2016. The
legal description of the 5.0-acre property is Lot R-69, Block 10, Starwood Ten, Starwood Subdivision.
The permit and record drawing was provided by Pitkin County Environmental Health Department (Parcel
ID #2643-352-02-002). The subject OWTS consists of one 1000-gallon, two-compartment concrete
septic tank. Effluent gravity flows to a 15’ x 85’ pipe and gravel seepage bed. Pitkin County Health
Department Permit Number 720302 documents this system. The system was sized to accommodate 3
bedrooms. The permit received final approval on October 21, 1975.
The septic tank is located to the west and between the two structures on the property. Both inlet and
outlet manhole lids are accessible under wooden pallets. There is evidence of electrical wiring in the inlet
side of the septic tank. It does not appear be connected. The inlet tee was present, and the outlet tee is
tucked under the lid. We took a photo and found the outlet tee to be broken above the invert. This
should not inhibit the functioning of the tee. The location of the tee would make a repair difficult, and the
PVC is fragile and attempts to repair could lead to further breakage. Levels in the septic tank appeared
to be normal at the time of the inspection. B&R Septic pumped the septic tank subsequent to the
inspection.
The soil treatment area (STA) is located south of the septic tank. We walked the STA and found no
evidence of surface saturation or failure at the time of our inspection. We did not observe any inspection
ports in the STA. This evaluation is not a guarantee of future system performance. This inspection is
good for one year.
While this system is aging, we did not see any signs of malfunction.
This report and a completed OWTS Use Permit Application must be submitted to the Pitkin County
Environmental Health Department. Applications may be submitted to Pitkin County Environmental Health

period of time in the future. Because of numerous factors which may affect the operation of an OWTS,
as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this
report shall not be construed as a warranty by the Inspector that the system will function properly for any
particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied,
arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.
